Occupational Background History: Laker is a 2.11 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression due to sensory processing difficulties and NO seasonal allergies.

Child will stop task due to difficulty processing what was asked limiting his engagement. Per parent report, child demos decreased emotional regulation, body awareness and safety; child demos difficulty with appropriate interactions with sibling due to decreased organization of behavior which leads to excessive meltdowns hindering his participation in ADLs. PDMS-2 - Per standardized assessment scores Laker falls below average in one subtests assed on the PDMS-2 evident by below average age equivalency scores ranging from 15 months to 27 months compared to his chronological age of 35 months, placing him at least (-2) standard deviations behind his peers. Laker demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Subjective: Patient was friendly and cooperative,
Assessment of Goals/Response to Treatment: Laker increased bilateral coordination skills when completing lacing beads IND and when using a spatula to stack pancakes during "pancake pile-up" while stabilizing the plate with his non-dominant hand with only MIN cues for compensations. Laker also stabilized the page when engaged in tracing lines with MIN verbal cues. He required MOD A to trace a cross and X to increase VMI and FM precision skills. Laker increased social reciprocity when completing a turn taking activity with only MIN verbal cues to wait for his turn. He initiated turn taking during 2/4 trials IND. Laker also commented and requested items during activities and followed 2-3 step verbal directions with only MIN A. He also required MOD A to sequence a visual 3 step-by-step activity to increase his ability to sequence multistep tasks.
