Feeding Background History: Sophia is an 18 year 10 month old female referred for a feeding evaluation by her pediatrician due to long standing feeding difficulties. According to her mother, who served as the primary informant, Sophia has been a very picky eater since the age of four, to the extent that the family avoids traveling due to the limitations of her eating habits. She tends to overstuff her mouth, eats rapidly, and often needs verbal prompts to slow down; on some occasions, she eats so quickly that she vomits. Her eating schedule is irregular, typically consisting of breakfast, dinner, and a late night snack. Sophia was diagnosed with Autism at age 3, intellectual disability at age 6, and OCD and ADHD at age 8. Her pediatrician suspects a possible diagnosis of Cushing’s syndrome. Sophia has no prior history of feeding therapy. A bedside feeding evaluation and skilled meal observation were completed to assess her feeding and swallowing skills in more detail.

She tends to overstuff her mouth, eats rapidly, and often needs verbal prompts to slow down; on some occasions, she eats so quickly that she vomits. Her eating schedule is irregular, typically consisting of breakfast, dinner, and a late night snack. Sophia was diagnosed with Autism at age 3, intellectual disability at age 6, and OCD and ADHD at age 8. Her pediatrician suspects a possible diagnosis of Cushing’s syndrome. ORAL MECH EXAM: Sophia’s oral structure and function present with some structural and functional differences; however, they appear adequate for feeding and swallowing at this time. Observations include an overjet bite, a flat, shallow, and wide palate, significant gapping between the front teeth, and spacing between other teeth. No tethered oral tissues were observed or reported. Functionally, Sophia demonstrates decreased tongue coordination and reduced functional tongue movements, which may impact her feeding habits (overstuffing and fast rate of intake). FEEDING/SWALLOWING: A bedside swallow evaluation and skilled meal feeding observation revealed that Sophia presents with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA. Sophia’s PFD is characterized by impaired oral intake that is not age appropriate and appears to be influenced by several factors, including medical history (e.g., Autism, ID, OCD, ADHD, reflux, constipation), nutritional deficits (limited food inventory), feeding skill impairments (sensory sensitivities to foods and decreased oral motor coordination), and psychosocial dysfunction (problematic mealtime behaviors). Sophia’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. These deficits likely contribute to her overstuffing behavior, which appears to be a compensatory strategy for her reduced oral motor abilities. FOOD REPERTOIRE: Sophia is reported to prefer foods that are crisp, crunchy, or hard. She shows a strong taste preference for salty flavors and favors cool/warm foods. Her appetite is reported to be good. Currently, Sophia’s food inventory consists of only 14 foods. For proteins, she accepts bacon, hot dogs, bologna, and cheese (only on pizza). She does not consume vegetables. For fruits, Sophia eats apples, pears, bananas, grapes, and oranges. For starches, Sophia eats crackers, chips, bread, french fries, pancakes, and pizza. In terms of liquids, she only drinks 2 beverages including water and orange juice. It is important to note that Sophia previously accepted a wider variety of foods, but has been consistently dropping items from her diet. Sophia’s mother expressed a desire for her to eat more proteins and salads. NORM REFERENCED ASSESSMENT: Sophia’s PediEAT (Pediatric Eating Assessment Tool) was not administered due to her age exceeding the tool’s intended range and limited evaluation time after her mother had forgotten the completed paperwork. Instead, the mother was interviewed to capture relevant information, describing both physiological symptoms (e.g., vomiting, rapid eating) and problematic mealtime behaviors (e.g., extreme picky eating, schedule irregularities), thereby providing valuable insight in lieu of formal PediEAT scoring. MEALTIME FEEDING OBSERVATION: Sophia was seated at the feeding table and allowed to self-feed her preferred items, during which she displayed a rapid rate of intake and habitual overstuffing of food in her mouth. Her mother confirmed that these behaviors are consistent at home, noting that Sophia typically relies on electronic distractions to eat. When presented with non-preferred foods, Sophia exhibited immediate refusal behaviors as she attempted to escape her seat, covered her nose, turned away, and tried to shut the container lid. With clinician guidance, she briefly tolerated touching the non-preferred foods with her fingertips, but only in an effort to push them away. At higher levels of the eating progression hierarchy, Sophia refused to engage with any of the non-preferred items. SWALLOW/ORAL MOTOR EVALUATION: Sophia demonstrated decreased oral motor skills with the consistencies offered during the evaluation. Her chewing pattern was observed to be primarily vertical, utilizing a basic munching motion, particularly when her oral cavity became overstuffed. She also exhibited decreased and disorganized tongue lateralization during chewing. Slightly delayed anterior posterior transit of preferred foods was noted, which may be attributed to prolonged chewing due to overstuffing. No clinical signs or symptoms of aspiration or penetration were observed during the session. Sophia’s mother stated she is concerned that Sophia will choke due to her fast rate of intake and overstuffing habits. However, Sophia has never had a choking episode and also does not have a history of aspiration pneumonia. Based on the foods trialed and the behaviors observed during the evaluation, Sophia’s oral motor skills appear to be reduced and are likely impacting her feeding function. IMPORTANT NOTE ON PFD DIAGNOSIS: Although the diagnostic criteria for PFD are formally defined for individuals up to 17 years of age, Sophia continues to demonstrate feeding difficulties that began in early childhood and remain consistent with the clinical features of PFD. The longstanding nature of the disorder, its persistence across developmental stages, and its ongoing impact on nutrition, hydration, and functional participation support the continued use of PFD as a secondary diagnosis. Assigning PFD acknowledges the chronicity and developmental origin of the feeding difficulties while allowing the primary diagnosis (Oral Dysphagia) to reflect her current age and medical status. