Kaizen’s mother and father expressed a desire for him to eat proteins (meats), pastas, grains, breakfast, and vegetables. NORM REFERENCED ASSESSMENT: Ms. Rosas completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver questionnaire for children ages 6 months to 7 years. Kaizen received an overall rating of β€œhigh concern,” with β€œ concern to high concern” noted in the areas of physiological symptoms, selective/restrictive eating, oral processing, and problematic mealtime behaviors. These caregiver reported concerns aligned with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Kaizen was observed seated in a high chair with full 90 90 90 postural support. It is important to note that this seating arrangement is used consistently at his father’s home, but not at his mother’s home. Kaizen typically only has opportunities with solids two times per day but does not always consume solids during those times. During observation with preferred foods, Kaizen demonstrated overstuffing behaviors, placing entire pieces into his mouth rather than biting off smaller portions. He exhibited difficulty with biting skills, showed only minimal vertical munching patterns, and demonstrated emerging but limited tongue lateralization. He primarily swallowed food pieces whole, which frequently resulted in coughing episodes and watery eyes. When presented with non preferred foods, Kaizen became distressed, crying and refusing to interact with the items. He required the support of an iPhone displaying Ms. Rachel to remain engaged in the meal. Parents reported that these behaviors are consistent with feeding at home, noting that while he may show interest in exploring new foods with his hands and mouth, he often does not know how to consume them and typically limits himself to a single bite. SWALLOW/ORAL MOTOR EVALUATION: Notable findings included drooling and bolus holding, indicating reduced oral motor control. Kaizen demonstrated minimal vertical munching, emerging lateral tongue movements, and difficulty biting solids (meltables and soft mechanicals). During feeding with solids (meltables and soft mechanical), he primarily swallowed pieces whole, resulting in coughing and watery eyes. Overstuffing behaviors were observed. In contrast, during bottle feeding, the patient demonstrated an adequate rate of intake and safe swallowing without loss of fluids or other difficulties. Overall, oral motor function appeared reduced during solid food consumption, impacting his ability to efficiently and safely manage solids. Kaizen’s pharyngeal swallow function is also at risk and a modified barium swallow study may be warranted over the course of care. Kaizen has no history of aspiration pneumonia. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 minutes to address Kaizen’s OROPHARYNGEAL DYSPHAGIA and CHRONIC PFD. Kaizen is at risk for compromised health and development due to a significantly limited food inventory, persistent picky eating behaviors, and decreased oral motor skills for safe chewing/swallowing. At his age, a child is typically expected to accept 8 or more foods across various food groups as well as consume 2 to 3 meals a day. Moreover, a child this age should be able to safely chew and swallow soft table foods and demonstrate emerging rotary chewing and full tongue lateralization. Occupational Background History: Kaizen is a 1.7 year old boy who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Father, was present in order to serve as the primary informant throughout the evaluation. Per father's report child was born at 36 weeks gestation via C-section. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and no seasonal allergies.
Occupational Medical History: Per parent report, medical history is not remarkable

Occupational Behavioral Observations: Attending Skills: Adequate, Response Rate: Mild delays, Level of Activity: Active, Cooperation: With some prompts, Behavior: Non-existent, Awareness of Others: Fair, Reliability of Scores: Good, Awarencess of Environmental Events: Usually, Social Interactions: Friendly, Prognosis for Improved Communicative Functioning: Good,
Occupational Communication Strengths: Cooperative, Grasp, Motivated, UE Strength,
Occupational Communication Weaknesses: Balance, Body Awareness, Endurance, Eye-Hand Coordination, Manual Dexterity, Postural Stability/Trunk Support, Sensory Process/Mod, Upper Limb Coordination, Visual Motor Skills, Visual Perceptual Skills,
Occupational Background Summary: Kaizen demos difficulty with in-hand manipulation skills due to delayed finger dexterity, finger isolation and palm translation skills. Fine motor skills involved the small muscles of the body that enable such functions as writing, grasping small objects, and fastening clothing. Weaknesses in fine motor skills can affect a child's ability to eat, write legibly, use a computer, turn pages in a book, and perform personal care tasks such as dressing and grooming.
Occupational Diagnosis/Impressions: Kaizen was assessed via: PDMS-2 Standardized Assessment, Clinical Observations and Parental Report PDMS-2 - Per standardized assessment scores Kaizen falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores ranging from 12 months to 14 months compared to his chronological age of 19 months, placing him at least (-2) standard deviations behind his peers. Kaizen demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to father, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Father verbalized understanding of treatment plan and agreed to the initiation of skilled therapy.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

