Occupational Background History: Legend is a 6.9 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born full term. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does have history of aggression due to sensory processing difficulties and no seasonal allergies

Occupational Background Summary: Legend displays ideational praxis difficulties as he is unable to develop an idea for action and plan, organize, and execute unfamiliar motor movements, when participating when feeling overwhelmed or over­stimulated during ADLs or when completing multi­step tasks. Pt demos decreased EF skills which are hindered by his sensory processing difficulties. Per parent report at times child demos difficulty with tolerance of functional activity demands, becoming overwhelmed and unable to complete task due to decreased emotional regulation skills; there is possible vestibular insecurity and increase hyper sensitivity to environmental stimuli.
Occupational Diagnosis/Impressions: Legend was assessed via: BOT-2 Standardized Assessment, Clinical Observations and Parental Report Per standardized assessment scores falls well below average in both subsets assed on the BOT-2 evident by below average age equivalency scores ranging from 5:4 years to 10:11 years compared to his chronological age of 6:9 years, placing him at least (-2 to -1) standard deviations behind his peers. Legend demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Mother verbalized understanding of treatment plan and agreed to the initiation of skilled therapy.
