Speech and Language Evaluation:

IDDateTypeEvaluation Diagnosis
2610301-23-2026Speech Initial EvaluationF80.2
Speech Background History: Ian is a 2-year-old male who was referred for a speech and language evaluation by his pediatrician due to language delay. Mrs. Yee accompanied him and served as the primary informant for the evaluation. Mrs. Yee expressed concern that Ian has difficulty communicating effectively when compared to same-aged peers. She also reported that Ian has a twin brother and that she observes similar communication difficulties in both children. During the evaluation, Ian’s expressive language was characterized primarily by single words, jargon, and symbolic gestures (e.g., pointing). According to developmental norms, a typically developing child of Ian’s age should have an expressive vocabulary of approximately 200 words and produce utterances of 1-2 words in length. Ian was evaluated using the Preschool Language Scales Fifth Edition (PLS-5) Spanish assessment. LANGUAGE BACKGROUND: LANGUAGE INPUT: Ian’s parents reported that he is exposed to Spanish within his home environment. LANGUAGE OUTPUT: During the evaluation, Ian had limited verbal output. LANGUAGE OF INTERVENTION: It is recommended that intervention be conducted primarily in Spanish.
Speech Medical History: According to the parent report, Ian was born without any complications prior to, during, or following the delivery.
Speech Medications:
Speech Behavioral Observations: Attending Skills: Adequate, Response Rate: Impulsive, Level of Activity: Active, Cooperation: Active, Communicative Intent: With some prompts, Awareness of Others: Present, Reliability of Scores: Adequate, Awarencess of Environmental Events: Good, Social Interactions: Usually, Prognosis for Improved Communicative Functioning: WNL,
Speech Communication Strengths: Interactive,
Speech Communication Weaknesses: Poor vocabulary, Unable to follow commands,
Speech Background Summary: Ian's mother accompanied him to the evaluation, which took place in the clinic room. Throughout the evaluation, Ian displayed signs of distractibility, as evidenced by his difficulty remaining seated and his need for FREQUENT and NUMEROUS prompts to participate. Ian was observed intentionally dropping to the floor when asked to wait or when access to preferred items was restricted.
Speech Diagnosis/Impressions: Ian is a 2-year-old male who was referred for a speech and language evaluation by his pediatrician due to language delay. During the evaluation, Ian displayed signs of distractibility, as evidenced by his difficulty remaining seated and his need for FREQUENT and NUMEROUS prompts to participate. Ian was observed intentionally dropping to the floor when asked to wait or when access to preferred items was restricted. Ian received the following Standard Scores on the PLS-5: Auditory Comprehension 73 (raw score: 21, percentile: 4, age equivalent:1 year 5 months), Expressive Communication 79 (raw score: 21, percentile: 8, age equivalent: 1 year 6 months), and Total Language Score 74 (percentile: 4, age equivalent: 1 year 5 months). According to the Standard Scores that Ian received, he presents with MODERATE MIXED RECEPTIVE -EXPRESSIVE LANGUAGE DISORDER. RECOMMENDATIONS: At this time, speech and language therapy at the rate of 1x per week for 45-60 minutes is warranted to treat Ian’s MODERATE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. Ian would greatly benefit from speech therapy that targets the following ageappropriate concepts: joint attention/sustaining attention to tasks, demonstrating functional play, following directions using common objects, imitating gestures/actions, and environmental sounds/words, and tolerating or demonstrating turn-taking. Speech therapy is highly recommended at this time as Ian's current speech and language skills are NOT DEVELOPMENTALLY FUNCTIONAL for his age. Without speech therapy, Ian's safety risk is high since he will not likely be understood if he needs to communicate pain, communicate need for assistance, or understand safety personnel/directions. Ian's mother will be responsible for his Home Exercise Program (HEP).

02-05-2026 02:21 PMJessica OrtizHola, buenas tardes.
**Ian: trabajar en atención sostenida, demostrar juego funcional, seguir instrucciones utilizando objetos comunes, imitar gestos o acciones, imitar sonidos o palabras y tomar turnos apropiadamente.
02-05-2026 02:23 PMJessica OrtizUna gran disculpa, pensé que sí se los habia mandado antes. Toda la documentacion ya se mandó; solamente está en espera de que den autorización para comenzar la terapia.
02-05-2026 02:27 PMIan OlivaIan (Prim) Replied: Hola buenas tardes muchas gracias..

02-05-2026 02:38 PMIan OlivaIan (Prim) Replied: Bueno muchas gracias buen día 😁
