Occupational Background History: Charlotte is a 5.6 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother report child was born at 40 weeks gestation. Child did not experience any complications at birth, does have visual deficits (glasses) and no hearing deficits. Child does have history of aggression with functional activity demands and no seasonal allergies.

Per parent report child has difficulties with emotional regulation, body and safety awareness; child constantly looks for proprioceptive input and oral seeking; child is a picky eater and demos difficulty with completion of multi-step tasks and tolerance to functional activity demands; per clinical observation child has retained primitive reflexes, rooting, ATNR and STNR, decreasing sensory processing. Per parent report child needs increased reminders to stay on task and complete tasks in allotted time due to decreased focus and attention to multi-step ADL tasks; decreased body and safety awareness decreased child’s ability to transition between tasks. Charlotte demonstrates deficits in emotional regulation, reflex integration, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Subjective: Patient was alert and cooperative,

Assessment of Goals/Response to Treatment: Patient presented as alert and cooperative, demonstrating active engagement in the session. Skilled intervention addressed progress toward self-care goals, with patient now requiring moderate assistance to complete the steps of tying shoes using the bunny ears method, showing significant improvement from a baseline of maximum assistance. Continued practice is required to achieve the goal of 3/4 successful trials. To address goals for fine motor and visual-motor integration, patient participated in a cutting activity with scissors, requiring minimal verbal cues to maintain accuracy within 1/4" of the line, demonstrating progress in lead/assist hand use and graded control. Further development of underlying components was targeted through chalkboard writing activities, which required minimal verbal and physical assistance to promote a functional grasp, enhance fine motor precision, and improve visual-motor integration skills. Patient made excellent progress toward social reciprocity goals, demonstrating 100% achievement of the objective. During a therapeutic board game with a sibling, patient successfully engaged in multiple back-and-forth verbal exchanges with only minimal cueing, exhibiting improved turn-taking and social participation without the meltdowns noted at baseline. While social reciprocity was improved, the patient demonstrated a fair understanding of the coping skills aspect of the game and required minimal to moderate verbal cues to participate appropriately, indicating an ongoing need for development in this area.
