Speech Background History: Tanner is a 9 year 10 month old male who was referred for a speech and language evaluation by his pediatrician and mother’s concern regarding language skills and speech production. Tanner was accompanied to the evaluation by his mother who served as the informant. Tanner has had a history of receiving speech therapy since he was 3 years old, and stopped services because the family relocated to El Paso. According to his mother, Tanner was diagnosed with autism spectrum disorder and attention deficit hyperactivity disorder when he was 6 years old. Mother stated Tanner is not on any medication and has learned to self-regulate. Per parent report, Tanner has regressed in his speech production since ceasing speech therapy. He is currently enrolled in a PASS Unit at Carrasco Elementary in the 3rd grade, and received speech therapy twice a week for 30 minutes. At this time, his mother is not concerned with Tanner’s language as he made great gains while attending speech therapy in Colorado. However, mother is concerned that Tanner’s speech production difficulties had given him a type of “anxiety” when speaking with new communication partners, and recently his teacher has stated some difficulties when reading at school. Based on clinical observations, Tanner’s current communication was characterized as short phrases that were slightly unintelligible to an unfamiliar listener. A typically developing child that is Tanner’s age should be 100% intelligible. It is estimated that Tanner was 80% intelligible. The Goldman-Fristoe Test of Articulation Third Edition (GFTA-3) was utilized to evaluate Tanner’s speech production. Based on Tanner’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Per parent report, medical history is not remarkable
Speech Medications: None
Speech Behavioral Observations: Attending Skills: Adequate, Response Rate: Mild delays, Level of Activity: WNL, Cooperation: WNL, Communicative Intent: With some prompts, Awareness of Others: Present, Reliability of Scores: Adequate, Awarencess of Environmental Events: Good, Social Interactions: Usually, Prognosis for Improved Communicative Functioning: Shy,
Speech Communication Strengths: Appropriate utterances, Follows verbal command with cues,
Speech Communication Weaknesses: Difficult to understand, Poor use of eye contact, Unintelligible,
Speech Background Summary: The evaluation took place at the therapy room table. Overall, Tanner demonstrated suitable social-communication with communication partners. However, Tanner displayed poor eye-contact, and spoke with a soft voice. Tanner was unintelligible which negatively impacts his social-communication skills.
Speech Diagnosis/Impressions: Tanner is a 9 year 10 month old male who was referred for a speech and language evaluation by his pediatrician and mother’s concern regarding language skills and speech production. Tanner was accompanied to the evaluation by his mother who served as the informant. Tanner has had a history of receiving speech therapy since he was 3 years old, and stopped services because the family relocated to El Paso. A typically developing child that is Tanner’s age should be 100% intelligible. It is estimated that Tanner was 80% intelligible. Tanner’s oral peripheral structure and function were judged to be adequate for the normal production of speech. The Goldman-Fristoe Test of Articulation Third Edition (GFTA-3) was used to evaluate Tanner’s speech production. Tanner received the following standard scores on the GFTA-3: Sounds-in-Words 40 (raw score/errors: 40, percentile: <0.1, age-equivalent: 2:10-2;11), and Sound-in-Sentences: 53 (raw score/errors: 26, percentile: 0.1, age-equivalent: 6;11 or younger), both of which are significantly BELOW AVERAGE for a child his age. According to Tanner’s standard scores on the GFTA-3, where a score from 85-115 is considered within functional limits, Tanner presented with a PROFOUND ARTICULATION/PHONOLOGICAL PROCESSING DISORDER. Tanner was observed to perform the following phonological processes: cluster reduction, devoicing, substitution processes: vowelization of /r, l/, distortion of /r, l/ and had difficulty producing the following phonemes: /z, th/ in the medial and final word positions. Per clinician’s observations, Tanner was unintelligible at the word, phrase, and conversational level. Speech therapy is highly recommended at this time. Without speech therapy, safety risks include but are not limited to Tanner presenting with difficulty communicating basic wants, needs, and thoughts to those in his immediate environment especially in the case of an emergency. Based on Tanner’s reported language background and language input/output, the language of intervention will be English.

03-04-2026 04:09 PMAnnette BarronGood afternoon. Here is a brief summary from yesterday's evaluation. Tanner has been diagnosed a profound articulation/phonological processing disorder. Keep in mind the severity is high due to his age. He would benefit from goals targeting the production of: "r", "z" and "th" sounds, as well as "s", "l" and "r" blend words. As soon as all the paperwork has been signed and approved, the treating clinician will reach out to you to begin treatment. If you have any questions, please let me know.
