Feeding Background History: Samuel is an 11 year 3 month old male referred for a feeding evaluation by his pediatrician due to avoidant food intake. His parents, who are recently divorced, served as informants and reported long standing feeding difficulties beginning at birth, as Samuel was born prematurely and experienced respiratory distress syndrome, impacting his suck swallow breathe coordination. Over time, he developed sensory sensitivities, initially to clothing, which progressed to food related sensitivities. Since the introduction of solids, Samuel has primarily consumed beige foods and has an extremely limited food inventory of about 10 items that continues to shrink. Samuel will only eat his preferred foods if they are served hot, refusing room temperature foods. Additionally, Samuel often skips breakfast and does not consume lunch at school. He was diagnosed with ARFID in February 2024, Asthma in March 2024, Benign Rolandic Epilepsy in May 2024, and Autism Spectrum Disorder in that same year. He has no prior history of feeding therapy. A bedside feeding evaluation and skilled meal observation were completed to assess his feeding and swallowing skills in more detail.
Feeding Medical History: Samuel was born prematurely at 33 weeks gestation via vaginal delivery. Ms. Pinon’s water broke at 29 weeks, which necessitated a 4 week hospital bed rest. Following his birth, Samuel spent 4 weeks in the Neonatal Intensive Care Unit (NICU).
Feeding Medications: Samuel is prescribed daily Albuterol via SpiriuA inhaler for asthma management. For seizure control, he uses Valtoco as a rescue medication and midazolam as an additional rescue medication.

Samuel’s PFD is characterized by impaired oral intake that is not age appropriate and appears to be influenced by several factors, including medical history (e.g.,respiratory distress syndrome, premature birth, asthma, autism, ARFID), nutritional deficits (limited food inventory), feeding skill impairments (sensory sensitivities to foods and decreased oral motor coordination), and psychosocial dysfunction (problematic mealtime behaviors). Samuel’s oral dysphagia is characterized by decreased oral motor skills required for efficient mastication. These deficits likely contribute to his overstuffing behavior, which appears to be a compensatory strategy for his reduced oral motor abilities. FOOD REPERTOIRE: Samuel is reported to prefer foods that are crisp or chewy. He shows a strong taste preference for salty, sweet, and bland flavors and favors hot/warm foods. His appetite is reported to be poor to variable. Currently, Samuel’s food inventory consists of only 10 foods. For proteins, he accepts bacon and mozzarella cheese (only on pizza). He does not consume fruits or vegetables. For starches, Samuel eats homemade bread, pizza crust, flour tortillas, french fries, hash browns, pancakes, and waffles. In terms of liquids, he only drinks 4 to 5 beverages including milk shakes, water, soda, and lemonade. It is important to note that Samuel previously accepted a wider variety of foods including chicken nuggets, chips, cookies, popcorn, and dry cereal, but has been consistently dropping items from his diet. Samuel’s mother and father expressed a desire for him to eat more meats, proteins, vegetables, and fruits. NORM REFERENCED ASSESSMENT: Mr and Ms. Pinon completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver questionnaire for children ages 6 months to 7 years. Since Samuel is outside of the age limit for this assessment , the findings should be interpreted descriptively. Samuel received an overall rating of β€œconcern,” with β€œhigh concern” noted in the areas of physiologic symptoms and problematic mealtime behaviors. These caregiverΒ­reported concerns aligned with clinical observations during the evaluation. MEALTIME FEEDING OBSERVATION: Samuel was seated at the feeding table and observed to self Β­feed his preferred food items. He demonstrated a fast rate of intake and habitual overstuffing of food in his mouth. Samuel only agreed to eat his bread as he stated his pizza, fries, and waffles were too cold. When presented with nonΒ­ preferred foods (room temperature foods), Samuel showed initial hesitancy but was able to interact with them using his hands, fingers, and parts of his face. With clinician support, he tolerated some contact with the foods on his lips and tongue via the use of his finger. However, Samuel consistently refused to allow non Β­preferred foods to be placed on his teeth or inside his oral cavity. He also demonstrated difficulty describing the sensory characteristics of the foods, which may indicate limited sensory awareness or underdeveloped foodΒ­related language. SWALLOW/ORAL MOTOR EVALUATION: Samuel demonstrated decreased oral motor skills with the consistencies offered during the evaluation. His chewing pattern was observed to be primarily vertical, utilizing a basic munching motion, particularly when his oral cavity became overstuffed. He also exhibited decreased tongue lateralization during chewing. Slightly delayed anteriorΒ­ posterior transit of preferred foods was noted, which may be attributed to prolonged chewing due to overstuffing. No clinical signs or symptoms of aspiration or penetration were observed during the session. Mr and Ms Pinon have not observed any concerns related to Samuel’s swallowing at home, including no episodes of coughing, choking, or signs of significant distress during meals. Samuel also does not have a history of aspiration pneumonia. Based on the foods trialed and the behaviors observed during the evaluation, Samuel’s oral motor skills appear to be reduced and are likely impacting both his feeding function. RECOMMENDATIONS: At this time, feeding therapy is recommended at a frequency of once per week for 30 minutes to address Samuel’s CHRONIC PFD and ORAL DYSPHAGIA. Samuel is at risk for compromised health and development due to a significantly limited food inventory and persistent picky eating behaviors. At his age, a child is typically expected to accept 30 or more foods across various food groups. Therapy will focus on increasing food acceptance through systematic desensitization techniques and the Steps to Eating hierarchy. In addition, food chaining strategies should be implemented to expand Samuel’s food repertoire by building on existing preferred foods. Parent education and oral motor desensitization will also be a key component of therapy. Assessment of Goals/Response to Treatment: The patient presented alert, oriented, and cooperative for the therapeutic session. He demonstrated significant progress toward improving tolerance and acceptance of non-preferred foods. The patient successfully tolerated tactile interaction, touching non-preferred foods to parts of his face, such as his chin and cheeks, in 8 out of 10 opportunities. Building on this, he further demonstrated progress with intraoral exploration by touching non-preferred foods to his lips and tongue in 8 out of 10 opportunities. During this session, Oreo cookies were incorporated for the fourth time and marshmallows were introduced for the first time; the patient accepted both food items into his oral cavity and was able to hold them for 3-5 seconds without signs of distress, indicating a notable increase in oral tolerance. To further reduce sensory sensitivities and build tolerance for feeding activities, the patient completed a series of facial and intraoral stimulation exercises utilizing a Z-vibe in 8 out of 10 given opportunities. Clinician provided parent counseling regarding the home management intervention program. Parents reported successful and accurate incorporation of recommended strategies into daily and weekly routines with no assistance needed, which is critical for the generalization of skills into the home environment.