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 minutes to address Sophia’s CHRONIC PFD and ORAL DYSPHAGIA. Sophia is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child of Sophia’s age is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. In addition, food chaining strategies should be implemented to expand Sophia’s food repertoire by building on existing preferred foods. Parent education on mealtime behaviors and safe swallow precautions (slow rate of intake, small bites, decreased distractions, etc.) will also be a key component of therapy. Feeding Assessment Summary: Sophia is now 19 years 4 months old and is due for a review of her feeding skills since beginning skilled intervention 6 months ago. According to mother, Sophia was diagnosed with Autism at age 3, intellectual disability at age 6, and OCD and ADHD at age 8. Her pediatrician suspects a possible diagnosis of Cushing’s syndrome. Sophia has no prior history of feeding therapy. However, recently began seeking services from a certified nutrition/dietician. A bedside swallow evaluation and skilled meal feeding observation revealed that Sophia continues to present with a CHRONIC PEDIATRIC FEEDING DISORDER (PFD) and ORAL DYSPHAGIA as characterized by impaired oral intake that is not age appropriate and is associated with medical diagnosis (Autism), nutritional status (limited nutritional intake), feeding skill (sensory sensitivities), and/or psychosocial dysfunction (problematic mealtime/learned responses). Although Sophia’s oral structure and function present with some structural and functional differences, they appear adequate for feeding and swallowing at this time. Functionally, Sophia has demonstrated decreased tongue coordination and reduced functional tongue movements, which may have impacted her feeding habits (i.e., overstuffing and fast rate of intake). FOOD REPERTOIRE Sophia’s food inventory: PROTEIN: bacon, hot dogs, bologna, cheese pizza, mozzarella cheese; STARCHES: chips (corn only), crackers, bread, french fries (Whataburger only); FRUITS: Β­ apples, pear, banana, grapes, orange; VEGETABLES: None. In January 2026, Sophia was seen by a dietician, who agreed with the findings from Sophia’s initial feeding evaluation. The dietician recommended an increase of fruits into Sophia’s diet and more exposure to vegetables. According to her mother, Sophia continues to maintain minimal interest with the family’s typically preferred foods during mealtimes, and Sophia will join the family as they gather together for meals. As per the dietician’s recommendations, Sophia’s mother has begun including fruit slices on Sophia’s plate during mealtimes. Although Sophia does not always consume the fruit, she has increased her tolerance of the items on her plate. Due to these small changes, mother has stated that Sophia has lost 8 pounds since beginning feeding therapy, along with the incorporation of the dietician’s recommendations. MEALTIME FEEDING OBSERVATION During Sophia’s bedside feeding and swallowing evaluation, she was presented with non-preferred food items (bologna sandwich, whole pear, corn on the cob, and raw broccoli), and preferred food items (hot dogs, and corn chips). Sophia did not vocally protest, and followed the visual schedule, preparing the non-preferred food items by handing item to clinician when ready to begin interacting with foods. She required maximum verbal cues and prompts to engage with foods via touching, picking up, touching the food to her lips and taking a small bite. Sophia made a bologna sandwich, used the cookie cutter to cut out a shape, she used a butter knife to cut slices of her pear. She used her hands and a fork to pick up the non-preferred foods, smelled them, and took small bites. Sophia tolerated the sight and smell of the raw broccoli, using her fingers to tear the broccoli into smaller pieces. Sophia displayed a rapid rate of intake and habitual overstuffing of food in her mouth when given the opportunity to consume her preferred foods. However, when reminded of the visual schedule to slow down, take small bites, and not to overstuff her mouth, Sophia followed the cues and prompts. According to her mother, Sophia has made an effort to slow down her food intake at home and her mother has worked on taking away the electronic devices during mealtimes. However, Sophia still maintains a poor eating schedule, often due to Sophia’s difficulty sleeping. When presented with non-preferred foods, Sophia did not exhibit immediate refusal behaviors nor did she attempted to escape her seat. Although she covered her nose, and turned away, Sophia did not try to shut the container lid. With clinician guidance, she briefly tolerated touching the non-preferred foods with her fingertips, examining and cutting the non-preferred foods with her fingertip before placing them in the throwaway bowl. At higher levels of the eating progression hierarchy, Sophia refused to engage with any of the non-preferred items. SWALLOW/ORAL MOTOR EVALUATION: Sophia demonstrated decreased oral motor skills with the consistencies offered during the evaluation. Her chewing pattern was observed to be primarily vertical, utilizing a basic munching motion, particularly when her oral cavity became overstuffed. She continued to exhibit decreased and disorganized tongue lateralization during chewing. Slightly delayed anterior posterior transit of preferred foods was noted, which may be attributed to prolonged chewing due to overstuffing. As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

She only has sensory issues with textures and smells
