Speech Medical History: Gia was born at 37 weeks via a vaginal delivery. Mother stated that she did not experience complications or concerns during her pregnancy with Gia. Mother reported that Gia also did not spend any time in the NICU. Gia has not experienced any major illnesses or accidents that required hospitalization. Gia experienced 1 ear infection in January 2025 and required antibiotics to treat the ear infection. Gia was evaluated on 10/02/2025 by Dr. Shutte for ASD. Mother stated she will receive the results in 2 weeks.

Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother’s report child was born at 37 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and no seasonal allergies.

Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Gia Sophia was assessed via: PDMS-2 Standardized Assessment, Clinical Observations and Parental Report PDMS-2 - Per standardized assessment scores Gia falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores ranging from 14 months to 21 months compared to his chronological age of 33 months, placing her at least (-2 to -1) standard deviations behind her peers. Gia demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Subjective: Patient was friendly and cooperative,

Assessment of Goals/Response to Treatment: Gia engaged in a VMI task while in quadruped with MOD A for compensations while engaged in crossing midline to assemble a sequencing hamburger to increase bilateral use of hands in ADL completion. She also manipulated play-doh with tools with MOD A while stabilizing with her "helper hand" to increase hand dexterity, strength, and bilateral coordination skills. Gia demonstrated increased social reciprocity skills when engaged in a turn taking activity with only MIN verbal cues to initiate turn taking. She also followed 2 step verbal directions with MIN A and repeated words/commented IND to demo increased social reciprocity skills. Gia was introduced to alternate forms of sensory input (vestibular and proprioceptive) and demonstrated MOD aversion via avoidance. It required MOD A via modeling and verbal cues for encourgament for her to initiate vestibular/proprioceptive sensory activities (rolling on a ball, crawling through a tunnel, jumping on a trampoline) to increase sensory integration and self-regulation skills via sensory input required for sustained attention to functional tasks. She also demonstrated signs of retained ATNR/STNR reflexes when showing compensations when engaged in tasks in quadruped positioning. To continue with addition of gross motor components to functional activities to further integrate primitive reflexes for motor planning and bilateral coordination skills.

I needed a bit more time to think about what we wanted to do regarding Gia and her speech therapy. It was my mistake to let Kaitlin know what was going on, I should have spoken with you directly instead. The last thing I wanted was to make this a bigger deal or to make you feel any type of way. I truly respect you and the work you do you are amazing. I just don’t feel that Gia is going to learn if she is crying the entire time, as I know she would continue to do so. With her, I believe we need to start more gradually until she understands what’s going on. There have been a lot of changes for her recently, and my main goal was for her to come here happy and leave the same way, not upset. She starts school next week, and I’m hoping she will be attending in the afternoon. I’ll keep you updated, and if that’s the case, we would like to continue her speech therapy with you, if you’re okay with that. I apologize again, and thank you for your understanding. ST is different than OT, in the manner that it tends to be more structured and it isn't as physical as OT. I would honestly suggest giving it more time, but if you still feel that you want a different therapist, you can ask the front office.

I needed a bit more time to think about what we wanted to do regarding Gia and her speech therapy. It was my mistake to let Kaitlin know what was going on, I should have spoken with you directly instead. The last thing I wanted was to make this a bigger deal or to make you feel any type of way. I truly respect you and the work you do you are amazing. I just don’t feel that Gia is going to learn if she is crying the entire time, as I know she would continue to do so. With her, I believe we need to start more gradually until she understands what’s going on. There have been a lot of changes for her recently, and my main goal was for her to come here happy and leave the same way, not upset. She starts school next week, and I’m hoping she will be attending in the afternoon. I’ll keep you updated, and if that’s the case, we would like to continue her speech therapy with you, if you’re okay with that. I apologize again, and thank you for your understanding.

Hopefully, school will assist with Gia getting use to coming to ST. It does take time for most children to come to therapy. What if we try Gia coming into the session by herself? Sometimes that helps the school decide to put the child in the afternoon so that the child can still receive private therapy services. Yes, they will provide therapy to Gia at school, however, the therapy will be in a group setting. The direct therapy we offer helps children make progress faster since the child receives undivided attention and therapy. Just something you might want to consider. :)
