Speech Background History: Jace is a 6 year 11 month old female who was referred for a speech and language evaluation by her pediatrician and parental concerns with language. Jace was accompanied to the assessment by her mother, who served as the informant. According to mother, Jace was diagnosed with autism spectrum disorder when Jace was 4 years old, and then with attention-deficit/hyperactivity disorder (ADHD) when Jace was 6 years old. Per parent report, Jace has been receiving speech therapy services since she was 18 months old due to her initial non-verbal diagnosis. There is also a history of applied behavioral analysis (ABA) therapy, but mother removed Jace from services when Jace began kindergarten in August of 2024. Since the family relocated to El Paso, Jace has been receiving speech therapy in the public school once a week. However, mother is concerned with Jace’s struggle to understand and use grammatical sentences. Based on clinical observations, Jace’s current communication was characterized as simple grammatical and non-grammatical sentences, with inconsistent age-appropriate vocabulary, and β€œbaby-talk” to communicate her wants/needs. Speech Medical History: Jace was diagnosed with autism spectrum disorder at 4 years, and then with attention-deficit/hyperactivity disorder (ADHD) when Jace was 6 years old. Jace was also found to have a tongue tie, but per recommendation of her dentist, removal of the tie has been pushed until Jace is older due to Jace’s resistance to follow through with doctor recommended stretches post-surgery.
Speech Medications: Jace consumes 10 mg of Adderall XR once a day.

Jace participated in the assessment virtually alongside her mother. During the assessment, she appeared to be oriented and engaged; however, she demonstrated frequent moments of fatigue. She benefitted from short breaks and snacks during the assessment to facilitate adequate participation. Assessment of Goals/Response to Treatment: Jace presented as alert, cooperative, and engaged throughout the session, demonstrating good rapport and willingness to participate in therapeutic activities. Moderate progress was noted in the area of receptive language, specifically in following multi-step directions. Jace successfully followed 2-step sequential commands containing two elements with 60% accuracy during structured tasks, exceeding the established goal of 40% accuracy and indicating improved auditory processing and comprehension. Intervention will now focus on increasing the complexity of commands and promoting generalization. In contrast, expressive language skills remain an area of significant need. When presented with tasks requiring grammatical judgment, Jace demonstrated an ability to determine if a sentence was correct, scoring 60% accuracy (6/10 trials). Similarly, when targeting narrative skills, Jace sequenced a story and provided relevant details, in 4 out of 5 trials toward the goal. The patient required maximal clinician prompting to generate specific ideas related to visual stimuli and could not yet formulate a cohesive narrative structure. Future sessions will target these foundational expressive language deficits, focusing on explicit instruction in basic syntactic structures and story grammar elements using a multi-sensory approach with visual supports to build prerequisite skills.

Jace gets approximately 10.5 hours of sleep. She only wakes through the night if her normal routine/environment changes. It typically takes her about 2 weeks to get readjusted to not waking through the night. Jace rarely snores and sleeps with her mouth open.

She was diagnosed with a severe to moderate mixed receptive-expressive language disorder, where her expressive language is more severe than her receptive language skills. As mention on Friday, her goals will consist of identifying grammatical/non-grammatical sentences, following 2-part directions, improve auditory processing skills, and discriminate and identify different common phrases with a one difference using antonyms and synonyms.
