Speech Background History: Liam is a 1 year 9 month old male who was referred for a speech and language evaluation by his pediatrician and mother’s concerns regarding speech delay. Liam was accompanied by his mother, Mrs. Smith, who served as the informant for the evaluation. According to mother, Liam has suffered numerous ear infections, and has 4 in 2025 already. Due to these infections, Liam will be evaluated by an ENT in August 2025 and will, according to mother, have PE tubes placed in both ears. In a recent audiological exam, Liam was found to display inconsistent hearing in high frequency sounds. It was strongly recommended that the family follow through with the recommended ENT evaluation due to Liam’s potential hearing concerns. Mrs. Smith reported that Liam utilizes 5-10 words and/or word approximations, but is inconsistent. Liam will gesture and/or use open mouth vocalizations to express his communication wants/needs to his communication partner(s). Based on clinical observations, Liam’s current communication was characterized as emerging use of 1-word phrases, utilizing word approximations or open mouth vocalizations, while pointing and/or grabbing for desired object(s) to communicate wants/needs. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Liam’s receptive and expressive language skills. Mother stated that Liam is exposed to English and Spanish equally by his caregivers, and immediate family, but favor’s English due to father only speaking English. Based on Liam’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Liam has suffered with numerous ear infections since birth. Most recently, Liam has suffered from 4 ear infections since Jan. 2025 and will see an ENT for the possibility of PE tube placement.

Speech Background Summary: The evaluation took place at the therapy room table and floor where Liam required some redirection. Liam demonstrated decreased social communication skills, which may be attributed to his decreased vocabulary and verbal output.

It is recommended that Liam’s treating clinician continue to monitor his oral cavity during treatment and perform an oral mech exam when patient permits. The Preschool Language Scales Fifth Edition (PLS-5) was used to evaluate Liam’s receptive and expressive language skills. Liam received the following standard scores on the PLS-5: Auditory Comprehension 92 (percentile: 30, age-equivalent: 1 year 6 months), Expressive Communication 72 (percentile: 3, age-equivalent: 1 year 0 months), and Total Language Score 81 (percentile: 10, age-equivalent: 1 year 3 months). According to the standard scores on the PLS-5, where a score from 85 to 115 is considered within functional limits, Liam presented with an overall MILD-MODERATE EXPRESSIVE LANGUAGE DISORDER, where is expressive (MODERATE) is more severe than his total language score. Liam would greatly benefit from speech therapy services that address the following: imitation of gestures/actions, imitation of speech sounds, imitate monosyllabic words, naming/labeling via word approximations, and use of total communication for a variety of pragmatic functions. At this age, Liam should be communicating in 1-word phrases. Speech therapy is highly recommended at this time as Liam’s current speech and language skills are NOT DEVELOPMENTALLY FUNCTIONAL for his age and for safety. Without speech therapy, Liam’s safety risk is high since he will likely not be understood if he needs to communicate pain, communicate need for assistance, or understand safety personnel/directions. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Liam demonstrates deficits in fine motor precision, fine motor integration, visual motor integration, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Child would greatly benefit from skilled OT interventions in order to address their established delays and facilitate functional independence and performance. Home programming alone is not sufficient to make the needed progress, direct one to one services are required for progress to be achieved. Results of the evaluation were reported and explained to mother, along w/ the diagnosis, prognosis, including the frequency, time and duration of treatment and contents of the plan of care. Family will be provided w/ a verbal report of progress noted along w/ any homework for the household and/or pt. as part of a home program (if applicable) at the end of every session. Mother verbalized understanding of treatment plan and agreed to the initiation of skilled therapy.
Occupational Progress to Date: Liam demonstrates fair and consistent progress with participation in skilled occupational therapy services evident by meeting all previous goals with at least 70% accuracy, per improved standardized assessment scores, and per clinical observations and per parental report. In 3 months, Liam will increase body awareness and safety during functional mobility as evidenced by transferring between 2-­4 tasks with MOD assist/verbal cues, 3/4 trials and per parental report. Assessment of Goals/Response to Treatment: The patient presented as friendly and cooperative, demonstrating positive engagement throughout the session. Skilled occupational therapy intervention focused on advancing motor, psychosocial, cognitive, and visual processing skills through structured sensorimotor play and standardized assessment. Progress toward functional mobility and transition goals was observed during the administration of the PDMS-2, where the patient successfully transitioned between four distinct subtest activities requiring only minimal verbal cues and setup assistance in 3/4 trials. This marks significant improvement from the baseline of requiring maximal assistance to shift between tasks. To address sensory integration, the patient engaged with slime, a novel tactile medium. He demonstrated improved tolerance by manipulating the material with both hands for 5 minutes, exhibiting only mild, transient aversive behaviors (e.g., initial brief hand withdrawal) that were overcome with minimal verbal encouragement. This progress from a baseline of maximal aversion is a positive indicator for increased participation in grooming tasks. Furthermore, social reciprocity skills were targeted through a turn-taking paradigm during the slime activity. The patient successfully participated in a 3-minute reciprocal game, requiring only minimal verbal cues to share and await his turn in 3/4 trials, with no tantrums observed. This demonstrates notable progress toward sustained engagement in social activities, an improvement from the baseline of requiring maximal cues. The session concluded with the completion of the PDMS-2 by the COTA/L under the supervision of the OTR to gather standardized data for the upcoming re-evaluation, which will be used to further refine the plan of care.

