Josephine’s mother served as the informant for this evaluation. According to mother, Josepine was introduced to purees and soft solids at 5 months old. Mrs. Gagliardo reported Josephine does not like purees and has difficulty with thicker consistencies. Josephine's primary source of nutrition is breastfeeding. Feeding Diagnosis/Impressions: Josephine is a 7-month-old female who was referred by her pediatrician for concerns regarding feeding difficulties and weight. Josephine’s mother served as the informant for this evaluation. According to mother, Josepine was introduced to purees and soft solids at 5 months old. Mrs. Gagliardo reported Josephine does not like purees and has difficulty with thicker consistencies. Josephine's primary source of nutrition is breastfeeding. A bedside feeding evaluation and skilled meal feeding observation were utilized to assess Josephine’s feeding/swallowing skills. Born full-term at 39 weeks via vaginal birth. Josephine was diagnosed with laryngomalacia.A formal oral peripheral exam could not be completed due to Josephine’s young age. However, the following observations were made: possible lingual/labial tethered oral tissues (TOTs), high narrow palate, small oral cavity, decreased muscle tone, and some lower dentition. At this time, Josephine’s oral structure/function is considered inadequate due to her possible TOTs. Clinician educated parent to seek further assessment/treatment with a pediatric dentist to determine if her TOTs require treatment. A bedside swallow evaluation and skilled meal feeding observation revealed that Josephine presents with OROPHARYNGEAL DYSPHAGIA and an ACUTE PEDIATRIC FEEDING DISORDER. It is characterized by impaired oral intake that is not age appropriate and is associated with medical diagnosis (low weight), nutritional status (parent reported limited variety of nutritional intake), feeding skill (sensory difficulties with foods, decreased oral feeding skills for age appropriate consistencies), and/or psychosocial dysfunction. FOOD REPERTOIRE: According to her mother, Josephine consumes around 10 foods and 1 liquid, with her complete food inventory documented in the feeding intake paperwork. The following are Josephine’s accepted food items: PROTEINS Chicken strips and bacon. STARCHES crackers, mashed potatoes, french fries, . FRUITS Apple, blueberries, pear, black berries (all in puree forms). VEGETABLES Green beans, carrots, broccoli, corn, peas (all in puree forms) LIQUIDS Water. Children who are Josephine's age are expected to consume 30 or more healthy foods across categories to gain the nutrients needed that are important for growth and development. It is important to note that the majority of Josephine’s accepted solids are starches and are limited in nutritional variety. Josephine’s caloric daily intake is unknown; however, based on the information provided, Josephine’s current food inventory doesn't meet the average food inventory for her age. NORM REFERENCED ASSESSMENT: Mrs. Gagliardo completed the Pediatric Eating Assessment Tool (PediEAT), a validated caregiver questionnaire for children ages 6 months to 7 years. Josephine received a rating of "No concern". Josephine’s ratings on the PediEAT are not consistent with her performance in the evaluation and diagnosis. MEALTIME FEEDING OBSERVATION: Josephine was seated in a high chair with appropriate 909090 positioning and was fed by her mother using a spoon and by hand. When presented with all trials of foods, Josephine turned her head away. Mother fed her at least one spoonful of each food item. Josephine was observed to gag with all consistencies. With the clinician’s support, Josephine interacted with the non-preferred veggie puree with her spoon; however, it was very brief. Mother breastfed Josephine in a side-lying position. SWALLOW/ORAL MOTOR EVALUATION: The consistencies trialed included veggie puree, meat puree, oatmeal, and meltables. Josephine demonstrated significantly decreased oral motor skills, including a phaisc bite pattern, openmouth chewing, minimal tongue lateralization, and oral residue. Mrs. Gagliardo does not report unexplained fevers or history of aspiration pneumonia. Josephine’s pharyngeal/esophageal swallow function should be monitored by her treating clinician to ensure safety and determine if further testing is warranted. At this age, a child should demonstrate a more developed munching pattern and management of thicker purees and soft solids. Gag reflex should be active but should be less sensitive. Josephine’s