Feeding Background History: Alexandrea is a 1-year-7-month-old female who was referred by her pediatrician for concerns regarding feeding difficulties and weight. Alexandrea’s mother served as the informant for this evaluation. Alexandrea's primary source of nutrition is breastmilk. Mother reported that feeding difficulties began at 6 months old when Alexandrea started purees and soft solids. Per parent report, Alexandrea is not consistent with her eating and will stop consuming food items she was previously eating. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Alexandrea’s feeding/swallowing skills. Language of intervention: English
Feeding Medical History: Per parent report, medical history is not remarkable. Alexandrea was born full-term at 38 weeks via vaginal delivery.

Feeding Diagnosis/Impressions: Alexandrea is a 1-year-7-month-old female who was referred by her pediatrician for concerns regarding feeding difficulties and weight. Alexandrea’s mother served as the informant for this evaluation. Alexandrea's primary source of nutrition is breastmilk. Mother reported that feeding difficulties began at 6 months old when Alexandrea started purees and soft solids. Per parent report, Alexandrea is not consistent with her eating and will stop consuming food items she was previously eating. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Alexandrea’s feeding/swallowing skills. Language of intervention: English. The structure and function of Alexandrea’s oral cavity could not be formally assessed due to Alexandrea’s resistance. Alexandrea is reportedly very protective of her mouth, and for this reason, a formal assessment could not be administered. Alexandrea presents with typical lip, dentition, and tongue structure; however, decreased facial tone was observed. Alexandrea’s assigned treatment therapist should ensure to monitor her oral cavity and perform a formal oral mech exam when she permits. Alexandrea’s mother stated that she does not have a history of tethered oral tissues. A bedside swallow evaluation and skilled meal feeding observation revealed that Alexandrea presents with a CHRONIC PEDIATRIC FEEDING DISORDER that is characterized by impaired oral intake that is not age appropriate and is associated with medical diagnosis (underweight), nutritional status (limited diet), feeding skill (sensory difficulties, decrease oral motor function), and/or psychosocial dysfunction (problematic mealtime behavior, selective/restrictive eating). FOOD REPERTOIRE: Alexandrea’s mother reported that she consumes 2 solids and 2 liquids, including the following: PROTEINS: yogurt, cottage cheese, LIQUIDS: milk, chocolate milk. Alexandrea’s mother would like her to consume more purees and soft table foods. NORM REFERENCED ASSESSMENT Mrs. Gonzalez completed the Pediatric Eating Assessment Tool (PediEAT), which is a questionnaire used to subjectively assess feeding behavior in children ages 6 months to 7 years. Alexandrea received a rating of "high concern" for problematic mealtime behavior and selective/restrictive feeding behaviors, both of which are consistent with her performance in the evaluation. Alexandrea’s mother reported on the assessment that Alexandrea will often become frustrated during mealtimes and watch television as a distraction. MEAL TIME FEEDING OBSERVATION: Alexandrea remained seated in her mother’s lap during the evaluation. Alexandrea was presented with the following food items in the evaluation: vanilla puree, Dole fruit pineapple cup, strawberry meltables, and cheese squares. Alexandrea refused all trials of food. She turned away from the food and verbally refused. With the clinician’s support, Alexandrea tolerated foods directly in front of her. SWALLOW/ORAL MOTOR EVALUATION: Alexandrea’s oral motor skills could not be formally assessed due to her refusing solid trials of foods. Mrs. Gonzalez does not report unexplained fevers or a history of aspiration pneumonia. Alexandrea’s pharyngeal/esophageal swallow function should be monitored by her treating clinician to ensure safety and determine if further testing is warranted. At this age, a child should demonstrate a more developed munching pattern and management of thicker purees and soft solids. RECOMMENDATIONS: Feeding therapy is highly recommended at this time. At this time, feeding therapy at the rate of 1x per week for 30 minutes is recommended to treat Alexandrea’s CHRONIC PEDIATRIC FEEDING DISORDER. Intervention should focus on increasing Alexandrea’s tolerance to solid food items using the Steps to Eating hierarchy and desensitization techniques. Therapy should also support the development of positive mealtime behaviors and routines, including the elimination of television during meals and the incorporation of opportunities for Alexandrea to explore food on her tray and with her hands. Targeted intervention will include facial and oral sensory stimulation and exercises to promote desensitization of the face and oral cavity, increase sensory awareness, and support the development of oral motor skills necessary for safe and effective feeding and swallowing. Therapy will also address selfΒ­feeding skills to increase Levi’s independence and engagement during meals. At her current age, Alexandrea’s primary source of nutrition should come from a variety of ageΒ­appropriate solid foods, including meltables, soft cubes, and soft mechanical mixed textures. She should be able to accept and safely manage foods across various consistencies, flavor profiles, and food groups. Without feeding therapy, Alexandrea may be at risk for compromised nutrition due to the limited and restricted nature of her current diet. 09-17-2025 11:55 AMAndrea Rodriguez StokesGood morning. This is Andrea from New Beginnings. Here is a summary from the evaluation. Alexandrea is diagnosed with a chronic pediatric feeding disorder that is characterized by impaired oral intake that is not age-appropriate. We will be targeting the following goals for the next few months: tolerating food directly in front of her, tolerating food on her fingertips and/or hands, self-feeding with preferred food items, completing facial and/or oral stimulation via z​vibe, and consuming solids of increased texture. Again, we will be working with food based on the steps to eating hierarchy that we discussed during the evaluation. At home, you can start implementing the family meals (I will be sending you a step-by-step on how to do it). We want to respect her boundaries and start modeling to her how we touch food with our fingers. We can always verbally model, too! Using phrases like "This cheese is very soft!", "This apple is wet", etc.

