2640303-11-2026Speech Initial EvaluationF80.2
Speech Background History: Lorenzo is a 6-year-old and 9-month-old male who was referred for a speech and language evaluation by her pediatrician due to POSSIBLE articulation/language delay. Mrs. Bakeraccompanied her and served as the primary informant. Mrs. Baker indicated that Lorenzo is diagnosed with ADHD. During the evaluation, Lorezon’s expressive language was characterized primarily appropriate sentences. Lorenzo was evaluated using the Preschool Language Scales Fifth Edition (PLSΒ-5) Language Assessment. LANGUAGE BACKGROUND: LANGUAGE INPUT: Lorenzo’s mother reported that he is exposed to only English within their home environment. LANGUAGE OUTPUT: During the evaluation, Lorenzo utilized full sentences however speech sounds errors were noticed during conversation.
Speech Medical History: Per parent report, medical history is not remarkable
Speech Medications: 1 mg of viloxazine for ADHD.
Speech Behavioral Observations: Attending Skills: Distractible, Response Rate: Appropriate, Level of Activity: WNL, Cooperation: WNL, Communicative Intent: Adequate, Awareness of Others: Present, Reliability of Scores: Adequate, Awarencess of Environmental Events: Good, Social Interactions: Moderately, Prognosis for Improved Communicative Functioning: Friendly,
Speech Communication Strengths: Appropriate use of eye contact, Follows verbal command with cues, Follows verbal command without cues, Initiate activity/dialogue, Maintains topic, Responds to greetings,
Speech Communication Weaknesses: Unintelligible,
Speech Background Summary:
Speech Diagnosis/Impressions: Lorenzo is a 6-year, 9-month-old male who presents with a diagnosis of Attention-Deficit/Hyperactivity Disorder (ADHD), referred for evaluation due to concerns regarding a possible articulation and language delay. Clinical assessment and standardized testing confirm the presence of a mild Expressive Language Disorder (F80.1) and a mild Speech Sound Disorder (F80.0). Objective language testing via the Preschool Language Scales, Fifth Edition (PLS-5) revealed measurable deficits, with all domains falling below the 50th percentile for his age. He demonstrated an Auditory Comprehension age equivalency of 6 years, 2 months, representing a 7-month delay, and an Expressive Communication age equivalency of 6 years, 0 months, representing a 9-month delay. His Total Language score reflects an overall delay of 8 months. Functionally, these delays, which are all greater than six months, impair his ability to consistently formulate grammatically correct, complex sentences and fully comprehend multi-step directions within the academic setting. This directly impacts his academic readiness, pragmatic interactions, and ability to effectively communicate his wants, needs, and ideas with peers and adults, leading to potential frustration and breakdowns in social participation. On the Goldman-Fristoe Test of Articulation, Third Edition (GFTA-3), Lorenzo’s performance on the Sounds-in-Words subtest yielded a standard score of 87, placing him at the 19th percentile. This score falls significantly below age-level expectations and approaches a clinically significant deviation of -1.0 standard deviations below the mean. This underlying deficit in phonological processing contributes to reduced overall speech intelligibility, which was noted as a primary weakness. Impaired intelligibility negatively impacts his ability to be understood by unfamiliar listeners, engage confidently in classroom dialogue, and build successful peer relationships. Other domains, including oral-motor function, feeding, fluency, and voice, were assessed informally and determined to be within functional limits. Prognosis for improved communicative functioning is considered excellent with therapeutic intervention. Based on the objective findings of this evaluation, skilled Speech Therapy services are medically necessary to address the identified deficits associated with Expressive Language Disorder and Speech Sound Disorder. The combination of a 9-month expressive language delay and a speech sound production deficit placing him in the 19th percentile creates a significant barrier to effective communication and places him at high risk for ongoing academic challenges and negative social-emotional consequences. Therefore, it is recommended that Lorenzo receive Speech Therapy two (1) times per week. This intensity is warranted to facilitate consistent progress in remediating phonological error patterns and developing age-appropriate language formulation skills, which is critical for his success in the first-grade curriculum. Postponement of intervention would result in a widening of the developmental gap between Lorenzo and his peers, further compromising his educational and social development. Skilled, evidence-based intervention is required to improve his speech intelligibility and expressive language abilities, thereby supporting his functional independence and meaningful participation in home, school, and community settings.

Speech Rehabilitation Potential: Good. with consistent Speech Therapy and parental support
Speech and Language Long Term Goals:
CategoryGoal Description
Home Management Intervention ProgramWhen counseled by the clinician, parents will incorporate the home management intervention program as a daily and weekly activity to transfer skills learned in therapy into the home environment. Home management training will optimize patients functional communication tasks in the home.
Expressive LanguageWithin 12 months, Lorenzo will improve expressive language skills by producing grammatically appropriate sentences to describe events, answer questions, and share ideas during structured and conversational tasks with the clinician and peers in order to effectively communicate in academic and social settings (e.g., participating in classroom discussions, retelling events, and expressing needs and opinions).
Receptive LanguageWithin 12 months, Lorenzo will improve receptive language skills by understanding age-appropriate vocabulary, multi-step directions, and language concepts during structured and classroom-based activities with the clinician in order to support comprehension and participation in academic and social interactions (e.g., following classroom instructions, answering WH-questions, and understanding spoken information).
Speech and Language Short Term Goals:
CategoryGoal Description
Oral Motor/ArticulationIn 6 months, Lorenzo will produce initial consonant blends (e.g., cr, st, bl, tr) at the word level during structured articulation tasks with 30% accuracy given verbal and visual cues across 3 consecutive sessions in order to improve overall speech intelligibility for functional communication in everyday and academic activities (e.g., participating in conversations and responding to questions).
Oral Motor/ArticulationIn 6 months, Lorenzo will produce /th/ (voiced and voiceless) in all word positions during structured articulation tasks with 30% accuracy given verbal and visual cues across 3 consecutive sessions in order to improve overall speech intelligibility for functional communication in everyday and academic activities (e.g., participating in conversations and responding to questions).
Receptive LanguageIn 6 months, Lorenzo will retell key details from a short story or passage (e.g., characters, setting, and main events) after reading or listening to the story with 30% accuracy given visual or verbal supports across 3 consecutive sessions in order to improve reading comprehension and ability to discuss academic material.
Receptive LanguageIn 6 months, Lorenzo will answer WH-questions (who, what, where, when) about a short grade-level passage read aloud or independently with 30% accuracy given minimal verbal cues across 3 consecutive sessions in order to improve comprehension of spoken and written information in academic activities.

03-03-2026 01:23 PMKaitlin MarslandHello, part of occupational therapy is working toward increasing self-regulation skills and giving Lorenzo the opportunity to understand his emotions and strategies he can utilize to maintain a calm demeanor so I believe it would be beneficial for him to attend to increase those skills. I hope I haven’t done anything in previous sessions to overwhelm him as that is not my intent. Occupational therapy is meant to utilize personal motivations to build upon skills so it shouldn’t feel overwhelming for him. Please let me know if he has struggled previously after our sessions
