Feeding Background History: Luca is a 5 year, 4­month­old male referred by his pediatrician for a feeding evaluation due to concerns with feeding. His father and mother reported that Luca began displaying feeding concerns around 2 years of age. Luca has a history of early feeding difficulties, including persistent gagging and reflux. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills. Language of intervention: Spanish with support in English.

Feeding Background Summary: Luca's pragmatic skills were not formally assessed as this was not the purpose of the evaluation.
Feeding Diagnosis/Impressions: Luca is a 5 year, 4­month­old male referred by his pediatrician for a feeding evaluation due to concerns with feeding. His father and mother reported that Luca began displaying feeding concerns around 2 years of age. Luca has a history of early feeding difficulties, including persistent gagging and reflux. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills. Language of intervention: Spanish with support in English. A formal oral mechanism examination revealed the absence of tethered oral tissues. Structurally, Luca appears to have all anatomical components necessary for safe and efficient feeding and swallowing. However, the function of his oral mechanism appeared reduced, with decreased facial muscle tone, limited jaw stability, and inconsistent tongue movement and oral control observed during feeding. These oral mechanism findings are consistent with his reduced oral motor feeding skills and may contribute to his difficulty coordinating chewing and swallowing higher bolus consistencies. A bedside swallow evaluation and skilled meal observation revealed that Luca presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. His PFD is characterized by impaired oral intake that is not age-appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures), and psychosocial (selective/restrictive eating). Luca’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: Luca's appetite is variable day to day. Currently, his food inventory includes fewer than 30 accepted foods and 6 liquids. It is important to note the majority of his accepted foods are starches which limit nutritional value. Accepted proteins include scrambled eggs, ham, meatballs, peanut butter, gogurt, French toast, and chicken nuggets. Fruits include blueberries, apples, pears, strawberries, blackberries, peach cup, banana, grapes, orange, tangerine. Vegetables include green beans, carrots, and corn. Starches include French fries, potato wedges, oatmeal, pancakes, breakfast bars, dry cereals, waffles, crackers, pretzels, vegetable chips, flour tortillas, rice, bread, donuts, cookies, popcorn. Liquids include orange juice, grape juice, fruit punch, apple juice, lemonade, milk and water. Luca's father expressed a desire for him to expand his intake to include more soft foods. NORM REFERENCED ASSESSMENT: Mrs. Pillados completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver report questionnaire for children aged 6 months to 7 years. Luca received an overall rating of “high concern,”. Luca received received “high concern” in the area of problematic mealtime behaviors and selective/restrictive eating. These concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Luca was seated and self-fed his preferred food items (pancakes and blueberries). Non­preferred foods offered were rice, carrots, celery, ham, and apple sauce. With clinician support and implementation of breathing exercises to reduce his gag reflex, Luca was able to consume at least two bites of each food trial. Mother reported that Luca is not receptive to trying new foods at home and will often say "no" when non-preferred items are presented. Mother also reported the use of electronics during mealtime routines. SWALLOW/ORAL MOTOR EVALUATION: Luca demonstrated decreased oral motor skills, including a munching bite pattern, open-mouth chewing, reduced tongue lateralization, and oral residue. Mr. Pillado does not report unexplained fevers or a history of aspiration pneumonia. Mother reported vomiting or gagging at home when Luca tried new foods. Luca’s pharyngeal/esophageal swallow function should be monitored by his treating clinician to ensure safety and determine if further testing is warranted. At this age, a child should demonstrate a more developed rotary pattern and management of thicker purees and soft solids. The gag reflex should be active, but should be less sensitive. Luca’s oral motor skills are NOT WITHIN FUNCTIONAL LIMITS for his age and/or feeding safety. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30-45 minutes to address Luca’s CHRONIC PFD and ORAL DYSPHAGIA. Luca is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. Feeding therapy should also target improving Luca’s oral motor skills for safe and effective feeding/swallowing. In addition, food chaining strategies should be implemented to expand Luca’s food repertoire by building on existing preferred foods. Without intervention, Luca may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Mrs. Pillado will be responsible for implementing the recommended Home Management Program to reinforce therapeutic strategies and support progress between sessions.

