Occupational Background History: Isabella is a 2.2 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother, was present in order to serve as the primary informant throughout the evaluation. Per mother's report child was born at 40 weeks gestation. Child did not experience any complications at birth, does not have any visual or hearing deficits. Child does not have history of aggression and seasonal allergies.

Isabella demos retained primitive reflexes hindering her ability to self-regulate and execute body movements necessary to participate in daily activities: babinski, rooting and STNR and ATNR reflexes. Assessment of Goals/Response to Treatment: Patient presented to the session alert and oriented, demonstrating readiness to engage in therapeutic activities. Skilled occupational therapy intervention focused on improving motor planning, bilateral integration, visual-motor skills, and functional self-care. The patient actively participated in a structured sorting and matching activity, requiring only minimal verbal and physical cues to facilitate sequencing, sustained attention, and fine motor precision. Progress toward the goal of improved bilateral coordination is evident, with the patient now demonstrating 75% achievement. This was highlighted by her ability to successfully string 20 out of 20 wooden beads onto a limp string with minimal assistance, a significant improvement from her baseline requiring maximum assistance. During a scissor practice task, the patient required moderate verbal and physical assistance to address lead/assist hand use and visual-motor integration. When addressing pre-writing skills, the patient continues to require moderate verbal cues and physical assistance for imitating lines and a circle, and maximum assistance to form a cross, indicating an ongoing need to develop praxis and visual-motor control. In the area of self-care, the patient has progressed to requiring only standby assistance for snaps, while moderate assistance is needed for zippers and buttons, and maximum assistance remains necessary for buckles, reflecting gains in in-hand manipulation and finger strength. Positive reinforcement was utilized throughout the session to promote task initiation and completion.

The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.
