According to his mother, Ms Jefferson, Kingston began exhibiting picky eating behaviors around 6 months of age where he was unable to consume solids. Kingston has no prior history of feeding therapy. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills.
Feeding Medical History: Kingston was born at 39 weeks via vaginal delivery. Ms Jefferson reported no complications during the pregnancy but at birth Kingston required a 1 week stay in the NICU due to being born with a fever and requiring antibiotics.

Feeding Diagnosis/Impressions: PATIENT HX: Kingston is a 3 year 6 month old male referred by his pediatrician for a feeding evaluation due to concerns related to picky eating and limited food inventory. Kingston potentially has a growth hormone deficiency which he is still being tested for. Due to this, Kingston is prescribed 2 Kate farms milks per day. According to his mother, Ms Jefferson, Kingston began exhibiting picky eating behaviors around 6 months of age where he was unable to consume solids. Kingston has no prior history of feeding therapy. A bedside feeding evaluation and skilled meal observation were completed to further assess his feeding and swallowing skills. ORAL MECH EXAM: A formal oral peripheral examination could not be completed due to Kingston’s shy behavior and difficulty following specific instructions. However, based on observable findings, Kingston appears to possess the oral structures necessary for safe feeding and swallowing. Oral mechanism function was noted to be decreased, as evidenced by a reduced chewing pattern, reduced tongue lateralization, and open-mouth chewing observed during mealtime. Additionally, it was reported that Kingston occasionally spits out food when he is unable to chew it thoroughly. Kingston has a history of lingual tethered oral tissues, which were diagnosed in infancy; however, a frenectomy was not recommended at that time. He does not demonstrate mouth breathing while awake or asleep. It is recommended that Kingston’s oral cavity and oral motor function be monitored throughout the course of treatment. FEEDING/SWALLOWING: A bedside swallow evaluation and skilled meal feeding observation revealed that Kingston presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. Kingston’s PFD is characterized by impaired oral intake that is not age appropriate and appears to be influenced by several factors, including medical history (possible hormone growth deficiency), nutritional deficits (limited food inventory, prescribed Kate farms), feeding skill impairments (sensory sensitivities to foods, oral dysphagia), and psychosocial dysfunction (problematic mealtime behaviors). Kingston’s oral dysphagia is characterized by reduced chewing pattern, reduced tongue lateralization, open mouth chewing, and reports of spitting out food when he is unable to chew thoroughly. FOOD REPERTOIRE: Kingston is reported to prefer foods with crunchy, crisp, and hard textures. He shows a strong taste preference for sweet and salty flavors and favors warm foods. His appetite is reported to be fair. Currently, Kingston’s food inventory includes 24 foods and 4 liquids. However, many of his foods are starches. Kingston has reportedly dropped many foods from his inventory. For proteins, he accepts chicken nuggets, fish sticks, and yogurt. His fruit intake includes apple and watermelon. Kingston only eats 1 vegetable which is carrots. For starches, Kingston eats french toast/waffles/pancakes, dry cereal, pizza, Mac n cheese, rigatoni, pizza, crackers, chips, bread rolls, cake/cookies/donuts, and popcorn. In terms of liquids, he drinks apple juice, milk, caloric supplement, and water. Kingston’s mother expressed a desire for him to eat chicken/rice, vegetables, carne asada, eggs, and spaghetti. NORM REFERENCED ASSESSMENT: Ms. Jefferson completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver questionnaire for children ages 6 months to 7 years. Kingston received an overall rating of β€œhigh concern,” with β€œhigh concern” noted in the area of problematic mealtime behaviors. These caregiver reported concerns aligned with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Kingston was seated on the floor next to his mother and observed to self feed his preferred food items using his hands. Kingston reportedly does not use utensils often. During the feeding session, Kingston moved around the room while eating. Kingston can take more than 30 to 60 minutes to finish a meal and hardly ever finishes a meal. Kingston typically has an iPad or tv on in front of him during meals. Kingston also reportedly spits out food sometimes when he is unable to chew it. Kingston’s mother was educated on techniques to decrease distractors during mealtime and encourage small bites of foods for successful chew/swallow. When presented with non preferred foods, Kingston showed immediate rejection of the foods by turning away from the foods items and saying β€œno”. With clinician support, he tolerated some brief contact with the foods on his finger tips and hands. Kingston refused to interact with the foods past his finger tips and hands. Kingston reportedly does not interact with non preferred foods at all at home. SWALLOW/ORAL MOTOR EVALUATION: Kingston demonstrated a vertical munch chewing pattern when consuming his preferred food items. Kingston demonstrated open mouth chew, decreased tongue lateralization and audible smacking. No clinical signs or symptoms of aspiration or penetration were observed during the session. According to Ms. Jefferson, there have been no concerns related to Kingston’s swallowing at home, including no episodes of coughing, choking, or signs of distress during meals. Additionally, Kingston does not have a history of aspiration pneumonia. However, it was reported that Kingston occasionally spits out food when he cannot chew it thoroughly. Based on the foods trialed and the behaviors observed during the evaluation, Kingston’s oral motor skills appear reduced at this time and warrant therapeutic intervention. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 minutes to address Kingston’s CHRONIC PFD and ORAL DYSPHAGIA. Kingston is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. In addition, food chaining strategies should be implemented to expand Kingston’s food repertoire by building on existing preferred foods. Kingston’s therapy will also focus on oral motor exercises to promote adequate mastication for safe and effective feeding/swallowing. Without intervention, Kingston may be at increased risk for nutritional deficiencies and inadequate growth due to his restricted dietary variety. Ms. Jefferson will be responsible for implementing the recommended Home Management Program to reinforce therapeutic strategies and support progress between sessions.