Assessment of Goals/Response to Treatment: The patient was alert, oriented, and demonstrated excellent cooperation throughout the session. Significant gains were observed in oral sensory tolerance, as the patient fully met the objective of tolerating non-nutritive oral sensory input, including the Z-vibe and oral swabs, to the lips, cheeks, and anterior tongue for 5-second intervals without eliciting a gag reflex. This was consistently achieved in greater than 2 out of 10 opportunities, indicating a marked decrease in oral hypersensitivity. Concurrently, progress was made in developing mature mastication patterns. The patient initiated a rotary chew during targeted oral motor exercises in approximately 2 out of 10 opportunities, representing 80% progress toward the terminal objective of 3 out of 10 opportunities. Regarding diet expansion, the patient successfully incorporated one new food-chained item of a soft-cubed consistency into his repertoire, achieving 50% of the intended goal to add one to two new foods. His acceptance of non-preferred foods has also improved considerably, as evidenced by his consumption of 2 to 3 bites of these items in up to 3 out of 10 opportunities across consecutive sessions, demonstrating 90% progress toward this goal. These advancements reflect an overall improvement in the patient's tolerance and acceptance of varied sensory inputs and food items in his immediate environment.

02-10-2026 10:08 AMAndrea RodriguezBuenos días! Los documentos los puede dejar con la persona del front desk. Un recordatorio de empezar a alentar a Luca a usar su nariz para respirar mientras come y a morder con los dientitos de atrás. El respirar por la nariz ayudará a regular su sistema nervioso, lo cual le ayudará a prevenir esa sensación de vómito al momento de probar cosas nuevas. Como habíamos platicado, pueden empezar a servir en su plato 30% de cosas que sí consume y 70% porciones nuevas. Si a Luca primero se le presenta comida no preferida y luego se le retira para después darle comida que sí consume podemos formar una rutina en donde él sabe que si no interactúa con la comida se le dará algo que sí quiere comer. A continuación le enviaré una guía (inglés) que dice paso por paso cómo pueden implementarlo en casa.
02-10-2026 10:09 AMAndrea RodriguezThe goal of family meals is to focus on the volume of foods consumed, with exposure to unfamiliar or unaccepted food as a secondary goal. Our goal for these meals will not be to consume non-preferred foods but to start tolerating them at the level of the plate and fingers. Throughout the meal, you can emphasize things like 'I am touching the chicken with my fingers' or 'I do not want to eat the ham, but I can put it on my learning plate'. The 'learning plate' should be an empty plate/bowl placed in front of the dinner plate or place mat. The purpose of this plate is that if Luca expresses that he does not want a non-preferred item on his plate, you can tell him to put it on his 'learning plate'. By doing this, he will still be interacting with non-preferred items, as he will have them directly in front of him. Our goal is for him to start tolerating food around him without any pressure, eventually leading to him interacting with it at the level of the fingers and oral cavity. This will decrease any frustration during meal times and will prevent him from putting the food in the trash or away. Make sure plates, napkins, cups, and placemats are all plain and white (again, the focus should be the food). These family meals should be performed once per day. Pick a time that works best for you where you can all model behaviors.

- It may be useful to make a change to where people sit at the table, to change the visual environment, and avoid negative cues that may have been formed previously.



4. Family-style serving
- Place all food items in the center of the table.
- Everyone has to take some of each food offered (you can include a "learning plate" placed in front of his tray, where he can place the food items he does not want)
- Make sure there are at least 2 preferred items so he can have something to eat.

- The focus of the meal should be the food
- Goal for parents is to be good role models. have pleasant conversation, and to let him do what he can by himself.
- We want to avoid giving him negative feedback during mealtimes. Praise if any good attempts are made.

6. Offer a drink (if needed and/or not eating)
- If not eating, you can offer a drink to help Luca engage with food.

- This will allow you and Luca to be weaned off the TV with time rather than just removing the item at once.


8. After 30 minutes, begin the clean-up routine.
- Have everyone empty their plate into a bowl/trash can
- If he has been eating all along and is done eating sooner, do clean up sooner
- If he really cannot sit any longer, you may need to do clean up earlier.

Si la respuesta de Luca es "no quiero", ustedes pueden proceder a seguir probando mientras describen el alimento. La meta es que Luca sea receptivo a lo que ustedes hacen. Muchas gracias!