It has been brought to my attention that during Samuel’s recent feeding evaluation, Mr. Pinon recorded the session without consent. This action is in direct violation of our company policiesβ€”which you signed and agreed to prior to beginning servicesβ€”and is also a violation of the Electronic Communications Privacy Act of 1986, a federal law that prohibits audio recordings without the consent of all parties involved in a private setting. In this instance, consent was not provided by the company, the evaluating therapist, or, according to messages received, by Mrs. Pinon.

There are clearly posted signs throughout our facility prohibiting unauthorized recordings, and our policies are designed to protect the privacy, safety, and rights of all parties. Violation of these policies may result in immediate discharge from all therapy services.

We are requiring the following actions immediately:
1. All copies of the unauthorized recording must be deleted from all devices and any cloud storage.
2. Written and signed confirmation must be provided to our office confirming that no copies remain and that the recording has not been shared with any third party.
3. An explanation in writing regarding this incident must be submitted to our office.
We explicitly insist that Mr. Pinon refrain from sharing, disclosing, or distributing the recording in any form. Failure to comply will escalate our response and may result in legal action in addition to termination of services.

Our policies are in place to ensure the protection, privacy, and dignity of our patients, their families, and our staff. This, or any other policy violation, cannot and will not be allowed to continue at our clinic.

In addition, we would like to remind you of our Zero Tolerance for Aggression Policy, we will not tolerate any form of aggression toward our staff or therapists. Our team is here to serve our patients, including your child, and we expect all interactions to be conducted with care and mutual respect.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

I could potentially try and offer Thursday August 7th at 4pm next week for the evaluation, I would just need to confirm with two parents if they can move their sessions for next week, let me know if you would like me to try. I also wanted to let you know after therapy last time with the dominos pizza. Sam has stopped eating pizza from there. So we lost a place. I'm not sure why. We are now down to only pizza at Dion's on the west side, French fries and milk shakes. Breakfast pancakes at Mc Donald's if I don't have to be to work early I try to get those for him.
