Feeding Background History: Calloway is a 10-month-old male who was referred by his pediatrician for concerns regarding feeding difficulties. Calloway’s mother and father served as the informants for this evaluation. Mrs Jones reported that Calloway's feeding difficulties began at 1 month old. Calloway had difficulty with latching and had severe reflux as an infant. Mother reported his current diet consists of only formula, purees, and meltables. Mr. Jones reported that Calloway will often gag during mealtimes and, on two occasions, choked while eating meltables. Calloway's primary source of nutrition is formula. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Calloway’s feeding/swallowing skills.
Feeding Medical History: Per parent report, medical history is not remarkable. Calloway was born at 39 weeks via C-section. Calloway was diagnosed with laryngomalacia at 1 month old. Mother reported Calloway had difficulty latching and experienced severe reflux during breastfeeding. He later transitioned to bottle feeding, where reflux persisted but was significantly reduced. Calloway has undergone two MBSS evaluations- one in Chicago at 3 months of age and another in El Paso at 9 months. Both studies yielded appropriate/within-normal-limit results with no signs of aspiration.

Feeding Diagnosis/Impressions: Calloway is a 10-month-old male who was referred by his pediatrician for concerns regarding feeding difficulties. Calloway’s mother and father served as the informants for this evaluation. Mrs Jones reported that Calloway's feeding difficulties began at 1 month old. Calloway had difficulty with latching and had severe reflux as an infant. Mother reported his current diet consists of only formula, purees, and meltables. Mr. Jones reported that Calloway will often gag during mealtimes and, on two occasions, choked while eating meltables. Calloway's primary source of nutrition is formula. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Calloway’s feeding/swallowing skills. Calloway was born at 39 weeks via C-section. Calloway was diagnosed with laryngomalacia at 1 month old. Mother reported Calloway had difficulty latching and experienced severe reflux during breastfeeding. FOOD INVENTORY: Calloway’s mother also completed a food inventory checklist in which she reported he currently accepts 25­-30 solids and 2 liquids. It is important to note that Calloway will only consume the following food items in a pureed or mashed form. The following are Calloway’s accepted food items: PROTEINS­ Salmon, ground beef, ground turkey, peanut butter. STARCHES­ crackers, vegetable chips, rice, mashed potatoes, oatmeal. FRUITS­ Apple, blueberries, pear, watermelon, strawberries, banana, pumpkin. VEGETABLES Squash, zucchini, carrots, sugar snap peas, peas, kale, broccoli, corn LIQUIDS­ Milk and water. His mother would like him to soft table foods and higher consistencies. CRITERION REFERENCED ASSESSMENT: Mrs. Jones completed the Pediatric Eating Assessment Tool (PediEAT), which is a questionnaire used to subjectively assess Calloway’s feeding behavior. Calloway received an overall rating of "High Concern". Calloway received "High Concern" ratings for physiologic symptoms and selective/restrictive eating. Calloway's ratings on the PediEAT are consistent with parent reports and with his performance in the evaluation and diagnosis. MEALTIME FEEDING OBSERVATION: Calloway was seated in a high chair with appropriate 90?-90?-90 positioning and was fed by his mother using a spoon and by hand. Calloway accepted all food trials. Calloway was observed to gag with purees. Calloway self-fed meltables and purees using his fingers. SWALLOW/ORAL MOTOR EVALUATION: The consistencies trialed included veggie puree and meltables. Calloway demonstrated significantly decreased oral motor skills, including a phaisc bite pattern, open?mouth chewing, minimal tongue lateralization, and oral residue. Mrs. Jones does not report unexplained fevers or history of aspiration pneumonia. Calloway’s pharyngeal/esophageal swallow function were evaluated at 3 and 9 months old where studies yielded appropriate/within-normal-limit results with no signs of aspiration. However, pharyngeal/esophageal function should continued to be monitored by his treating clinician to ensure safety and determine if further testing is warranted. At this age, a child should demonstrate a more developed munching pattern and management of thicker purees and soft solids. Gag reflex should be active but should be less sensitive. Calloway’s oral motor skills are NOT WITHIN FUNCTIONAL LIMITS for his age and/or feeding safety. RECOMMENDATIONS: Feeding therapy is highly recommended at this time at the time of once per week for 30 minutes, and should target increasing Calloway’s tolerance to non-preferred foods via steps to the eating hierarchy and desensitization methods. At this age, Calloway should be able to consume solids of various consistencies to include purées, soft mechanicals, and hard munchables. Calloway should also consume foods from all nutrition food groups and have at least 10 foods per nutrition food group (proteins, starches, fruits, and vegetables) in his food inventory. Feeding therapy should also target improving Calloway’s oral motor skills for safe and effective feeding/swallowing. Parent education and mealtime behavior will also be targeted to support feeding development. Without feeding therapy, safety risks include but are not limited to lack of appropriate nutrition due to restricted diet characterized by foods of inappropriate nutritional value and food jagging on limited foods in food repertoire. Parents will be responsible for the Home Education Program (HEP).

We have been trying to give him chunkier purées as recommended after 6 months old and the dissolvable crackers etc... he has continuously choked or gags to the point of projectile vomiting .. he got his swallow test done came back normal so now trying to see if it is a texture issue? Or chewing possibly?

The bedside swallow evaluation and skilled meal feeding observation revealed that Calloway presents with oral dysphagia ( difficulty swallowing that starts in the mouth, involving problems with lips, tongue, jaw, or saliva, making it hard to form and move food (bolus) to the throat).

You can also start offering him hard munchables during his snack/mealtimes. A hard munchable is a food that Calloway can hold independently, and when inserted into his mouth, it will not break apart and is long enough that a portion of the food remains outside of the mouth. I will be sending you a document with more information and examples.

01-15-2026 11:11 AMAndrea RodriguezStart introducing chewy food like chips, pretzels, freeze-dried fruit and also hard mechanicals like steak, chicken cubes, fish skin. Mix textures together. Keep modeling mouth movements. Work on desensitizing his lips to reduce tongue protrusion reflex.

1. Avoid placing the spoon straight onto the tongue. Instead place the spoon downward onto the middle of the tongue, then apply gentle downward pressure before moving.
2. Touch the upper lip with spoon or food and wait for lip closure then place the spoon in.
3. Keep introducing the chewy tube for jaw stability
4. Let's start reducing thin purees and instead use thicker purees and/or mashed food. Heavier bolus gives better input.
5. Avoid stimulating tongue tip which can trigger protrusion.
