Speech Background History: Adalyn is a 2 year 2 month old female who was referred for a speech and language evaluation by her pediatrician for concerns regarding speech delay. Adalyn was accompanied by both her mother and father, as well as her younger brother. Mother reported that Adalyn was born at 36 weeks due to minimal fetal movement, and upon delivery it was discovered that Adalyn was suffocating as the umbilical cord was wrapped around her neck. However, no extended hospital stay was necessary post birth. According to mother, Adalyn utilizes “baba” and “mama” infrequently to obtain the attention of a communicating partner, but does not consistently engage with direct eye-contact. Both parents claim Adalyn will try to “escape” when in an unfamiliar room, but does not do so at home due to the child safety locks. There is a familiar history of speech delay as Adalyn’s older four year old brother has autism and required speech therapy as well. Based on clinical observations, Adalyn’s current communication was characterized as no functional words, and was heard to use “baba” once as she searched for a toy that had fallen. Adalyn was observed to pull/grab/reach for objects she desired to communicate wants/needs, and attempted multiple time to leave the room when clinician spoke to Adalyn. A typically developing child that is Adalyn’s age should have 25 to 100 words in their expressive vocabulary and use 1 word utterances consistently with 2 words emerging. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Adalyn’s receptive and expressive language skills. Speech Medical History: Parents claim Adalyn has suffered 3-4 ear infections since the family moved to El Paso with in the las year. Parents stated the current pediatrician was not concerned with possible hearing difficulties due to the high number of infections because the family caught the infections early-on.

Intervention focused on enhancing receptive and expressive language skills to promote safety and functional communication. For receptive language, the patient was targeted for responding to her name and successfully did so by stopping an activity or turning toward the speaker with 50% accuracy when provided with multisensory cues. She demonstrated stronger performance in responding to the inhibitory commands "no" and "wait," achieving 60% accuracy in stopping her actions, indicating progress in understanding safety-related directives. Work also continued on following one-step commands paired with gestural cues, which the patient performed with 40% accuracy during play-based activities, suggesting this skill is still emerging. Please complete when you have a moment. Thank you. Speech History Questionnaire for Adalyn: 1.) Name and DOB 2.) Any complications with pregnancy or birth? 3.) Any major illnesses (i.e., ear infections) or accidents since last check up? Any medical diagnoses (i.e., Autism, ADHD, Down syndrome, etc.) or allergies (i.e., medications, foods, seasonal, etc.)? 4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained, 1st words, combined word? 5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils? 6.) Education history (i.e., attends daycare, preschool, etc.) and past therapy history? 7.) Which language(s) is the child exposed to regularly? 8.) Vision or hearing issues? 9.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

Sat up around and stood up 6- 7m, 7-8m feeding herself with hands , walked at 11 1/2m. She's not potty trained . Baba and mama is all she really says. No combined words. 5. No sleeping or breathing issues. She doesn't sleep 8-10hours. Wakes up probably 1-2times at night but goes back to sleep. 6. No education history yet. 7. Mostly English sometimes Spanish. 8. No to both . 9. I would like for her to just talk and be able to say what she needs/wants or how she feels . Just fully communicate and understand.

Adalyn presented with a profound mixed receptive-expressive language disorder. Adalyn would greatly benefit from speech therapy services that address the following: responding to her name, responding to "no" and "wait," following one step commands, relational play, imitating environmental/non¬speech sounds, and imitating one to two syllable words. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the assigned treating therapist will reach out to you to discuss beginning treatment.
