Speech Background History: Alexander is a 2 year 11 month old male who was referred for a speech and language evaluation by his pediatrician after his mother voiced concerns regarding language development. Alexander was accompanied to the evaluation by his mother. Mother reported that Alexander was born at full-term without pregnancy or delivery complications. According to mother, Alexander β€œnames a lot,” but has difficulty expressing himself. Alexander was previously receiving speech therapy services from another local facility, but mother chose to discharge Alexander after 1 month of service due to β€œdisapproving therapy practices.” Based on clinical observations, Alexander’s current communication was characterized as 1-2 word utterances and scripted phrases, to name/label familiar object, using jargon as he played with assessment items, and was observed to pull/grab/reach for objects or lead the clinician’s hand to communicate his wants/needs. Alexander did not utilize appropriate joint attention eye-contact, and/or walked away when he was not interested in an object or task. A typically developing child that is Alexander’s age should have 100-200 words in their expressive vocabulary, and use 2-3 word utterances consistently to label, comment or request. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Alexander’s receptive and expressive language skills. Based on Alexander’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Per parent report, medical history is not remarkable. Parent stated Alexander suffers from alopecia and takes medication to treat the alopecia 5 times out of the week. According to parent, Alexander frequently tugs his ears or plugs his fingers in his ears, and parent is currently seeking a specialist to rule out hearing difficulties.

Speech Assessment Summary: Alexander, a 3-year 2-month-old male, was seen for a speech and language re-evaluation following ongoing parental concerns regarding his communication development. He was accompanied by his mother, who reported that Alexander frequently names/labels objects but has continued difficulty expressing himself and often uses leading behaviors to communicate wants and needs. During the evaluation, Alexander primarily communicated with 1–2 word utterances, scripted phrases, jargon during play, and nonverbal strategies such as pulling, grabbing, or leading the clinician’s hand to desired items. Joint attention and eye contact were observed to increase since last evaluation. Alexander was noted to disengage when disinterested and benefited from redirection. Behavioral observations included intermittent joint attention and visual contact, variable intent to communicate, withdrawn social interaction, high activity level, moderate environmental awareness, and the need for frequent prompts to participate. A full oral mechanism exam could not be completed due to limited cooperation; ongoing monitoring is recommended, with a complete exam performed when possible. The Preschool Language Scales, Fifth Edition (PLS-5) was administered to assess receptive and expressive communication skills. Alexander required frequent redirection from both the clinician and his mother. PLS-5 Results: Auditory Comprehension: Raw Score 21, Standard Score 53, Age Equivalent 1;5 Expressive Communication: Raw Score 30, Standard Score 80, Age Equivalent 2;3 Total Language Score: Raw Score 136, Standard Score 66, Age Equivalent 1;9 These scores indicate significant receptive and expressive language delays, with receptive language more impacted than expressive. During structured tasks, Alexander demonstrated strengths including naming familiar objects, emerging self-directed play skills, and ability to follow commands when paired with gestural cues. Weaknesses included difficulty identifying basic body parts, inconsistent response to familiar directions, limited initiation of social routines, and reliance on max cues to keep engagement. His use of scripted speech at times affected his ability to initiate or maintain meaningful interactions, contributing to difficulties with social communication skills, including reduced reciprocity and difficulty engaging in back-and-forth exchange. Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative, demonstrating appropriate engagement in all therapeutic activities. During the session, expressive language was targeted through structured and play-based tasks. The patient demonstrated progress in imitating functional 1-2 word phrases, including NOUN + VERB and ADJECTIVE + NOUN combinations, achieving success in 5 out of 10 opportunities when provided with a direct clinician model. The use of phrases to facilitate turn-taking was observed in 3 out of 10 opportunities. The independent initiation of social interactions using 2-3 word phrases remains an emerging skill, with data indicating success in 1 out of 10 opportunities during cooperative play. Receptive language skills were also addressed, with the patient showing notable progress in following 1-2 step directions, successfully completing the command in 6 out of 10 opportunities without demonstrating impulsivity. His understanding of functional verbs during pretend play was demonstrated in 3 out of 10 opportunities given fading verbal and gestural cues.

