Feeding Background History: Zane is a 4 year, 5 month­ old male referred by his pediatrician for a feeding evaluation due to concerns with feeding. His mother, Mrs. Hartstield, reports Zane began solids at 6 months while breastmilk remained the primary source of nutrition. Breastfeeding ceased at approximately 16 months of age. Subsequent to weaning, the patient demonstrated significant food regression, eliminating previously accepted items. Mother estimates prior food repertoire included around 40 foods. At present, Zane's primary source of nutrition is Pediasure. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills.
Feeding Medical History: Per parent report, medical history is not remarkable. Born via c-section at 40 weeks. Per parent's report, fetal heart rate decelerations were noted near delivery. No complications were reported postnatally. Zane is diagnosed with an adolescent heart murmur.

Feeding Diagnosis/Impressions: Zane is a 4 year, 5 month­ old male referred by his pediatrician for a feeding evaluation due to concerns with feeding. His mother, Mrs. Hartstield, reports Zane began solids at 6 months while breastmilk remained the primary source of nutrition. Breastfeeding ceased at approximately 16 months of age. Subsequent to weaning, the patient demonstrated significant food regression, eliminating previously accepted items. Mother estimates prior food repertoire included around 40 foods. His PFD is characterized by impaired oral intake that is not age-appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures), and psychosocial (selective/restrictive eating). FOOD REPERTOIRE: Zane's food inventory includes fewer than 5 accepted foods and 3 liquids. Accepted proteins none. Fruits include smoothies. Vegetables include smoothies. Starches include Cheerios and fruit gummy snacks. Liquids include Pediasure and water. NORM REFERENCED ASSESSMENT: Mrs. Hartstield completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver report questionnaire for children aged 6 months to 7 years. Zane received an overall rating of “high concern". These concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Zane was seated and self-fed his preferred food items utilizing his fingers (Cheerios and gummies). Non­preferred foods offered were strawberries and cookies. Zane verbally refused trials of non-preferred. With clinician support, Zane tolerated non-preferred at the level of his head and nose. SWALLOW/ORAL MOTOR EVALUATION: Zane demonstrated a rotary chewing pattern when consuming preferred food items, with some instances of open-mouth chewing. Anterior­ posterior bolus transit was timely, and no clinical signs or symptoms of aspiration or penetration were observed. His intake rate was slightly elevated, though manageable. Mother does not report unexplained fevers or history of aspiration pneumonia, however she reported consistent gagging and/or throwing up during mealtimes. Zane’s pharyngeal/esophageal swallow function should be monitored by his treating clinician to ensure safety and determine if further testing is warranted. Based on foods trialed and behaviors observed, Zane’s oral motor skills appear functional at this time. Continued monitoring is recommended throughout treatment to ensure safe feeding and swallowing. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 minutes to address Zane’s CHRONIC PFD and ORAL DYSPHAGIA. Zane is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. In addition, food chaining strategies should be implemented to expand Zane’s food repertoire by building on existing preferred foods. Without intervention, Zane may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Mrs. Hartstield will be responsible for implementing the recommended Home Management Program to reinforce therapeutic strategies and support progress between sessions.

Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative for this initial therapy session. A primary focus was placed on establishing a positive therapeutic rapport. Therapeutic intervention targeted feeding and swallowing skills, utilizing patient-preferred (Cheerios) and non-preferred (seaweed) food items brought from home. To promote oral awareness and confidence, oral motor stimulation and desensitization exercises were introduced; the patient participated in these exercises in 5 out of 10 presented opportunities (50%), demonstrating a foundational tolerance for oral sensory input. Efforts were made to improve acceptance and exploration of non-preferred foods to increase consumption across food groups. The patient interacted with the non-preferred food (seaweed) using his whole hands in 3 out of 10 opportunities (30%) and subsequently brought the food to his face to smell and touch in 3 out of 10 opportunities (30%). To facilitate positive interactions with new foods, the patient worked on identifying and describing sensory features, achieving 30% accuracy with multisensory cueing.
