She continues to demonstrate distress when told “no” or asked to “wait,” often pulling items from the clinician or crying. However, she has demonstrated improvement in her attention to tasks, as she can complete activities with moderate prompting and remain seated for longer periods. A typically developing child of Izkaira's age should have an expressive vocabulary of approximately 1,000 words and produce sentences of 3-4 words. The Preschool Language Scales Fifth Edition (PLS-5) Spanish was used to re-evaluate Izkaira’s receptive and expressive language skills. LANGUAGE BACKGROUND: LANGUAGE INPUT: Izkaira's mother reported that Izkaira is exposed to both Spanish and English in her home environment. Occupational Assessment Summary: Izkaira was assessed via: PDMS-2 Standardized Assessment, Clinical Observations, Parental Report PDMS-2 - Per standardized assessment scores Izkaira falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores ranging from 20 months to 26 months compared to her chronological age of 43 months, placing her at least (-2) standard deviations behind her peers. Gain of +6 months in grasping and +4 months in VMI. Izkaira demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 10 seconds before attempting to flee. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection. Izkaira demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to parents, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Parents verbalized understanding of treatment plan and agreed to the initiation of skilled therapy.

Occupational Progress to Date: Izkaira demonstrates consistent progress with occupational therapy services evident by meeting 100% of previous goals with at least 70% accuracy, improved standardized assessment scores on the PDMS-2, per clinical observations and parental report. Izkaira is diagnosed with a profound mixed expressive-receptive language disorder. Izkaira would greatly benefit from speech therapy services that focus on the following concepts: following 1step commands with common objects, joint attention, imitating environmental sounds, imitating functional signs, transition between activities, and engaging with the clinician during play. At home, you can start modeling the "more" sign when she is requesting an item. You can also target joint attention by placing items on your head and making exclamatory sounds like "Achu!". This reinforcers that back and forth communication while playing. Once the office has processed the report, obtained the needed signatures, and insurance approves the report, the treating clinician will reach out to you to schedule your first session. We worked on turn taking with a fine motor task; she was able to transfer throughout 5 activities with cleaning up after using; she practiced scissor skills, legos, and 4 different textures: slime, rice, oatmeal and shaving cream; I also did vibration exercise outside at both of her cheeks to get that rooting reflex and she tolerated 3xs and she even asked for more; I got a few smiles and giggles from her today!
