Assessment of Goals/Response to Treatment: The patient presented as friendly and cooperative but demonstrated significant difficulty maintaining attention to structured tasks, requiring frequent verbal and tactile cues to re-engage. This session focused on establishing baseline data for the current treatment plan. Regarding expressive language, the patient is not yet imitating 2-3 word phrases, as demonstrated by a performance of 0 out of 10 opportunities when provided with a direct verbal model. Communication was primarily characterized by the use of single words and vocalizations to make requests and comments, with no spontaneous use of 2-3 word phrases observed (0/10 opportunities). Social engagement and play skills were also assessed, revealing challenges with reciprocal turn-taking; the patient typically participated in a single turn before moving to a new activity, resulting in a baseline of 0/10 opportunities to complete three consecutive turns. Similarly, sustained attention to clinician-directed tasks was limited to approximately 20-30 seconds, falling short of the 1-2 minute target in 0 out of 10 opportunities presented. An initial assessment of articulation skills was conducted for target phonemes /t/, /d/, /g/, and /k/. The patient was able to produce /t/ and /d/ in isolation with a direct model and minimal cueing; however, production of the velar phonemes /g/ and /k/ in isolation was not observed. Bryson would greatly benefit from speech therapy that target the following age­appropriate concepts: joint attention/sustaining attention to tasks, imitating 2-3-word phrases, using 2-3 words in a variety of pragmatic functions, and taking turns during play-based activities without adverse behaviors and produce /t/,/d/,/g/,/k/ in isolation and at the syllable level.