Occupational Background History: Alexander is a 3.1 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 41 weeks gestation. Child did not experience any complications at birth, however deliver via vacuum suction; child does not have any visual or hearing deficits. Child does not have history of aggression and no seasonal allergies.

Subjective: Patient was friendly and cooperative,
Assessment of Goals/Response to Treatment: Alexander attended to and completed a 3Β­ step novel task when following a visual to assemble a fruit stacking tower with MOD VCs, to improve task sequencing and attention during daily routines. He also followed a visual given MAX verbal cues for sequencing to stack pancakes during "Pancake pile-up". He also engaged in a wooden dot art activity using tweezers with MOD tactile A to move colored balls onto the matching space with MOD verbal cues for redirection to increase his FM dexterity, hand strength, and ability to sequence multistep tasks. Alexander increased social reciprocity when engaged in a turn taking activity with only MIN cues to complete turn taking. He also imitated words and followed 2 step verbal directions with only MIN A to demo increased joint attention and social reciprocity skills. Alexander completed pre-Β­writing tasks with imitating of simple shapes (vertical and horizontal lines, circles, cross) with MAX verbal and visual assistance to increase fine motor control needed for writing task. He was able to color within 1 inch of the border with MIN cues to demo increased FM control and VMI skills.

I've just been doing everything for him before he gets the chance.” β€œSo now that I'm letting him do more on his own I think that he should be caught up in a few and he's in this program where a social worker comes over weekly and that's been helping too.” β€œI want him to get evaluated for it in a few months. Is that okay?” OTD replied and explained that we would have to get a new referral if parent would like to get evaluation done.
09-12-2025Annette BarronThe following are the messages mother send prior to the evaluation regarding her complaints with Alexander's former speech agency: - Alexander Chalmers 09-03-2025 09:09 AM Alexander (Prim) Replied: He doesn't say hi or bye and he doesn't express his wants and needs clearly. Like he can talk and identify many things but to mommy or daddy or to say mommy I'm hungry. It's very rare. If something is bothering him and you didn't see what happen he has a hard time explaining it. He can tell me something hurts but not what. -Alexander Chalmers 09-03-2025 09:09 AM Alexander (Prim) Replied: He doesn't do back and forth conversations like the other kids -Alexander Chalmers 09-03-2025 09:14 AM Alexander (Prim) Replied: He was at a previous speech therapy place. They said he needed it 2 times a week for 30 mins. When I got the speech therapist she had someone who's in training or I guess a graduate student helping her. I don't mind once the graduate student is doing her job. But they would make the meeting 25 mins. I mentioned it and they said they needed time to clean before the next appointment. I thought it was unfair to my child but I accepted it. -Alexander Chalmers 09-03-2025 09:16 AM Alexander (Prim) Replied: I would come on time and sometimes they would let him in late and she started to end the meeting after 23 mins. The place had street parking in a strange area and it was in a house that they made into a facility. When the graduate student wasn't there or she changed programs, the other speech therapist who would usually talk to me, continued to talk to me and focus less and less on my child. She would just have him playing and fully facing me. I get that I need to contribute but you can explain things after. -Alexander Chalmers 09-03-2025 09:18 AM Alexander (Prim) Replied: I have to pay a co pay and I'm driving very far. The time kept getting cut down and I saw no real improvement from the therapy itself. I think it should be a bit for repetitive and structured. My son would do better with some structure instead of just having him play with the potato head today and something random the next day. -Alexander Chalmers 09-03-2025 09:20 AM Alexander (Prim) Replied: If you say put on arm or put on hat. I know he would start to repeat it eventually. But jumping around with different things makes it hard to learn. I asked her what's her plan and she said she doesn't really have one she's going with his flow. But he's 2 he's not directing her. The last session I saw him watching the back of her head and then looking at me and went back to playing. She completely turned away from him to talk to me and I wasn't even prolonging the conversation. It's like I'm bringing him there to play for 23 mins and pay them. -Alexander Chalmers 09-03-2025 09:22 AM Alexander (Prim) Replied: I really want what's best for my child and I want the speech therapist to be proactive with doing their job. Give him the full 30 mins if that's what was recommended and focus on him. Have some structure to build on. If there's little repetition i don't know how he would really improve. -Alexander Chalmers 09-03-2025 09:53 AM Alexander (Prim) Replied: There's nothing wrong with being play based but to just talk to me mostly and have him play by himself. That part I didn't understand how that was helping him. I think at least play with him. I would rather most of the dialogue be towards the end and not during the entire session. -Alexander Chalmers 09-03-2025 10:02 AM Alexander (Prim) Replied: Even when it was 23-25 mins long the graduate student would engage with him the entire time. I can see him bonding with her and she didn't let him play by himself or turn her back away. I can see from that consistent engagement throughout the session that he can expand his vocabulary.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