You can begin family meals with the following. Try to have meals at a scheduled time, so that Alexandrea knows what to expect. Children typically learn best with structure.
1. Give a verbal warning prior to mealtimes (5-minute warning)
- This prepares for the sensory input shift, allowing her to take on a new task.
2. Introduce transition activity (washing his hands)
-Input for calming and washing for tactile preparation.
3. Sit at the table
- Start with empty tray/plate to decrease frustration before meals.
- Check Alexandrea's seat so she is positioned 90 degrees at the hips, knees, and ankles. We want to make sure her feet have support. You may need to adjust the footrest or place towels or any item that can help with support.
- It may be useful to make a change to where people sit at the table, to change the visual environment, and avoid negative cues that may have been formed previously.
4. Family-style serving
- Everyone has to take some of each food offered (you can include a "learning plate" placed in front of her tray, where she can place the food items she does not want)
- Make sure there are at least 2 preferred item so she can have something to eat.
5. Everybody eats for the first 10 minutes
- The focus of the meal should be the food
- Goal for parents is to be good role models. have pleasant conversation, and to let her do what she can by herself.
6. Offer a drink (if needed and/or not eating)
- If not eating, you can offer a drink to help Alexandrea engage with food.
7. After 20 minutes, if she is still not eating, you can introduce her preferred item, in her case, TV.
- This will allow you and Alexandrea to be weaned off the TV with time rather than just removing the item at once.
7. After 30 minutes, begin the clean-up routine.
- Have everyone empty their plate into a bowl/trash can
- If she has been eating all along and is done eating sooner, do clean up sooner
- If she really cannot sit any longer, may need to do clean up earlier.

The goal of family meals is to focus on the volume of foods consumed, with exposure to unfamiliar or unaccepted food as a secondary goal. Our goal for these meals will not be to consume non-preferred foods but to start tolerating them at the level of her plate and fingers. Throughout the meal, you can emphasize things like 'I am touching the chicken with my fingers' or 'I do not want to eat the ham, but I can put it on my learning plate'.