Kingston is currently undergoing medical evaluation for a possible growth hormone deficiency and is prescribed two Kate Farms nutritional supplements daily to support caloric intake. Per caregiver report, Kingston began exhibiting feeding difficulties around 6 months of age, characterized by difficulty consuming solid foods. Kingston has no prior history of feeding therapy. A bedside feeding evaluation and skilled mealtime observation were completed to assess current feeding and swallowing function. ORAL MECHANISM EXAM A formal oral peripheral examination could not be completed due to limited cooperation; however, observable findings indicate that Kingston possesses oral structures adequate to support feeding and swallowing. Oral motor function appears reduced, as evidenced by immature chewing patterns, decreased tongue lateralization, and open-mouth chewing observed during mealtime. Caregiver report indicates that Kingston intermittently spits out food when unable to chew it effectively. Kingston has a history of lingual tethered oral tissues diagnosed in infancy, with no surgical intervention recommended. He does not demonstrate mouth breathing during wake or sleep. Ongoing monitoring of oral motor function is recommended. Potatoes have been identified as an upcoming target food. STANDARDIZED ASSESSMENT The Pediatric Eating Assessment Tool (PediEAT) was completed by Kingston’s caregiver. Results indicated an overall rating of β€œhigh concern,” with elevated concern for problematic mealtime behaviors. These findings are consistent with clinical observations. MEALTIME OBSERVATION Kingston was observed self-feeding preferred foods using his hands while seated on the floor. He demonstrates limited utensil use, frequent movement during meals, and prolonged mealtimes exceeding 30–60 minutes, often without meal completion. Meals frequently occur with electronic distractions present. When presented with non-preferred foods, Kingston demonstrated immediate refusal behaviors. With clinician support, he tolerated brief tactile contact with non-preferred foods at the fingertips but did not progress beyond this level. Caregiver reports minimal interaction with non-preferred foods at home. SWALLOW / ORAL MOTOR FUNCTION Kingston demonstrated a vertical munch chewing pattern with preferred foods, accompanied by open-mouth chewing, decreased tongue lateralization, and audible smacking. No clinical signs or symptoms of aspiration or penetration were observed during the evaluation. Caregiver reports no history of coughing, choking, respiratory distress, or aspiration pneumonia. However, spitting out food when unable to chew adequately continues to be reported. Findings indicate reduced oral motor skills requiring skilled intervention. RECOMMENDATIONS Skilled feeding therapy is medically necessary and recommended once per week for 30-minute sessions to address Kingston’s chronic pediatric feeding disorder and oral dysphagia. Therapy will focus on: Increasing food acceptance using systematic desensitization and the Steps to Eating hierarchy Expanding dietary variety through food chaining strategies Improving oral motor skills to support efficient mastication and safe swallowing Reducing maladaptive mealtime behaviors and increasing caregiver implementation of strategies At Kingston’s age, children are typically expected to accept 30 or more foods across food groups. The patient successfully touched non-preferred food items to his upper extremities (fingers, hands, or arms) in 7 out of 10 opportunities across three consecutive sessions and demonstrated increased tolerance for proximal interaction by touching non-preferred foods to his head in 6 out of 10 opportunities. Olfactory exploration was also targeted, with the patient smelling non-preferred foods in 6 out of 10 given opportunities. Furthermore, he demonstrated substantial gains in oral tolerance by placing a non-preferred food inside his mouth for at least 3–5 seconds before spitting out as needed in 3 out of 10 opportunities. Building upon these sensory gains, the patient successfully added one to two food-chained items to his repertoire, expanding the quantity and variety of his accepted food inventory to better support nutritional needs. Concurrently, oral-motor skills for mastication showed marked improvement. The patient demonstrated a closed-mouth, rotary chewing pattern during feeding tasks in 6 out of 10 opportunities, reflecting a significant reduction in previous patterns of vertical munching and associated compensatory behaviors such as smacking and open-mouth chewing.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

08-21-2025 12:29 PMKingston JeffersonKingston (Prim) Replied: He has always been a very picky eater to the point where he only eats 3 to 4 food items with me at home and won't even look interested or trying food. I've noticed that certain textures or if it looks a certain way, he won't try it and in the past when he was brave enough to try something new he would spit it out and start gagging or throwing up and this was since he was able to eat solid foods. He does go to daycare five days a week where they do provide two meals for him and I've been told that he eats his food, but I've never seen it with my own eyes so I'm not sure but I know with me he just doesn't wanna try anything else.

Be sure to pack a total of 5 foods. 1 food should be a food chained item and the other 4 can be non preferred foods that you would like to begin targeting. I recommend picking 1 protein, 1 starch, 1 fruit, 1 vegetable. We will target these same foods for at least 1 month. I recommend selecting foods in their most basic form - for example if you want to work on carrots. I would recommend raw fresh carrots that are not cooked or seasoned. Let me know if you have any questions or need ideas. :)

For fruits (Apple or Oranges ) and veggies ( carrots but boiled or steamed that way theyre softer)!
