Overt clinical signs of pharyngeal compromise (i.e., coughing, increased respiratory effort) were observed during the evaluation, most likely related to overstuffing behaviors rather than a primary medical condition. Lucah’s exact daily caloric intake is unknown; however, based on caregiver report, his limited food acceptance, restricted variety, and frequent refusal of non-preferred foods suggest his intake may not be meeting age-appropriate dietary expectations. FEEDING OBSERVATIONS: During the feeding observation, Lucah was seated in a stable 90-90-90 position (hips, knees, ankles) at the table for the duration of the assessment. A variety of foods were presented, including non-preferred items (cheddar cheese) and preferred items (mac and cheese, Nutri-Grain bar). With moderate verbal prompting, Lucah accepted bites of his preferred foods and consumed approximately 90% of the items offered. When presented with non-preferred foods, his responses were variable. He initially stood up from the table and moved closer to his mother, actively avoiding the foods, indicating clear avoidance behavior. With verbal and visual models, he eventually touched the non-preferred foods with his finger pads and palm. Based on his responses, Lucah’s interaction with non-preferred foods fell within early sensory exploration levels of the Steps to Eating hierarchy. Per caregiver report, Lucah demonstrates heightened sensory aversions to non-preferred foods during mealtimes at home, and his behaviors during the session were consistent with these reported patterns. These difficulties appear to be influenced by sensory dysregulation and underlying sensory processing differences that impact his ability to tolerate a wider range of textures and expand his diet. An occupational therapy referral is recommended to further evaluate and address sensory integration needs in collaboration with ongoing feeding therapy. Overall, these observations suggest a persistent sensory-based feeding difficulty that significantly impacts Lucah’s willingness to explore and expand his food repertoire. ORAL MOTOR SKILLS: During the observation of oral intake, Lucah demonstrated an emerging diagonal chew pattern with active lip movement and anterior protrusion/retraction of the tongue, suggesting limited dissociation of oral structures. He presented with several oral motor deficits, including reduced tongue lateralization, limited rotary movement, shortened mastication time, and frequent overstuffing behaviors. Poor bolus formation was observed, and he did not consistently pull or bite from larger pieces of food, further contributing to inefficient mastication. Overt signs of pharyngeal dysphagia—such as increased respiratory effort and coughing—were observed during consumption of mac and cheese and a Nutri-Grain bar, which may be attributed to poor coordination of oral and swallow function. Lucah also verbalized that “hard foods” tend to “hurt” when swallowing, which may further impact his food choices. By approximately 4 years of age, children are typically expected to demonstrate emerging rotary chewing skills, improved tongue lateralization, and adequate bolus formation across a variety of textures. Ongoing monitoring is recommended as Lucah is introduced to increasingly complex textures during intervention. Overall, he presents with decreased oropharyngeal motor coordination and efficiency, which may be contributing to concerns regarding the safety and effectiveness of his oral intake. RECOMMENDATIONS: Feeding therapy is strongly recommended to address Lucah’s current feeding challenges. His existing feeding deficits, including oral motor inefficiencies, overstuffing behaviors, sensory-based aversions, and limited food repertoire, place him at risk for nutritional compromise if not addressed. Intervention should focus on increasing tolerance for non-preferred foods using a structured approach that incorporates the Steps to Eating Hierarchy, desensitization techniques, sensory-based feeding activities, and food chaining strategies. Therapy will also target improved oral motor coordination (chewing, bolus formation, tongue lateralization) and pacing to promote safe and efficient intake. Parent education and implementation of structured mealtime strategies will be integral to reduce stress, increase consistency, and support Lucah’s feeding progress across environments. A Home Management Intervention Program (HMIP) will be provided to reinforce therapy strategies at home and promote generalization into daily routines. Given Lucah’s noted sensory sensitivities and regulation challenges, an occupational therapy referral is warranted to address underlying sensory processing needs in collaboration with feeding therapy. Without appropriate intervention, Lucah remains at risk for inadequate nutrition secondary to food selectivity and food jags. Based on caregiver preference and language exposure, therapy sessions will be conducted in Spanish as appropriate.
Feeding Progress to Date: In 12 months, Lucah will improve his feeding and swallowing skills to support safe, efficient, and ageappropriate nutrition, as demonstrated by achieving 50% accuracy across 5 consecutive sessions. In 6 months, Lucah will consume solids and liquids using pacing strategies (e.g., smaller bites/sips, pausing between swallows) in 3 out of 10 opportunities across 3 consecutive sessions without overt signs of aspiration (e.g., coughing, increased respiratory effort), in order to support safe and efficient intake. CONTINUE GOAL In 6 months, Lucah will demonstrate an emerging rotary chewing pattern with ageappropriate textured solids in 3 out of 10 opportunities across 3 consecutive sessions, in order to promote safe and efficient bolus breakdown. CONTINUE GOAL In 6 months, Lucah will demonstrate tongue lateralization with preferred solids in 3 out of 5 opportunities across 3 consecutive sessions, in order to improve bolus manipulation and support safe swallowing. CONTINUE GOAL When counseled by the clinician, parents will incorporate the home management intervention program as a daily and weekly activity to transfer skills learned in therapy into the home environment. We work with a lot of picky eaters and yes, your child should eat quite a lot more than 4 items by 4yrs old. In order to get started, you would need to get a referral from your child's pediatrician. The evaluation typically lasts about an hour. I recommend arriving 15 minutes early to allow time for paperwork. Alternatively, I can email you the documents in advance so you can complete them ahead of time and bring them with you on the day of the evaluation.

For this upcoming week, I currently have an opening on Thursday at 2:00 PM. Please let me know if this time works for you. If not, I’ll be happy to offer additional availability for the following week.

11. Lucah has been a very picky eater since he was born. He eats 4 different normal meals, and he refuses to try anything else. Every night he ends up crying/screaming and complaining about what is in front of him. If he does try something, he spits it out. He is a big boy, he does eat a lot of cereal bars, chocolate milk and yogurt. I think that is what keeps him going. I am concerned that at 4 years and a half, he still has so many complications with food.

I completely understand your concerns, especially since picky eating has been present since he was a baby and is now impacting mealtimes. Based on what you’ve described, it sounds like this is more than typical “picky eating” and may be related to sensory preferences, oral motor patterns, or feeding anxiety.

During the evaluation, I’ll be looking at how Lucah responds to different foods, textures, and feeding routines so we can get a better picture of what’s making eating so challenging for him. Estos alimentos serán los mismos durante un período de un mes. Para ayudar al sistema sensorial de Lucah, seguiremos un esquema de color (cualquier alimento que sea claro, como blanco o amarillo). Aquí tienes algunas ideas:

Proteínas: yogur, jamón, nuggets de pollo, jamón de pavo, palitos de queso, queso, huevo.

Almidones: galletas de arroz, tortas de arroz, tiras de vegetales, pasta, fideos, pan, panecillo inglés.
