Occupational Background History: Mia is a 11.8 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 41 weeks gestation. Child did not experience any complications at birth, does not have any visual (does wear glasses) or hearing deficits. Child does not have history of aggression and no seasonal allergies.

Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.

Assessment of Goals/Response to Treatment: Mia participated in therapeutic activities targeting fine motor skills, bilateral coordination, and hand strengthening. She initiated familiar task with moderate assistance , requiring verbal and occasional physical prompts for sequencing and task completion. Demonstrated bilateral coordination by using both hands to open various containers (screw-top lids, snap containers). Required intermittent assistance for grip adjustment and force modulation. Engaged in fine motor manipulation tasks involving small objects (beads, small game pieces). When using tongs, Mia showed a preference for stabilizing with the ulnar side of the hand, indicating decreased radial-side strength and precision. Completed hand exercises ( finger flexion/extension, squeezing warm therapy putty, gentle range-of-motion activities) to support circulation and manage symptoms associated with Raynaud’s phenomenon. Mia would continue to benefit from skilled ot services to improve short term and long term goals.
