Speech Background History: Jeremiah is a 1 year 2 month old male who was referred for a speech and language evaluation due to speech delay and bilateral hearing loss. Jeremiah was accompanied to the evaluation by both her parents, and both served as informants. According to mother, Jeremiah was born at 37 weeks and required an 11 day NICU stay due to breathing issues. During that time, Jeremiah was administered the newborn hearing screener and he failed the exam twice. A follow-up hearing evaluation was completed on January 21st, 2025 by audiologist Dr. Manuel Hita, and ENT Dr. Jonathan Chiao when Jeremiah was about 4 months old via natural sleep Auditory Brainstem Response (ABR). The evaluation found that Jeremiah presented with a moderate to moderately-severe hearing loss in the left ear, and at least a severe to profound hearing loss in the right ear. Bone conduction threshold and tympanograms suggested Jeremiah’s hearing loss was sensorineural on the right with a possible mixed component with the left ear. Jeremiah was fitted for a hearing aid for the left ear with the recommended wear time of 8 hours per day. Per the most recent medical report, Jeremiah was found to have Microtia of the ears, left facial asymmetry, and right cochlear nerve aplasia. According to mother, Jeremiah usually wears his hearing aid 8 hours, ranging from the morning until early evening, only removing the hearing aid during times in the car to prevent Jeremiah from self-removal and mouthing the device. Jeremian was given the Preschool Language Scales Fifth Edition (PLS-5) to evaluate his receptive and expressive language skills. Based on Jeremiah’s reported language background and language input/output, the language of intervention will be English.

Currently sounds and vocalizations consist of sounds to gain attention and cries when frustrated or not knowing what is happening. Speech therapy is warranted at this time, as Jeremiah becomes more aware and comfortable with sounds within his community and daily routine. Jeremiah needs to be aware of sounds and localize as to where and who may be calling him, or what may be making noises. Jeremiah needs strategies as well as prompts and cues to listen and learn those sounds around him. He will need to understand types of voice and intonation from excited/happy, to danger and needing assistance, and demonstrating understanding of 'wait' and 'no'. 1.) Name and DOB
2.) Any complications with pregnancy or birth?
3.) Any major illnesses (i.e., ear infections) or accidents since last check up? Any medical diagnoses (i.e., Autism, ADHD, Down syndrome, etc.) or allergies (i.e., medications, foods, seasonal, etc.)?
4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained, 1st words, combined word?
5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?
6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history?
7.) Which language(s) is the child exposed to regularly?
8.) Vision or hearing issues?
9.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

2.) I did have a high risk pregnancy due to the fact that I was diabetic. As far as birth, Jeremiah was in the NICU for 11 days. He has trouble breathing when he was born.
3.) no major illnesses or accidents. No medical diagnosis or allergies
4.) crawl 9 months; sat up March; stood up 8 months ; walked 13 months; fed himself 11 months.. still learning how to use utensils.
5.) does sound like he's congested when he sleeps, he does get between 8-10 hours of sleep. He does wake up a few times throughout the night..at least 1-2 times. Sometimes he does snore and sometimes he does sleep with his mouth open.
6.) no education history
7.) English language
8.) he does have hearing issues
9.) because Jeremiah does have hearing loss, I think his speech and language is a little behind. I've tried to incorporate sign language when I can and I feel like he's picking it up but very little. He says no actual words but does talk a lot.
