Occupational Background History: Jordan is a 8.0 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 39 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and does have seasonal allergies.

Per parent report child demos hyper-sensitivity to car rides, upside down activities, playing outside BOS, demos increased sensitivity to sound and textures, demos difficulty with taking a bath and tooth-brushing; per parent report child also demos difficulty with emotional regulation decreasing his ability to participate in daily activities; during clinical observation it was noted child did not integrate primitive reflexes: STNR, SGR, rooting, palmar and TLR, decreasing his bilateral coordination, hypersensitivity to sounds and textures, difficulty maintaining posture and utilizing wider BOS when ambulating, and decreased postural control. Jordan demonstrates deficits in primitive reflex integration, manual dexterity, manual coordination, sensory processing, emotional regulation and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parent(s), along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Jordan completed 3 sets of 10 repetitions of seated marches and supine leg raises with max A. Demonstrated fair control with occasional compensatory trunk movement. He was prompted to begin 3 familiar tasks ( picking up materials, starting simulated grooming activity). Initiated tasks within 45–60 seconds in 2/3 trials with mod verbal prompting; required moderate cues for 1 trial. He then engaged in activities requiring coordinated use of both hands (ball passing, midline crossing tasks). Demonstrated improved coordination with 75% accuracy, requiring occasional verbal and visual cues. Practiced shoe-tying using adapted method. Completed 4 trials with moderate assistance, showing improved sequencing but continued difficulty with loop formation and tightening. Participated in simulated toothbrushing task using visual schedule. Completed sequence with mod verbal cues, demonstrating appropriate grasp and movement, though missed 1–2 steps without prompting. Jordan would continue to benefit from skilled ot services to improve short term and long term goals.
