Speech Background History: Andrew is a 1 year 9 month old male who was referred for a speech and language evaluation by his pediatrician for concerns regarding speech delay. Andrew was accompanied to the evaluation by his mother, who served as the informant. Mother stated that due to complications during the pregnancy, Andrew was born at 35 weeks and with low birth weight. According to mother, Andrew failed the first two hearing screenings at the hospital, and passed the third time. Although mother has stated that Andrew has never had an ear infection, she has felt that Andrew suffers from allergies. It was recommended mother seek a hearing evaluation for Andrew to rule out hearing loss and/or excessive fluid in the ears as Andrew sleeps with a bottle of milk every night. Mother was educated on the risks involved and the higher percentage of ear infections that occur when children fall asleep with bottles. Per mother’s report, Andrew has 32 words, but uses them inconsistently and the words are “not clear” to unfamiliar listeners. Based on clinical observations, Andrew’s current communication was characterized as no functional words, two instances of an open mouth vocalization, reaching/grabbing/taking items of desire to express his communication wants/needs. Andrew has been diagnosed with a profound to mild mixed receptive-expressive language disorder, where his expressive (profound) language skills were more severe than his receptive (mild) language skills. Andrew would benefit from speech therapy targeting the following: vocalizing two different consonant sounds, babbling two syllables together, identifying familiar objects from a group of objects, following one step commands, relational play, and imitating environmental/non-speech sounds. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the assigned treating therapist will reach out to you to discuss beginning treatment. Please let me know if you have any questions. Thank you.
