Speech Background History: Juan Diego is a 2 year, 3 month old male who was referred for a speech and language evaluation by his pediatrician for concerns with his speech and language. Juan Diegos’s mother  and father served as the informant for this evaluation. Mrs and Mr. Melendez reported concerns regarding Juan Diego’s verbal output as he mumbles and has little to no words. Mr. Melendez also reported that Enzo is not talking as much as kids his same age and only has around 10 words. Mrs. Melendez also reports concerns that something more is going on with Enzo than his speech delay. According to clinician's observations, Juan Diego’s current communication is characterized by use purposeful vocalizations and crying to express his wants/needs. Juan Diego was evaluated using the Preschool Language Scales 5th Edition English version (PLS 5). Due to Juan Diego’s language background and input/output, the language of intervention will be English.

Assessment of Goals/Response to Treatment: The patient was alert, cooperative, and oriented throughout the session. As this was the initial session, a primary focus was on establishing rapport through motivating, structured play activities. The patient demonstrated significant progress toward the goal of sustaining attention, successfully maintaining joint attention on play-based tasks for 30-45 seconds in 8 out of 10 opportunities. Intervention also targeted expressive language development. To improve functional communication, the patient was prompted to imitate gestures modeled by the clinician, achieving success in 2 out of 10 opportunities. Similarly, when working on the imitation of environmental and speech sounds, he produced accurate imitations in 2 out of 10 trials. Efforts were made to encourage the use of a total communication approach to express wants and needs, which the patient utilized in 2 out of 10 opportunities presented. The patient's parents were counseled on a home management intervention program, and strategies were provided to help incorporate therapeutic activities into daily routines to promote the generalization of skills to the home environment.

8. I am just hoping to help him learn to use words and for him to communicate what he wants and needs. It's hard to understand what he is upset about or what he wants. Doesn't refer to me as mom or to his dad as dad.