When I got the speech therapist she had someone who's in training or I guess a graduate student helping her. I don't mind once the graduate student is doing her job. But they would make the meeting 25 mins. I mentioned it and they said they needed time to clean before the next appointment. I thought it was unfair to my child but I accepted it.

The place had street parking in a strange area and it was in a house that they made into a facility. When the graduate student wasn't there or she changed programs, the other speech therapist who would usually talk to me, continued to talk to me and focus less and less on my child. She would just have him playing and fully facing me. I get that I need to contribute but you can explain things after.

I think it should be a bit for repetitive and structured. My son would do better with some structure instead of just having him play with the potato head today and something random the next day.

But jumping around with different things makes it hard to learn. I asked her what's her plan and she said she doesn't really have one she's going with his flow. But he's 2 he's not directing her. The last session I saw him watching the back of her head and then looking at me and went back to playing. She completely turned away from him to talk to me and I wasn't even prolonging the conversation. It's like I'm bringing him there to play for 23 mins and pay them.

However, I can tell you that a lot of clinician will use a play-based approach when working with younger patients. We get down to their level, but language is always focused, providing language for all actions and items used. Should your child qualify for services, we start off at 1 time a week and the session is about 45 min. and we use the last few min to clean up and dialog with the parent post session.

4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained, 1st words, combined word?
5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils?
6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history?
7.) Which language(s) is the child exposed to regularly?
8.) Vision or hearing issues?
9.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

That part I didn't understand how that was helping him. I think at least play with him. I would rather most of the dialogue be towards the end and not during the entire session.

Alexander has been diagnosed with a severe to profound mixed receptive-expressive language disorder. Alexander would greatly benefit from speech therapy services that address the following functionally appropriate concepts: following commands with cues, turn-taking, naming objects, using words more often than gestures to communicate, and using words/word combinations for a variety of pragmatic functions. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the assigned treating therapist will reach out to you to discuss beginning treatment. Since we aren't in his normal environment, I make up different activities like drawing different animals and I see he's trying to draw them too. He's using the toilet more. He plays and interacts with me more. He wants me to run with him. He tries to explain what he wants more and repeats after me. I saw a jump in his progress but I think it's because of the one on one attention he's getting more of since we aren't traveling.

I just wanted you to be aware of your options so you can make an informed decision moving forward because during the 6 months of authorization it is expected he will attend sessions each week and if he misses due to vacations or illness we need to conduct make-up appointments. So whatever you decide works best for you guys is what we can do moving forward :)

I want to ensure we work on things that matter to you and Alex; OT is versatile in a way that we don't have to focus so much on the scores, but more so on development itself- Alex demonstrates decreased focus and attention due to decreased executive functioning and processing skills, not because he lacks ability to perform, but more so understanding what is required of him to perform- therefore, I believe he can benefit from OT- but he can also very well learn this from you, school or with developing his language more- just let me know what you decide, so I can complete his report (: also let me know if you have any other questions for me.

Just now I asked him if he wanted more juice and he said more juice. Then I said it's cold outside do you want a sweater and he said cold sweater. Then he wanted to take it off but he was having trouble with the zipper so he said zipper to me and then I zipped it half way and I asked if he wanted me to take it off and he said take it off.