Kyro can keep his usual Wednesday appointment, and I can see Kaizen on Thursday, October 16th at 9:30 AM would that work for you? Starting the following week of October 20th, I can permanently schedule the boys back to back on Wednesdays with Kaizen at 1:15 PM and Kyro at 1:45 PM. Would this work? 10-22-2025 03:54 PMJordan OrozcoHi! Kaizen did a great job in therapy today. He stayed in the high chair for about 15 minutes (10 minutes longer than last time), and I used bubbles to support his regulation and prevent crying. He explored puree with his hands and practiced picking up small pieces of his Gerber meltable crackers, which were broken into smaller pieces to help prevent overstuffing. I recommend continuing to practice high chair time at home without TV or iPad distractions, encouraging food exploration, and breaking meltables into small, soft pieces to place on the sides of his mouth to promote chewing. Continue pacing him by offering only a few small pieces at a time. Overall, Kaizen had a positive session, and we’ll keep taking small steps toward improving his comfort with the high chair and feeding therapy.

They sell them on amazon and this can be a really beneficial tool for us to use on Kaizen’s feeding development journey. :)

I recommend to continue to seat him in the high chair for meals, no screen time or distractions, allowing him to play with his food, and offering small soft bites and not larger or hard foods. I told his dad to bring soft cubed consistency for our next visit so we can start working on chewing and swallowing that consistency of food since he is starting to consistently chew and swallow meltables now. :)

He ate several pieces of strawberries and tried to eat a grape but I think it was too soft. I noticed when i cut the strawberries into smaller cubed pieces he was able to chew and swallow them rather than the big pieces that would fall out of his mouth or he would spit out. We only played with corn with out hands and he refused his corn dog and fries. His grandmother and I agree to maybe air fry the corn dog and fries so that they don’t get soft and he is more likely to want to eat them. :)”

11-18-2025 01:22 PMAndrea Rodriguez StokesThe goal of family meals is to focus on the volume of foods consumed, with exposure to unfamiliar or unaccepted food as a secondary goal. Our goal for these meals will not be to consume non-preferred foods but to start tolerating them at the level of the plate and fingers. Throughout the meal, you can emphasize things like 'I am touching the chicken with my fingers' or 'I do not want to eat the ham, but I can put it on my learning plate'. The 'learning plate' should be an empty plate/bowl placed in front of the dinner plate or place mat. The purpose of this plate is that if Kaizen expresses that he does not want a non-preferred item on his plate, you can tell him to put it on his 'learning plate'. By doing this, he will still be interacting with non-preferred items, as he will have them directly in front of him. Our goal is for him to start tolerating food around him without any pressure, eventually leading to him interacting with it at the level of the fingers and oral cavity. This will decrease any frustration during meal times and will prevent him from putting the food in the trash or away. Make sure plates, napkins, cups, and placemats are all plain and white (again, the focus should be the food). These family meals should be performed once per day. Pick a time that works best for you where you can all model behaviors.
11-18-2025 01:22 PMAndrea Rodriguez StokesYou can begin family meals with the following. Try to have meals at a scheduled time, so that he knows what to expect. Children typically learn best with structure.


1. Give a verbal warning prior to mealtimes (5-minute warning)
- This prepares for the sensory input shift, allowing him to take on a new task.


2. Introduce transition activity (washing his hands)
-Input for calming and washing for tactile preparation.


3. Sit at the table
- Start with an empty tray/plate to decrease frustration before meals.
- Check his seat so he is positioned 90 degrees at the hips, knees, and ankles. We want to make sure his feet have support. You may need to adjust the footrest or place towels or any item that can help with support.
- It may be useful to make a change to where people sit at the table, to change the visual environment, and avoid negative cues that may have been formed previously.

- Place all food items in the center of the table.
- Everyone has to take some of each food offered (you can include a "learning plate" placed in front of his tray, where he can place the food items he does not want)
- Make sure there are at least 2 preferred items so he can have something to eat.


5. Everybody eats for the first 10 minutes
- The focus of the meal should be the food
- Goal for parents is to be good role models. have pleasant conversation, and to let him do what he can by himself.
- We want to avoid giving him negative feedback during mealtimes. Praise if any good attempts are made.

6. Offer a drink (if needed and/or not eating)
- If not eating, you can offer a drink to help him engage with food.

- This will allow you and Kazien to be weaned off the TV with time rather than just removing the item at once.

- Have everyone empty their plate into a bowl/trash can
- If he has been eating all along and is done eating sooner, do clean up sooner
- If he really cannot sit any longer, you may need to do clean up earlier.
