Speech Background History: Noah is a 0 year 10 month old male who was referred for a speech and language evaluation by his pediatrician. Noah was accompanied by his mother, Sara Ramos, who served as the informant for the evaluation. According to mother, Noah was born 3 weeks early and was found to have jaundice, but either factor did not call for treatment nor a prolonged hospital stay. Mother stated that Noah is quiet and does not β€œbabble as much.” However, mother does not feel that his hearing is an issue as he passed his newborn hearing screener and reacts to all soft and loud noises. Mother did state that Noah does not care for loud noises, and will cover his ears if something is too loud. Mother claims that Noah is β€œusually very calm” and will whine to communicate his wants/needs. Noah said his first word β€œmama” on December 31st, and inconsistently repeated β€œmama.” Based on clinical observations, Noah did not verbalize words, but he whined once to get mother’s attention. Noah mouthed select items as he explored the therapy materials. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Noah’s receptive and expressive language skills. Speech Medical History: Per parent report, medical history is not remarkable. According to mother, Noah has never had a fever or ear infection.

Speech Communication Strengths: Appropriate use of eye contact, Auditory and visually alert and attentive,

Speech Background Summary: The evaluation took place on the therapy room floor as Noah does not walk and had difficulty transitioning from the sitting up position to standing while holding on to something. Noah required numerous prompts to engage with the tasks. Overall, Noah’s pragmatic skills presented with more weaknesses than strengths at this time.

Noah received the following standard scores on the PLS-5: Auditory Comprehension 90 (raw score: 13, percentile: 25, age-equivalent: 0 years 9 months), Expressive Communication 57 (raw score: 6, percentile: 1, age-equivalent: 0 years 2 months), and Total Language Score 72 (raw score: 19, percentile: 1, age-equivalent: 0 years 5 months). According to the standard scores on the PLS-5, Noah presented with PROFOUND EXPRESSIVE LANGUAGE DISORDER, where his expressive (PROFOUND) is more severe than his total language (MODERATE) score. Noah would benefit from speech therapy services targeting the following age-appropriate concepts: vocalizing when talked to, moving arms and legs during vocalizations, imitation of facial expressions, vocalizing different vowel sounds, combining sound (vowel-vowel, consonant-vowel, or vowel-consonant), and taking multiple turns vocalizing. 4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained, 1st words, combined word?
5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?
6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history?
7.) Which language(s) is the child exposed to regularly?
8.) Vision or hearing issues?
9.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

He would greatly benefit from speech therapy services targeting the following age-appropriate concepts: vocalizing when talked to, moving arms and legs during vocalizations, imitation of facial expressions, vocalizing different vowel sounds, combining sound (vowel-vowel, consonant-vowel, or vowel-consonant), and taking multiple turns vocalizing. Once the office obtains all the necessary signatures, and the insurance approves the paperwork, the treating therapist will reach out to you to begin services.
