A bedside swallow evaluation and skilled meal observation revealed that Dominic presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. His PFD is characterized by impaired oral intake that is not ageยญappropriate and appears to be influenced by multiple domains: medical (history of feeding difficulties), nutritional (limited food inventory), feeding skill (sensory sensitivities to food textures), and psychosocial (selective/restrictive eating). Dominicโ€™s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: It is important to note that Dominic, according to father, will only consume foods in a pureed or blended form. His appetite is variable day to day. Currently, his food inventory includes fewer than 20 accepted foods and 5 liquids. Accepted proteins include peanut butter, ground beef, yogurt, breakfast shake, French toast, and ground turkey. Fruits include pear, raspberries, pineapple, blackberries, banana, and plums. Vegetables include potatoes. Starches include bread, oatmeal, pancakes, and dry cereals. Liquids include orange juice, apple juice, lemonade, milk, and water. Dominic's father expressed a desire for him to expand his intake to include more soft foods. NORM REFERENCED ASSESSMENT: Mr. Solis completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver report questionnaire for children aged 6 months to 7 years. Dominic received an overall rating of โ€œno concern,โ€ however, he received โ€œhigh concernโ€ in the area of problematic mealtime behaviors. These concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Dominic was seated and self?-ยญfed his preferred food items utilizing a spoon (vegetable soup and pureed eggs and bananas). Non-preferred foods offered were sopita de estrellas and rice. Dominic attempted a small portion of rice and exhibited gagging followed by emesis. With clinician support and implementation of exercises to reduce the gag reflex, Dominic was able to consume an additional three spoonfuls of sopita de estrellas. SWALLOW/ORAL MOTOR EVALUATION: Dominic demonstrated significantly decreased oral motor skills, including a munching bite pattern, open?mouth chewing, minimal tongue lateralization, and oral residue. Mr. Solis does not report unexplained fevers or a history of aspiration pneumonia. Dominicโ€™s pharyngeal/esophageal swallow function should be monitored by his treating clinician to ensure safety and determine if further testing is warranted. At this age, a child should demonstrate a more developed rotary pattern and management of thicker purees and soft solids. The gag reflex should be active, but should be less sensitive. Dominicโ€™s oral motor skills are NOT WITHIN FUNCTIONAL LIMITS for his age and/or feeding safety. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 45? minutes to address Dominicโ€™s CHRONIC PFD and ORAL DYSPHAGIA. Dominic is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. Feeding therapy should also target improving Dominicโ€™s oral motor skills for safe and effective feeding/swallowing. In addition, food chaining strategies should be implemented to expand Dominicโ€™s food repertoire by building on existing preferred foods. Without intervention, Dominic may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Mr. Solis will be responsible for implementing the recommended Home Management Program to reinforce therapeutic strategies and support progress between sessions.

Dominicโ€™s oral motor skills are NOT WITHIN FUNCTIONAL LIMITS for his age and/or feeding safety. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 45 minutes to address Dominicโ€™s CHRONIC PFD and ORAL DYSPHAGIA. Dominic is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. Feeding therapy should also target improving Dominicโ€™s oral motor skills for safe and effective feeding/swallowing. In addition, food chaining strategies should be implemented to expand Dominicโ€™s food repertoire by building on existing preferred foods. Without intervention, Dominic may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Mr. Solis will be responsible for implementing the recommended Home Management Program to reinforce therapeutic strategies and support progress between sessions.
Feeding Rehabilitation Potential: Good. with consistent Speech Therapy and parental support. Prognosis is good with consistent feeding therapy, parental support, and parental implementation of therapeutic strategies outside of the therapy room. Dominic will be discharged when his feeding inventory is clinically deemed adequate for his age, nutritional needs, and safety.

You can start placing 1-2 soft cubes on Dominic's plate alongside his puree/soup (for now, do not add the soft cubes into his puree/soup), such as cheese cubes, banana cubes, pasta cubes, avocado cubes, cubes of vegetables, peas, potato cubes, etc. Anything soft and easy to break down (if you can squish it easily between two fingers, it's appropriate). You can model to him how you eat the cubes by sending it to your side teeth and crunching it down while reminding him to breathe through his nose. This will teach him how to move his tongue side to side, oral awareness. reduces panic when texture touches his tongue and builds tolerance without activating the gag. We're not asking him to eat chunks yet. We're teaching his mouth to tolerate them being present so his gag reflex can calm down over time. If he gags when interacting with soft cubes, you can stop in that moment, but continue to try with each meal. Pushing through the gag teaches the nervous system that food = danger, which makes the gag reflex stronger next time. The goal is to keep experiences below the gag threshold. Eventually, with time and practice, we will be able to tolerate soft cubes.

5. No breathing issues, does get 8-10 hrs of sleep, might wake up once to use the restroom, does not snore, no enlarged tonsils that we know of.
6. No daycare, no school, no therapy.
7. English mostly.
8. No vision or hearing issues.
9. Basic concerns with his speech is his inability to communicate with us. He tries using 1-3 words to tell us something particularly what he wants to watch.
I would like for him to be able to tell me how his day was or what he did in school or hiw his day was by the time he does start school.

Meaning one week will be for feeding, the following for speech, and so on. Speech sessions are 45 minutes, and feeding sessions are 30 minutes. Our current appointment is only for 30 minutes, so I wanted to ask if, starting next week, I can permanently see Dominic on Tuesdays at 2:45 pm, for feeding and speech. That day and time will allow us to have our full 45-minute sessions. Let me know if this time works for you