oral motor skills are NOT WITHIN FUNCTIONAL LIMITS for her age and/or feeding safety. RECOMMENDATIONS: Feeding therapy is highly recommended at this time, and should target increasing Josephine’s tolerance to non?preferred foods via steps to eating hierarchy and desensitization methods. Feeding therapy should also target improving Josephine’s oral motor skills for safe and effective feeding/swallowing. Parent education and mealtime behavior will also be targeted to support feeding development. Josephine is due for a feeding re-evaluation to determine if continued feeding therapy is warranted. A bedside feeding evaluation and skilled meal feeding observation were utilized to reassess Josephine’s feeding/swallowing skills. PROGRESS TO DATE: During this authorization period, Josephine is consuming solids of various consistencies to include purees, soft mechanicals, and hard mechanicals. Josephine consumes foods from all nutrition food groups and has at least 10 foods per nutrition food group (proteins, starches, fruits, and vegetables) in her food inventory. Josephine’s oral motor skills are age-appropriate for safe and efficient feeding/swallowing of solids. There is improved family mealtime behavior/routine and parent education on pediatric feeding disorders and dysphagia. FEEDING/SWALLOWING: Based on the bedside swallow evaluation and skilled meal observation, Josephine presents with feeding/swallowing skills that are WITHIN FUNCTIONAL LIMITS for her age. Her pediatrician did set up this referrals before we stated on baby foods, but Josi is not a big fan of purees since we have started. She is exclusively breast fed, so any help is greatly appreciated!

07-28-2025 10:58 AM	Andrea Rodriguez Stokes	Good morning! This is Andrea from New Beginnings. Here is a brief summary from the evaluation. Josephine presents with an acute pediatric feeding disorder. Feeding therapy is highly recommended at this time, and should target increasing Josephine’s tolerance to nonpreferred foods via steps to eating hierarchy and desensitization methods. Feeding therapy should also target improving Josephine’s oral motor skills for safe and effective feeding/swallowing. Parent education and mealtime behavior will also be targeted to support feeding development. Whenever you have a moment to bring in the feeding intake forms, I will go ahead and add them to her report. From there, I will send everything to her primary physician. Once the office has processed the report, obtained the needed signatures, and insurance approves the report, I will reach out to you to schedule your first session. If you have any questions, please reach out. Thank you :)

Example: • Steamed carrot sticks, sweet potato fries, banana strips, avocado slices. • Let her touch, squish, smear, and mouth the food without pressure to eat. • Use a silicone plate with sections so she can explore different textures. Why it helps: Encourages hand-eye coordination, sensory exploration, and self-regulation. 2. Food Paint Play Goal: Sensory exploration of textures and colors. • Use foods like mashed peas, beets, pumpkin, or yogurt. • Let Josephine “paint” on a tray or a highchair surface with her hands. • Supervise closely—this is taste-safe and mess-positive! 3. Dip & Taste Goal: Build understanding of hand-to-mouth motion. • Offer toast sticks or banana strips and a bowl of yogurt or hummus. • Show her how to dip and encourage her to imitate. • No pressure if she just plays with it! 4. Preloaded Spoon Practice Goal: Early utensil use and hand-mouth coordination. • Preload a soft-tipped spoon with yogurt, applesauce, or oatmeal. • Hand it to baby by the handle or lay it on the tray. • Encourage her to grab it and explore. 5. Highchair Water Bag Goal: Promote fine motor skills while seated. • Fill a ziplock bag with water and a few small floating toys or slices of cucumber. • Seal tight with duct tape and place it on her highchair tray. • Let her push and pat the squishy bag 6. Sink or Tub Splash Time Goal: Freedom to explore water and develop upper body control. • Let her sit in a clean sink or small tub (with supervision). • Give cups, ladles, or rubber ducks to manipulate and grasp. • Let her splash, pour, and observe how things move in water. Here are a few options you can use at home! Talk to her and model actions while she explores: “You’re touching the banana—it’s soft and squishy!”. Offer food when she’s calm, alert, and not overtired. Avoid distractions (no screens or loud music). Thank you and see you next week!
