Assessment of Goals/Response to Treatment: Sophia presented as friendly and cooperative, demonstrating good engagement in all therapeutic activities. The clinician provided parent counseling regarding a home management program designed to facilitate the generalization of skills. The parent demonstrated a clear understanding of the strategies discussed and verbally committed to incorporating these activities into the family's daily and weekly routines to support functional communication in the home environment.The primary focus of this session was to establish baseline data for the patient's articulation and phonological objectives. An informal assessment of the patient's conversational speech revealed multiple phonological processes impacting overall intelligibility. Baseline data indicated the presence of Stopping: less than 90%, Fronting: less than 90%, Gliding: less than 90%, Cluster Reduction: less than 90%. These patterns significantly reduce the patient's ability to be understood by unfamiliar listeners.Further baseline data was collected for specific phoneme production goals. For production in isolation, the following data was collected: /z/: 10%, /sh/: 30%, /ch/: 10%, /k/: 10%, /g/: 20%, /v/: 40%, /l/: 20% . Allergies to treenuts and peanuts. Severe eczema. Takes Dupixent injection every 4 weeks in thigh.
Crawling (don’t remember exactly, maybe 7 months old?), sitting up (don’t remember, maybe around 7-8 months old), standing up (like 9 months old), feeding self (around 7 months old), dressing self (like 3.5 years old), potty training (3.5 years old), first word (maybe at 9 or 10 months old), word combinations (maybe at 1.5-2 years old).
Wakes up a few times a night
She attends public school, in kinder
No other languages
Vision; she wears glasses, last hearing was in 2023, and she passed, has had screenings at school and no concerns
Her teacher has a concern that she cannot understand certain sounds that she says, there are some words that she struggles to pronounce and even I can’t understand sometimes, for example tomorrow, remember
