Feeding Diagnosis/Impressions: Michael is a 6 year 5 month old male who was referred by his pediatrician for concerns regarding feeding difficulties due to concerns with picky eating and limited nutritional variety. His mother, Mrs. Velez, reports that Michael began displaying picky eating behaviors around 4 years of age and is highly resistant to trying new foods, with no reward or incentive proving effective. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Michael’s feeding/swallowing skills. Structurally, Michael appears to have all anatomical components necessary for safe and efficient feeding and swallowing. Michael presents with the oral structures necessary for feeding and swallowing. Oral mechanism function appeared intact, as evidenced by his ability to chew and swallow solid foods during meal observation. No tethered oral tissues were reported by his mother. Hismother reports that Michael occasionally sleeps with his mouth open and snores. The family was advised to consult with an ENT specialist and/or pursue a sleep study if concerns persist. A bedside swallow evaluation and skilled meal observation revealed that Michael presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD). His PFD is characterized by impaired oral intake that is not ageΒ­appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures, tastes, and smells), and psychosocial (selective/restrictive eating and problematic mealtime behaviors). FOOD REPERTOIRE: Michael is reported to prefer foods with crunchy consistencies and demonstrates strong preferences for salty, sweet, and flavored items. His appetite is variable day to day. Currently, his food inventory includes around 50 accepted foods and 6 liquids, with the majority being carbs, snackΒ­type items, low in protein, and vegetables. It is important to note that the majority of proteins Michael will only accept a few bites per meal. Accepted proteins include chicken, chicken strips, chicken nuggets, turkey, beef, hamburger, pepperoni, hot dogs, steak, sausage, corn dogs, peanut butter, Monterrey jack cheese, boiled eggs, and ground beef. Fruits include apple, kiwi, lime, strawberry, pineapple, banana, orange, dried mango, and mango. Vegetables are limited to lettuce. Starches include crackers, chips, pretzels, bread rolls, pizza crusts, rice, bread, donuts, cake, cookies, French fries, spaghetti, lasagna, chicken noddleFrench toast/waffles/pancakes, pizza, and dry cereal. Liquids include orange juice, lemonade, milk, chocolate milk, drinkable yogurt, and water. Michael’s mother expressed a desire for him to expand his intake to include more proteins and vegetables. NORM REFERENCED ASSESSMENT: Mrs. Velez completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiverΒ­report questionnaire for children aged 6 months to 7 years. Michael received an overall rating of β€œno concern,” however, he received β€œhigh concern” in the area of problematic mealtime behaviors. These concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Michael was seated at the feeding table and self-fed his preferred food items using utensils (French toast, white rice, and mango. He remained seated throughout the session and demonstrated a slightly fast rate of intake without signs of overstuffing. When offered non-preferred foods (blueberries, watermelon, and beans), Michael refused by pushing the food away. With clinician support, he licked, smelled, and touched non-preferred items with his teeth. SWALLOW/ORAL MOTOR EVALUATION: Michael demonstrated a rotary chewing pattern when consuming preferred food items, with some instances of open-mouth chewing. AnteriorΒ­posterior bolus transit was timely, and no clinical signs or symptoms of aspiration or penetration were observed. His intake rate was slightly elevated, though manageable. According to Mrs. Velez, there are no concerns with swallowing at home, no coughing, choking, or signs of distress during meals, and Michael has no history of aspiration pneumonia. While his fast intake rate has occasionally been observed, it was addressed during the session. Based on foods trialed and behaviors observed, Michael’s oral motor skills appear functional at this time. Continued monitoring is recommended throughout treatment to ensure safe feeding and swallowing. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30-45 minutes to address Michael’s CHRONIC PFD. Michael is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. In addition, food chaining strategies should be implemented to expand Michael’s food repertoire by building on existing preferred foods. Without intervention, Michael may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative for the feeding therapy session. He demonstrated progress across all feeding and swallowing objectives. Progress was noted toward the goal of interacting with non-preferred foods using his fingers, which he achieved in approximately 1-2 out of 10 opportunities. He showed continued improvement in bringing non-preferred foods to his face to smell or touch, demonstrating this skill in approximately 2 out of 10 opportunities. During today's session, spinach and lettuce were presented for the second time, and cranberry raisins were introduced for the first time; the patient independently brought one of these items to his face to smell on one occasion. Furthermore, his tolerance for oral interaction with new foods is increasing, as he is now licking and/or biting non-preferred foods with approximately 21% accuracy given multisensory cues. This was particularly evident during the introduction of a novel protein, chicken, which was targeted for the first time and resulted in the patient taking 5 independent bites. The patient demonstrated the most significant gains in his ability to verbally engage with foods, as he identified and described sensory features with approximately 24% accuracy when provided with multisensory cues.

Well he basically refuse to eat anything. Breakfast are a little easy because he likes pancakes, cereals and bread. But proteins are a hard no. He rarely eats egg. Won't eat any type of beans. Nothing seafood related. Chiken will take a few bites and nothing else. Beef only in spaghetti and steak. No veggies, at all. I've tried in soups, rice, with the meat, cheese and nothing.
