Occupational Background History: Jaaziah is a 8.3 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother’s report child was born full term. Child did not experience any complications at birth (refer medical history), does have visual deficits and no hearing deficits. Child does have history of aggression and no seasonal allergies.

Occupational Background Summary: Jaaziah demos increased motivation to participate in activities, however becomes frustrated easily, decreasing his ability to independently complete tasks. Per parent report child demos difficulty with tolerance to functional activity demands, hypersensitivity to sounds, decreased emotional regulation and sensory processing, and decreased ability to motor plan and execute movement for multi-step tasks; parent conveyed concerns with ODD and possible schizophrenia which decrease child’s safety and body awareness; OT to engage child in functional activities and structure to increase safety and emotional regulation, with further testing recommended from other pediatric professionals in the future if necessary. Jaaziah demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Jaaziah was assessed via: Standardized Assessment, Clinical Observations and Parental Report BOT-2 - Per standardized assessment scores Jaaziah falls well below average in one subset assed on the BOT-2 evident by below average age equivalency scores ranging from 5:6 years to 6:8 years compared to his chronological age of 8:3 years , placing him at least (-2 to -1) standard deviations behind his peers. Jaaziah demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Occupational Progress to Date: Jaaziah demonstrates consistent progress with occupational therapy services evident by meeting 100% of previous goals with at least 70% accuracy, improved standardized assessment scores on the BOT­-2, per clinical observations and parental report. Jaaziah is able to transition between all tasks during sessions IND or with a single verbal cue. He has completed multistep (4-­6) tasks with only up to MIN A to demo increased problem solving and executive functioning skills. He is able to initiate, sequence, and complete all tasks to demo increased activity tolerance and attention and he maintains a calm and positive demeanor during challenging tasks with only MIN cues at times for assistance to initiate calming strategies. Jaaziah is able to tie his shoes IND and he has shown the ability to sequence novel 6 step tasks such as building a lego structure IND. Per parent report Jaaziah has had decreased the amount of outbursts at home but he has still required MOD assistance to increase his self­-regulation demonstrating emerging skills in his ability to utilize coping strategies. He also sustained attention to each task until completion with no cues for redirection to demo increased self-regulation/attention. Jaaziah participated in a social story activity with MOD cues for assistance during 2/4 trials to develop ideas for action to increase problem solving and emotional regulation skills. Jaaziah attended to and independently completed a 4 step activity (pancake pile-up), to improve attention and task sequencing skills. As long as you let us know if you need to cancel a session due to illness or whatever other reasons. The reminders are sent out by the system to attempt to be helpful but I can understand having 2 children with multiple therapies the reminders can become frustrating. I will bring up your concerns with the company and I appreciate your feedback.

He's really big on eating a bunch of junk food like chips cookies candies, and I told him that if he wasn't careful that he was gonna get fat and I did tell him his dad used to be fat.... this morning his pants didn't fit very well because they are adjustable waste so I had to let them loose a little bit and I was trying to explain to him that he's not fat and that normal part of getting taller is also gaining healthy weight, but I seem to just be making it worse 🙈

I spoke with Medicaid this morning and wanted to clarify. Jaaziah has transitioned from traditional Medicaid STAR to Medicaid STAR Kids (for children on SSI/disability). His traditional Medicaid ended January 31, STAR Kids began February 1, and although Superior will remain his plan, Superior coverage under STAR Kids will not be active until March 1 so as of now he is straight Medicaid star kids, idk if that makes a difference with authorization needs for this month.

Joshua will follow the same process. He will transition from traditional Medicaid to Medicaid STAR Kids on March 1, and Superior is expected to become active for him on April 1.

This appears to be the standard transition timeline rather than a loss of coverage. Please let me know if you need anything from me in the meantime

03-17-2026 04:52 PMKaitlin MarslandHe had a really good day, I think today was one of the first times he was actually open with me about things so we discussed that he will have his re-eval to see if he still needs therapy and I asked him if he felt like he needed it and he initially said no so I asked how he would feel if he didn't come anymore and he said that would make him feel pretty frustrated and I asked why he likes to come to therapy and he told me it is because its more calm here. He said at school its loud and kids annoy him because they stand up when they're finished their work and then he can't see the board and at home Joshua can get on his nerves sometimes and he said he likes to have space to himself where its more calm sometimes so I told him that's wonderful he recognizes what he needs to regulate and that maybe he needs to express that at home with you. He said he feels like he is in control of his emotions but people think he isn't and that's frustrating. So I was really happy with all his answers because he was honest and didn't avoid the conversation. He completed some activities with me, was cooperative and engaging, and won a little prize from the scavenger hunt :)

He is to my understanding the biggest distraction of the in the class.
