Assessment of Goals/Response to Treatment: The patient presented to the session alert and oriented, demonstrating engagement and readiness for skilled intervention. Skilled occupational therapy focused on enhancing motor planning, bilateral integration, and functional independence through structured sensorimotor activities. Significant progress has been made toward established goals, with the patient successfully meeting objectives for functional mobility and bilateral coordination. She now consistently transitions between 3-4 tasks with moderate assistance and verbal cues for safety and sequencing in 3 out of 4 trials, a marked improvement from her baseline of maximum assistance. This demonstrates improved body awareness and praxis. Furthermore, she has met her bilateral coordination goal by participating in gross motor activities for a sustained 8-10 minute period, utilizing only moderate compensatory movements to perform symmetrical and asymmetrical tasks, compared to maximal compensations at baseline. During today's session, this was demonstrated as she completed a multi-step obstacle course with only minimal verbal cues and assistance, effectively targeting visual-motor integration, gross motor coordination, and motor planning. Progress was also noted in self-care skills, as she addressed tooth-brushing using a tabletop model with moderate assistance. The patient also successfully completed a sorting and matching task with minimal assistance, indicating gains in sustained attention, sequencing, and in-hand manipulation. She continues to benefit from skilled intervention to generalize these gains and further refine perceptual processing and self-care skills for increased independence.

Each have 26 visit but for speech there is no more visits left and we are unable to request more since it is a hard max. There is still the 26 visits for occupational available to use.

At this time, there are 14 visits remaining, so we will schedule Olivia for occupational therapy sessions every other week under this authorization. Once these visits are used, any additional sessions through the end of December will need to be paid out of pocket. Your visit allowance will reset in January.
