Speech Background History: Luca is a 3 year 3 month old male who was referred by his pediatrician for a speech and language evaluation due to mother’s concerns regarding speech delay. Luca was accompanied by his mother, who served as the informant. Although mother suffered a stroke 7 weeks into the pregnancy, the issue was resolved, and Luca was born at full-term with no complications. According to mother, Luca is “very talkative” but difficult to understand, often struggling with “certain consonants in words.” Luca often utilizes a leading behavior, pointing and gestures to assistance when expressing his communication wants/needs. Luca has received speech therapy in the past with ECI and with another private group. However, both times mother discontinued services due to lack of progress or Luca becoming frustrated with the group setting. Based on clinical observations, Luca’s current communication was characterized as 1-2 word utterances and word approximations, mixed with use of jargon, gestures, and pointing to express his communication wants/needs. A typically developing child that is Luca’s age should have 100-200 words in their expressive vocabulary, and use 2-3 word utterances consistently to label, comment or request. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Luca’s receptive and expressive language skills. Based on Luca’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Per parent report, medical history is not remarkable. According to mother, Luca has not been diagnosed with any medical conditions, but is concerned about his congestion. Mother will give Luca over the counter (OTC) allergy medication when Luca seems congested.

Luca received the following standard scores on the PLS-5: Auditory Comprehension 94 (raw score: 37, percentile: 34, age-equivalent: 3 years 0 months), Expressive Communication 74 (raw score: 27, percentile: 4, age-equivalent: 1 year 11 months), and Total Language Score 83 (raw score: 64, percentile: 13, age-equivalent: 2 years 6 months). 5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?
6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history?
7.) Which language(s) is the child exposed to regularly?
8.) Vision or hearing issues?

2.) while I was pregnant 7 weeks I had a stroke. I was able to come out of it without having any issues after.

Physically he has had no issues he stood up and was walking before his first birthday, his first word was mama. Combined words would be last year maybe February . He liked to do a lot of continuous talking like he wanted to have a conversation with you but didn't have the words yet

5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?

He usually sleeps 10 hours or more, he doesn't wake up usually unless he is sick during the night. We have noticed he's been snoring as he's gotten older I would say after he turned 2 we can hear it more. Sleeps with both closed and open mouth just depends on the day .

6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history?
He was going to a speech therapist September 2024 and was attending a speech therapy play group with other kids led by the same SLP.
He was also getting help through ECI through the city prior to the SLP so I would say that was the beginning of 2024.


7.) Which language(s) is the child exposed to regularly? English and Spanish
8.) Vision or hearing issues? None

I would love for Luca to be able to say more words correctly, he has a tendency to not say certain consonants in words, I want others to understand him better!

Luca has been diagnosed with a profound to moderate to mild expressive language disorder, where his expressive (moderate) language skills were more severe than his overall total language (mild) score. Luca would benefit from speech therapy targeting the following: imitation of functional gestures, imitation of speech sounds, imitation of monosyllabic words, naming common objects via word approximations and/or gestures, and using total communication to express wants/needs. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the assigned treating therapist will reach out to you to discuss beginning treatment.
