A bedside swallow evaluation and skilled meal feeding observation revealed that Isaac presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. Isaac’s PFD is characterized by impaired oral intake that is not age-appropriate and appears to be influenced by several factors, including medical history (e.g., structural differences, prior frenectomy), nutritional deficits (limited food inventory), feeding skill impairments (decreased oral motor coordination), and psychosocial dysfunction (problematic mealtime behaviors). Isaac’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. FOOD REPERTOIRE: Isaac’s mother also completed a food inventory checklist in which she reported he currently accepts 25­-30 solids and 3 liquids. The following are Isaac’s accepted food items: PROTEINS­ scrambled eggs, ham, pork, hot dog, lunch meat STARCHES­ crackers, chips, flour tortillas, bread, cookies, popcorn, french fries. FRUITS­: apple, banana, grapes. VEGETABLES broccoli. LIQUIDS­: Milk, chocolate milk, and water. It is important to note that the majority of Isaac’s accepted solids are starches and are limited in nutritional variety. Isaac’s mother expressed a desire for him to eat meat, vegetables, and more fruits. NORM­REFERENCED ASSESSMENT: Isaac's mother completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver questionnaire for children aged 6 months to 7 years. Isaac received an overall rating of “concern,” particularly in the domains of selective/restrictive eating. These caregiver­reported concerns were consistent with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Isaac was seated at the feeding table and observed to self­-feed his preferred food items (grapes and sausage biscuits). During the feeding session, Isaac often required redirection to remain engaged with food items. When presented with non­ preferred foods (fig bar and cheese crackers), Isaac showed initial hesitancy but was able to interact with them at the level of his oral cavity. With clinician support, he tolerated a few bites. Mother stated this behavior does not occur at home. Isaac is often offered a "prize" to eat what is on his plate. He also demonstrated difficulty describing the sensory characteristics of the foods, which may indicate limited sensory awareness or underdeveloped food­related language. SWALLOW/ORAL MOTOR EVALUATION: Isaac demonstrated decreased oral motor skills with the consistencies offered during the evaluation. He exhibited decreased tongue lateralization during chewing, leading to food residue in his oral cavity. No clinical signs or symptoms of aspiration or penetration were observed during the session. Mrs. Carter has not observed any concerns related to Isaac’s swallowing at home, including no episodes of coughing, choking, or signs of distress during meals. Isaac also does not have a history of aspiration pneumonia. Based on the foods trialed and the behaviors observed during the evaluation, Isaac’s oral motor skills appear to be reduced and are likely impacting both his feeding and swallowing function. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 to 45 minutes to address Isaac's CHRONIC PFD and ORAL DYSPHAGIA. Isaac is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

No complications during pregnancy. He was born with an irregular heart beat but it's resolved now.

He had Strep throat in June, just seasonal allergies, no food or medicine allergies, We suspect Autism but has not been officially diagnosed. Pediatrician suspects a touch of ADHD but said it should resolve on its own.

Sitting up about 7 months, Crawling age 9 months, standing up 10 months, walking 12 months, feeding self 18 months, dressing self age 3 years, potty trained 3 years, 1st word age 7 months (Momma), word combinations 2-3 years.

No known sleeping or breathing issues.

Starting kindergarten on 08/11/25, no preschool or daycare before.

Only speaks English.

No known hearing or vision issues.

He's always had trouble with pronunciation, but a few months after we moved here from Georgia he started stuttering. Now, it's hard to understand him at all sometimes. We've tried working with him on our own, but we need professional intervention because we aren't seeing any improvement with what we're doing.

He started his picky eating around 2 years old. We moved to a new house around that time and had some pretty big changes so maybe that's a part of it. Currently, I put much less on his plate at dinner so it's not as traumatic to get him to eat. He can have something afterwards if he wants anything else, or he's rewarded with a treat. It doesn't always work though. He's going to kindergarten on Monday 08/11 and I don't want this to be an issue if he has to have lunch at school.

Isaac presents with a profound articulation/phonological disorder. Therapy will target the following phonemes: /s/, /r/, /l/, and /th/. In addition, Isacc presents a profound fluency disorder. Treatment will focus on increasing his awareness of “bumpy” versus “smooth” speech, coordinating exhalation with phonation, understanding terminology used in fluency therapy, and using easy, relaxed speech at the word level.

Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. Pizza, egg sandwiches (mayo, fried egg, and sourdough bread), ham and cheese sandwiches, cheese puffs, only one brand of chocolate cereal (Three Wishes), Ritz crackers only, bananas, green grapes, sometimes he'll eat an apple, only one brand of yogurt (Keto vanilla), chick fil a nuggets only, hamburgers from restaurants but not at home, power crunch bars, cliff kid cookies and cream bars, frozen croissant breakfast sandwich from Walmart (egg, cheese, sausage on a croissant) Tennessee Pride sausage biscuits, home made scrambled eggs with no cheese, and I think that's it. If I forgot anything I'll send it later.

As the form says, change one slight thing at a time. You can start by introducing him to the same items but different brands. Try a different cheese that is similar in color in his sandwich.
