Speech Background History: Cesar is a 2-year-old male who was referred for a speech and language evaluation by his pediatrician due to language delay. Mrs. Yee accompanied him and served as the primary informant for the evaluation. Mrs. Yee expressed concern that Cesar has difficulty communicating effectively compared to peers of the same age. She also reported that Cesar has a twin brother and that she observes similar communication difficulties in both children. During the evaluation, Cesar’s expressive language was characterized primarily by the use of single words and symbolic gestures (pointing). According to developmental norms, a typically developing child of Cesar’s age should have an expressive vocabulary of approximately 200 words and produce utterances of 1-2 words in length. Cesar was evaluated using the Preschool Language Scales Fifth Edition (PLS­-5) Spanish assessment. Cesar was observed intentionally dropping to the floor when asked to wait or when access to preferred items was restricted. Occupational Background History: Cesar is a 2.2 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 38 weeks gestation via c-section (twin). Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression due to sensory processing difficulties and no seasonal allergies.

Cesar demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. 02-10-2026 02:39 PMCesar OlivaI think it may sound a little like I'm changed and changed everything but I wanted to mention that from March I will have a baby and who can help me continue taking the children to therapy is available from 3:00, they had mentioned to me that they could make room for me in the afternoon. At the moment I can take them at 12:00 but eventually to continue with the therapies I would have to change it to 3:00, I don't know if it would be more convenient to wait to find someone who can see them from that time from now on so that later I don't have to change them.. sorry

We can only offer you from this point on the slots i got which are the 12noon or maybe earlier in the morning. How would you like to proceed?

02-09-2026 02:03 PMCesar OlivaCesar (Prim) Replied: Me habían comentado que podían asignarles un terapeuta a cada uno para ver si las terapias pueden ser al mismo tiempo y ya estaría media hora con uno y media hora con el otro, en ese caso si pudiera ir yo sola a cualquier hora, de otra forma tendría que buscar la forma de acomodarme en la tarde por qué la gente que puede ayudarme le digo se desocupa a partir de las 2:30 de lunes a viernes o hay otra persona que se desocupa a las 2:00 en viernes. También quería mencionar que en unas semanas tendré a mi otro bebé entonces si o si tendría que encontrar un lugar en la tarde por qué como le comento solamente tengo ayuda en las tardes. Me dijeron que iban a dejar una nota con toda esa información.
