Assessment of Goals/Response to Treatment: The patient presented as alert, oriented, and cooperative for the duration of the session. Therapeutic activities targeted the imitation of consonant-vowel (CV) combinations as a means of improving functional verbal expression. The patient demonstrated progress, successfully producing CV word approximations in 6 out of 10 opportunities (60%) given clinician modeling and tactile cues. Intervention also focused on the use of a total communication approach for expressing wants and needs; the patient independently utilized a combination of gestures, signs, and/or vocalizations to make requests in 5 out of 10 opportunities (50%). Receptive language skills were addressed through structured play tasks targeting pronoun comprehension. The patient demonstrated understanding of pronouns such as 'mine' and 'yours' by correctly identifying the associated object in 3 out of 10 discrete trials (30%). Further receptive language tasks addressed the comprehension of action words, with the patient accurately performing the requested verb (e.g., 'eat,' 'sleep') in 1 out of 10 opportunities (10%). Occupational Diagnosis/Impressions: Genesis was assessed via: PDMS-2 Standardized Assessment, Clinical Observations and Parental Report PDMS-2 - Per standardized assessment scores Genesis falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores ranging from 14 months to 18 months compared to her chronological age of 32 months, placing her at least (-2 to -1) standard deviations behind her peers. Genesis demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 60 seconds with decreased completion of task. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child demos retention of primitive reflexes: moro reflex, babinski reflex, SGR, palmar and potential ATNR and STNR reflexes hindering her ability to execute body movements and decreasing her sensory processing and emotional regulation skills; this creates concern for safety and body awareness. Occupational Assessment Summary: Genesis was assessed via: PDMS-2 Standardized Assessment, Clinical Observations and Parental Report PDMS-2 - Per standardized assessment scores Genesis falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores ranging from 14 months to 18 months compared to her chronological age of 32 months, placing her at least (-2 to -1) standard deviations behind her peers. Genesis demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 60 seconds with decreased completion of task. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child demos retention of primitive reflexes: moro reflex, babinski reflex, SGR, palmar and potential ATNR and STNR reflexes hindering her ability to execute body movements and decreasing her sensory processing and emotional regulation skills; this creates concern for safety and body awareness. Assessment of Goals/Response to Treatment: Patient presented as friendly and cooperative, demonstrating excellent engagement in all therapeutic activities. Significant progress was demonstrated toward the goal of increasing body awareness and safety during functional mobility. The patient successfully transitioned between therapeutic tasks with only minimal verbal assistance for sequencing and safety, a notable improvement from the maximum assistance required at baseline. Further progress was observed in bilateral coordination; during a structured activity, the patient sustained participation and utilized both hands simultaneously with only minimal compensatory movements, compared to the maximum compensations previously noted. This indicates improved integration of both sides of the body, which is crucial for functional tasks. To address self-care goals related to sensory tolerance, the patient engaged with various textures including marshmallows, icing, and slime. The patient tolerated tactile input to the hands with minimal aversive behaviors, a marked improvement from the maximum aversion observed at baseline. She also explored marshmallows and icing orally in which she licked, chewed, and swallowed after moderate assistance via modeling and verbal encouragement to increase participation in mealtimes. Throughout these interventions, the patient practiced sustained attention, sequencing, and visual-motor integration. Overall, the patient made measurable gains in all goal areas. Continued skilled occupational therapy is medically necessary to build upon this progress to further enhance safety, motor planning, and sensory participation for increased independence in daily routines. Parent was provided with visual aid of steps to feeding to increase carry over of skills at home.

Since you are going to use insurance, then yes we will need a referral from your child's pediatrician before we could schedule an evaluation. Then next steps would be as follows: 1.) Need the referral first 2.) I will send you a link to fill out all our forms electronically. There you would also upload the referral. 3.) After that, one of our therapists or front office will reach out for the initial evaluation appointment. Rio Grande Therapy is also a reputable company. I believe we are larger and we may offer slightly different services. I do not know how they operate but I can say that I have also not heard anything bad about them as I have with some of the other clinics. Another thing to consider is wait time. After school slots are very limited for us however we are onboarding other therapists very soon. Rio Grande may or may not be able to get you started sooner...that I am not sure. I hope that helps!

In a few words write your parental concerns with patient's speech and/or language, and how you would like to improve within a year?

She crawled about 6-7 months, sit in 6 months, stand in 1 year, walk in 1 year and 3 months, fed self at 2 years, she cant really dress self, and still potty training. 5.) No sleeping or breathing issues nor waking up several times at night, and most of the time she sleeps with her mouth open. 6.) None 7.) No 8.) Hopefully she can talk complete sentences and ask for stuff that she wants or doesn't. She has that Bi-labial where she separates word like "open". She makes noises to communicate and a little bit of sign language like "open, more, I Want, please" and whenever we try to teach her to pronounce things she always say "da-tum". But all in all, we would like for her to improve her verbal communication