As always, we at New Beginnings strive to keep our patients, parents, therapists, and staff safe and healthy. We have thermometers in our front lobbies and will be checking for high temperatures as we begin the traditional time of year for an increase in flu, colds, and COVID. Any child or parent with a temperature of 100 degrees and above will be asked to reschedule for another day & time.
If you or your child or someone in your home has COVID, we continue to ask that you stay home and quarantine to stop the spread. The CDC urges people sick with symptoms of respiratory viruses to stay home until their fever has disappeared for at least 24 hours without relying on fever-lowering medications.

Please do not come to therapy if you or your child are experiencing the following symptoms: fever, body aches, chills, vomiting, diarrhea, headache. Your treating therapist would be happy to reschedule your visit for you.

Liam has been diagnosed with a moderate expressive language disorder. He would greatly benefit from services targeting: imitating consonant-vowel (CV) and CVCV sounds, imitating speech sounds, imitating monosyllabic words, and naming basic common core vocabulary (items around his everyday environment). Once the report has processed, we have obtained the needed signatures, and a therapist has been assigned, the treating clinician will reach out to you to establish a schedule to begin speech services. 08-19-2025 09:59 AMKaitlin MarslandLiam did very well :) he was shy so we just took our time today exploring things and getting him comfortable but he participated in all the activities and I got him to go down the slide and crawl through the tunnel to work on some motor planning. He also did a few fine motor activities and he did well with them and matched his colors. He was a little nervous about the swing and to sit/bounce on the ball or trampoline but we will get him exploring everything to increase his gross motor skills. He even did well with sharing a ball and allowing me to show him different ways to play.

The big thing we have seen at home with him is you can't correct him he gets extremely upset and goes and hides behind the couch or throws anything he has on his hands or hurts himself and that's been a struggle at home
08-26-2025 01:07 PMKaitlin MarslandI have found using positive directives can help reduce the tantrums because often times the word "no" itself sends children into a tantrum when they might not even understand why there is a "no" but they associate it as negative and it increases tantrums. For example, if he were hitting a toy against a table or other object, instead of saying "no, don't do that" I would say " Lets bang on the drum instead of the table!" This gives him a clear option of what he is allowed to do and usually redirects him from the thing we don't want him to do.

He completed all the activities and we worked on crossing midline by picking up objects with one hand and handing it to the other hand to increase bilateral coordination. We also did some gross motor activities in the gym and he wasn't demonstrating signs of retained primitive reflexes so he is doing great :) We also worked on 2 and 3 step sequence tasks and he did really well following directions

It was reported to me from his school, that he doesn't want to engage with other kids, he always separates himself. Also he throws his food on the floor everyday.

He may still be in the parallel play stage where he can play near other children or doing the same activity but not together with another child. As he is around other children more with school he will begin to engage with others. As for throwing food, he may be doing it in a way to gain attention as typically people respond right away when we drop food on the floor. Or he may not like the food and is just engaging in play. It is hard for me to say what his motivation is for dropping the food since I am not able to witness what is happening before/after to determine his motivation. I would give him a “no” bowl so he can put whatever he doesn’t want to eat in the bowl instead of dropping it

I also gave him cues to use a “helper hand” to stabilize objects with his left hand while he used his right hand to manipulate. He would benefit from more practice of this skill at home as it will help when he gets to writing/coloring and manipulating buttons or other fasteners :)

He sometimes shows a "floating hand" so one hand is manipulating and the other is just held in the air so I just guide his floating hand to hold the other side of the object he's manipulating to cue using his "helper hand". We also practiced lacing beads and he only needed a little help to pull the string on the opposite side. I introduced some sensory play today with kinetic sand and slime and he seemed a little aversive to the textures but with modeling how to wipe his hands together he was more comfortable touching the sand. I like incorporating sensory play because it can help with tactile and food aversions :)
