Per parent report, Mathis attends physical therapy due to his “tip toe” walking. Speech Background Summary: The evaluation took place on the therapy room floor as Mathis. Mathis required constant redirection to tasks, as well as numerous prompts to cooperate. Overall, Mathis’ pragmatic skills presented with more weaknesses than strengths at this time.

According to his mother, Mathis has been delayed to meet milestones for his age, but could not recall the dates of his milestones. Per parent report, Mathis attends physical therapy due to his “tip toe” walking. Mother stated that Mathis is regularly exposed to French and English, but Mathis has only been heard to use “mama” and “dada” inconsistently, crying, yelling, and/or whining to get the attention of his mother, and/or will reach and grab the items his wants/needs. Based on clinical observations, Mathis did not verbalize words, but babbled “baba”, whined, grunted, yelled, and/or grabbed/reached for desired objects to communicate wants/needs. The following behavioral observations were also noted: INCONSISTENT attending skills, INTENT to communicate, WITHDRAWN social interaction, IMPULSIVE response rate, ACTIVE level of activity, POOR awareness of others/environment, and RESISTS to cooperate. A formal oral mechanism exam could not be completed due to Mathis’ difficulty following instruction. It is recommended that Mathis’ treating clinician continue to monitor his oral cavity during treatment and perform an oral mech exam when patient permits. According to mother, Mathis snores and keeps an open mouth posture when sleeping, but will breathe out of his nose while awake. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Mathis’ receptive and expressive language skills. Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative for the duration of the therapy session. Parental counseling focused on the implementation of a home management intervention program to facilitate the generalization of skills to the home environment; parents demonstrated independence in incorporating these strategies into daily routines. Therapeutic activities targeted the acquisition of age-appropriate pragmatic and play skills. The patient demonstrated progress by engaging in relational/functional play-based tasks, such as stacking blocks and driving cars, in 4 out of 10 opportunities with decreasing multi-sensory cues. Expressive language was addressed by targeting imitation of environmental and non-speech sounds, with the patient achieving 60% accuracy during structured play activities. Further intervention focused on improving functional vocabulary, wherein the patient imitated functional actions, gestures, and/or words modeled by the clinician in 3 out of 10 opportunities. Receptive language tasks focused on comprehension of instructions for activities of daily living.
