A formal oral peripheral examination could not be completed due to Charles’s distractibility and difficulty following instructions. However, observable findings suggest that Charles presents with the oral structures necessary for feeding and swallowing. Oral mechanism function appeared intact, as evidenced by his ability to chew and swallow solid foods during meal observation. No tethered oral tissues were reported by his mother. Visualization of the palate was not possible at this time. It is recommended that Charles’s oral cavity be monitored throughout the course of treatment. A bedside swallow evaluation and skilled meal observation revealed that Charles presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. His PFD is characterized by impaired oral intake that is not age-appropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures, tastes, and smells), and psychosocial (selective/restrictive eating ). Charles’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: Charles is reported to prefer foods with crunchy and soft consistencies and demonstrates strong preferences for salty, sweet, and flavored items. His appetite is variable day to day. Currently, his food inventory includes fewer than 30 accepted foods and 3 liquids, with the majority being carbs, snack-type items, and low in protein. Accepted proteins include chicken nuggets, fried chicken, beef, pork, bacon, sausage, fish, and scrambled eggs. Fruits include blueberries, strawberries, peaches, bananas, coconut, and mango. All fruits are preferred in a smoothie or yogurt. Vegetables include fried zucchini. Starches include crackers, chips, cheese puffs, pizza crusts, bread, biscuits, mac n cheese, french fries, waffles, tater rounds, potatoes wedges. Liquids include milk, drinkable yogurt, and water. Charles's mother expressed a desire for him to expand his intake to include more proteins and vegetables. Mother would like Charles to consume more vegetables, proteins, and fruit. NORM REFERENCED ASSESSMENT: Mrs. Atkins completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver report questionnaire for children aged 6 months to 7 years. Charles received an overall rating of “no concern,” however, he received “concern” in the area of selective/restrictive eating. These concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Charles was walking around the room and self­-fed his preferred food items (French fries, chicken nuggets, and crackers). Mother reported that during mealtimes at home, Charles will sit in his high chair. When offered non­preferred foods (grapes and veggies), Charles touched the grapes and proceeded to put them in the bag. With clinician and parental support, he smelled the grapes. Father mentioned Charles is receptive to touching non-preferred food at home, but will not interact with it at the level of his face. Parents reported the use of television during mealtimes. SWALLOW/ORAL MOTOR EVALUATION: Michael demonstrated an emerging rotary chewing pattern when consuming preferred food items, with some instances of open­mouth chewing. Charles repeatedly pushed the French fries into his mouth with the use/support of fingers to manipulate food in his oral cavity. Emerging tongue lateralization was observed. No clinical signs or symptoms of aspiration or penetration were observed. His intake rate was slightly elevated, though manageable. According to Mrs. Atkins, there are no concerns with swallowing at home, no coughing, choking, or signs of distress during meals, and Charles has no history of aspiration pneumonia. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30­ minutes to address Charles’s CHRONIC PFD. Charles is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. Feeding therapy should also target improving Charles’s oral motor skills for safe and effective feeding/swallowing. To improve the acceptance and exploration of non-preferred foods, therapeutic activities targeted tactile interaction using his whole hands. Progress in this area remains limited, as the patient made brief contact with non-preferred food items infrequently, achieving approximately 20% of the targeted 3 out of 10 opportunities. Caregiver education was a primary component of the session, with a focus on demonstrating and implementing the home feeding plan. Strategies including the removal of TV during mealtimes, food chaining, positive mealtime modeling, and ensuring proper seated positioning were reviewed and modeled. Per caregiver report and direct observation, the family is beginning to implement these strategies, with continued support required to achieve consistent application with 50% accuracy to promote safe feeding and reduce choking risk. Continued skilled intervention is warranted to address oral sensory processing deficits and expand the patient's food repertoire through hierarchical exposure.

Our goal for these meals will not be to consume non-preferred foods but to start tolerating them at the level of fingers and face. Throughout the meal, you can emphasize things like 'I am touching the chicken with my fingers' or 'I will smell the grapes'. Our goal is for him to start tolerating food around him without any pressure, eventually leading to him interacting with it at the level of his oral cavity. These family meals should be performed once per day. Pick a time that works best for you where you can all model behaviors.

1. Give a verbal warning prior to mealtimes (5-minute warning)
- This prepares for the sensory input shift, allowing him to take on a new task.

2. Introduce transition activity (washing his hands)
-Input for calming and washing for tactile preparation.

3. Sit at the table
- Start with an empty tray/plate to decrease frustration before meals.
- Check his seat so he is positioned 90 degrees at the hips, knees, and ankles. We want to make sure his feet have support. You may need to adjust the footrest or place towels or any item that can help with support.
- It may be useful to make a change to where people sit at the table, to change the visual environment, and avoid negative cues that may have been formed previously.

- Everyone has to take some of each food offered
- Make sure there are at least 2 preferred items so he can have something to eat.

5. Everybody eats for the first 10 minutes

- We want to avoid giving him negative feedback during mealtimes. Praise if any good attempts are made.

8. After 30 minutes, begin the clean-up routine.
- Have everyone empty their plate into a bowl/trash can
- If he has been eating all along and is done eating sooner, do clean up sooner
- If he really cannot sit any longer, you may need to do clean up earlier.
