Speech Background History: Imix is a 5 year 11 month old female who was referred for a speech and language evaluation due to speech delay and bilateral hearing loss. Imix was accompanied to the evaluation by both her parents, and both served as informants. According to mother, Imix was born full-term without any pregnancy or birth complications, and passed her new born hearing screener. Parents stated Imix was developing within normal functioning limits for her age until the age of 2 when she became severely ill during the COVID-19 pandemic. According to mother, after the age of 2 Imix’s speech development decelerated and within 3 months, Imix began speech therapy services with another local agency. Due to lack of progress and further parent concerns, additional testing was requested for Imix. In April 2023, Imix was evaluated by Audiologist Andrea Sweetnam who diagnosed Imix with an unspecified hearing loss. By the end of 2023, Imix was referred to Dr. Jonathan Chiao who then diagnosed Imix with a sensori-neural hearing loss and determined that Imix was a candidate for bilateral cochlear implants. In December 2023, Imix had surgery and her bilateral cochlear hearing aids were activated one month later in January 2024. Parents would like to begin auditory-verbal training as Imix needs to regain her awareness of sounds around her and gain information through listening and imitating speech sounds. English is the dominate language used in home, as well as Signing Exact English. Imix is currently enrolled at the Regional Day School Program for the Deaf where she spends majority of her time in a general education classroom with an interpreter, and 2 hours a day in a classroom for children with hearing loss. Therapist is familiar with Signing Exact English (SEE) and Total Communication sign language. Parents have seen progress since Imix began attending the Regional Day School Program for children with hearing loss, and would like Imix to increase her use of expressive language while given the support of SEE. Imix was given the Comprehension Assessment of Spoken Language-Second Edition (CASL-2) as the family speaks English in the home and Imix listens to videos and family members’ voices in English, and attempts to repeat and/or respond to what they direct towards her.
Speech Medical History: Per parent report, medical history is not remarkable. Per parent report, Imix suffered to a serious illness during the COVID-19 pandemic, which parents believe led to her hearing issues. Imix was implanted for bilateral cochlear hearing aids in 2023 and they were activated January 2024.

Speech Background Summary: Imix is a very shy young girl and entered into the office with no prompting. Imix participated fully in the evaluation with some prompting. Sign language was limited as well as verbal output, but she looked at the pictures and pointed to corresponding pictures. Imix imitated onomatopoeias, she imitated and used sign language she knew to help support what she was attempting to say.

Imix’s standard score for her Expressive Language is 66, at least 3 standard deviations below that of average score and of her same-aged hearing peers. Imix’s raw score on the Sentence Expression sub-test was 1 as she actually only repeated one 3-word utterance. Imix’s standard score on the Sentence Expression sub-test was 54, at least 3 standard deviations below that of average scores and below that of her same-age hearing peers. Imix’s raw score on the Sentence Comprehension sub-test was 5 as she listened and showed understanding by pointing to the matching picture scene. Speech Assessment Summary: Imix is now 6 years 4 months old and is due for a review of language growth and improvement, as well as an update on the use of her auditory acuity for listening and language since beginning speech and language services 6 months ago. Both parents have reported a noticed improvement and progress made since beginning speech therapy, increasing her inclusion in a general education class, and minimizing her time in the Regional Day School Program for children with hearing loss. Imix was given the Comprehensive Assessment of Spoken Language-Second Edition (CASL-2), and it will continue to be used as the tool of assessment to measure growth and progress since these last 6 months of speech therapy. Imix participated fully in the assessment, but needed frequent prompts and/or redirection to attend and/or return to the assessment. Imix utilized signs to support her verbal output, but increased her use of verbal output during this assessment. Imix pointed to accurate pictures and selectively utilized sign to support her verbal output. Imix received the following scores on the CASL-2: Receptive Language sub-test raw score was 35, as she listened and showed understanding by pointing to basic CORE words that labeled common core vocabulary items (i.e., dog, cup, party). Imix’s standard score for Receptive Language was 92 (percentile: 30, age-equivalent: 5;6-5;8). Imix has made steady gains with listening and understanding to what is needed. On the CASL-2: Expressive Language sub-test, Imix received a raw score of 16 as she could tell and give information about the pictures she saw in the assessment. Imix’s Standard Score on Expressive Language is 78 (percentile: 7, age-equivalent: 4;3-4;5). Imix has made steady gains as she has increased her use of verbal output. Imix’s Standard Score on Expressive Language is at least 2 STANDARD DEVIATIONS BELOW that of average score and BELOW that of her same-aged hearing peers. Expressive LanguageIn 6 months, using auditory-verbal strategies and total communication approach, Imix will produce multisyllabic words with all speech sounds given with 30% accuracy by imitating and/or verbalizing two, and 3-syllable words with early developing speech sounds in at least 3 consecutive sessions in order to be understood when conveying important ideas and information and to increase her expressive language skills with family and others in her daily routine.

The holiday season is right around the corner and I wanted to let you know that our offices will be closed Thursday and Friday during the week of Thanksgiving. In December, the offices will close beginning December 24th (Wednesday) and I will return to the office on Monday, Jan. 5th. If your appointment happens to fall during the time of a closure, I will offer you a make-up appointment based on availability. You will receive notice shortly regarding any cancellations during the holiday closing.
