Occupational Background History: Jackson is a 12.6 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and no seasonal allergies.

Jackson demonstrates deficits in visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Subjective: Patient was friendly and cooperative,
Assessment of Goals/Response to Treatment: Jackson followed verbal cues to wash/cut vegetables in preparation for the feeding activity with only MIN cues for assistance. He was also able to follow a 4 step process to warm chicken noodle soup in the microwave to demo increased attention and ability to sequence multistep tasks. Jackson tolerated 4 different textures of food items when manipulating with bilateral hands to explore with no aversion. He also engaged in SOS steps for feeding IND to demo increased ability to explore novel foods with independence. He smelled, licked, bit, and chewed all items. He swallowed all items except the grape tomato to demo decreased aversion to novel foods. Jackson has tolerated individual foods with increased success, to continue with exploration of mixed foods to further increase repertoire of foods to increase participation at mealtimes. Jackson has also demonstrated increased ability to describe foods and states similarities between novel foods with preferred foods to decrease aversion with only MIN cues for assistance.

He was able to chew and swallow the celery but he didn't seem like he loved the taste so I showed him celery and peanut butter together which he said he liked better. He ate majority of the orange and said "it wasn't bad" and he ate a few spoons of the rice. He said it was alright but not much flavor or anything so we talked about things he could try with the rice like chicken. I asked him if he could attempt being more open about requesting to try exploring foods that he thinks looks/smells interesting to him when everyone is eating if he thinks it looks/smells good he could ask to taste. He told me about the carrots and that it wasn't his favorite but I praised his effort in exploring a new item. I think it helps when I ignore or act "nonchalant" when he is exploring as opposed to commenting or praising him each time because I noticed today when I left pineapple on the plate he actually went for another piece on his own while I was heating the rice. I told him I won't give him a number but over the weekend if he thinks something looks or smells interesting if he could request to explore it. He said he couldn't think of anything specifically to try but that he would look at the grocery store. Hope you have a good weekend, if you have any questions let me know :)

03-09-2026 12:06 PMKaitlin MarslandJackson did really well today, he ate most of the blackberries, he liked the strawberries but better with a little sugar on top since he said they weren't as sweet. He attempted the Brussel sprout raw and boiled in water but he was not able to tolerate that one however he did chew it. Those darker greens will be a little harder but continued exposure will help. He enjoyed "Lipton" chicken noodle soup in the packet I brought since it was similar to the ramen noodles and he said he could eat it. He tried just the noodles and with the broth and enjoyed it. He also said he preferred the melon most from the fruits today and he related it to watermelon which he enjoys. Through the week continue to facilitate opportunities for him to explore new foods or re-introduce foods we have explored in therapy. It is wonderful he explores them to the point of swallowing but if he is able to chew and swallow the foods then he is accepting of them so he should be able to incorporate it into his meals. You can start offering small pieces of those foods or ask him how much he would be willing to eat. Start small and he can always add more if he wants it.

03-27-2026 10:50 AMKaitlin MarslandSo Jackson is now comfortable with broccoli. He said it was the highest ranked vegetable he has eaten. He liked it cooked. I also had him try another version of chicken noodle soup that he liked. it was a Campbells chunky chicken noodle soup. He stated he wants to explore more soups and we also talked about different restaurants and the foods similar to what he has tried. He said he sees broccoli a lot at restaurants and he would be willing to order it. I told him about the raw vegetables in salads like at Texas Roadhouse and I told him they put egg in it and he asked what kind of egg and he stated he needs to explore more types of egg so maybe another avenue you all can explore over the weekend :) For Monday, can you assemble a breakfast or lunch meal for him to bring? I feel like he has explored a good amount of singular items and it would be good to try a meal type setting. Then Friday I will bring items from Chipotle for him to try. He already let me know which items he is willing to explore from there.
