Her parents, Mr. and Mrs. Moreno, reported that she currently says only one word, “momma,” and although she has produced a few other words in the past, they were only heard once or twice and not repeated. Evelyn does not imitate language and tends to walk away when her parents attempt to engage her verbally. She was previously evaluated by Early Childhood Intervention (ECI), where her parents were told she may have Autism; however, they felt that the assessment conversations did not align with the written report. Evelyn did not receive therapy through ECI and has no prior history of speech therapy. During the evaluation, she used some purposeful vocalizations and crying to express wants and needs. Speech Medical History: Evelyn was born at 36 weeks gestation via emergency c-section due to her mother falling on her stomach. 2 blood transfusions were required due to low iron.

Evelyn would greatly benefit from speech therapy services that address the following functionally appropriate concepts: completion of play tasks, following routine directions, identifying common objects, imitation of functional gestures, imitation of environmental sounds, and use of functional communication to express wants/needs. Speech Assessment Summary: **Due to a change/pause in insurance which resulted in a pause in visits for approximately two months, not all goals were met. Current treating therapist to continue with goals to allow opportunity for proper execution and measurement of goals due to insufficient time in the prior period. Evelyn is a 2 year, 4 month old female that has been receiving speech and language therapy at New Beginnings Pediatric Therapy services and was re-evaluated to determine need for continued services. During the evaluation, patient required moderate to maximum prompts to participate due to decreased engagement/participation. Due to decreased participation some portions of the PLS-5 were completed via caregiver report and observation during previous sessions. During the assessment, Evelyn was re-evaluated using the Preschool Language Scales Fifth Edition. On the PLS-5, Evelyn obtained the following scores: Auditory Comprehension: 63 (raw score: 18, percentile rank: 1, age equivalent: 1-2), Expressive Communication: 59 (raw score: 15, percentile rank: 1, age equivalent: 0-10), and Total Language: 58 (raw score: 122, percentile rank: 1, age equivalent: 0-11). Evelyn’s scores are ate least -2 standard deviations below the mean. Based on scores reported, Evelyn presents with a SEVERE MIXED RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. Scores should be viewed with caution due to portions of the assessment completed via caregiver report due to Evelyn’s decreased participation. Since previous re-evaluation, Evelyn has made measurable progress as evidenced by an increase in expressive communication raw score, and increase in expressive standard score, and an increase in total language standard score. Mother reported that Evelyn now only uses a pacifier at night. Education will continue to be provided on prolonged pacifier use. Not all goals were met during authorization period due to a pause in insurance eligibility. Similar goals to be targeted during new authorization due to not enough sufficient time to target goals due to a pause in insurance. Evelyn’s expressive and receptive language skills are still not functional for her age. Speech therapy continues to be warranted to promote safety and independence due to Evelyn’s high safety skill. Parents will remain responsible for implementing the Home Education Program (HEP).

Evelyn used gestures OR purposeful vocalizations to request her wants/needs to the clinician in 1 out of 10 opportunities with maximum verbal cues. Evelyn followed 1 step directions during play tasks in 2 out of 10 opportunities with maximum verbal and gestural cues. When counseled by the clinician, parents incorporated the home management intervention program as a daily and weekly activity with 80% accuracy to transfer skills learned in therapy into the home environment. Home management training optimized patients functional communication tasks in the home.

07-22-2025 10:09 PM	Evelyn Moreno	Evelyn (Prim) Replied: 1. Evelyn Moreno 2. When I was around 36 weeks pregnant I lost balance and felt on my belly. I had low iron so I had two blood transfusions. I ended up having an emergency c-section because her heartbeat was drastically dropping. 3. No 4. Crawling, between 6-9 months. Sitting I believe she was around 4 moths. Standing without support around 7 months. Walking when she was one year and one month. Not potty trained. She doesn't dress by herself yet. Sometimes she'll put her arm into the shirt or try to take off the shirt. 5. No sleeping or breathing issues. Doesn't snore. She doesn't fully wake up during the night but she starts moving a lot. I just give her a bottle of milk and go back to sleep. She mostly sleeps with her pacifier, not sure if that counts as mouth open or closed. When she doesn't sleep with the pacifier her mouth is closed. 6. No 7. She was referred to an audiologist and optometrist since she isn't talking 8. She doesn't talk. She picks up a few 3-4 letter words but when we try to make her talk she walks away from us. I would like for her to talk instead of just pointing to things.

Insurance only gave us a 2.5 month authorization when the authorization length is typically closer to 6 months. So her goals will remain relatively the same. Her performance on the standardized test did not change much since we just took the assessment at the end of July. So I will continue to request for more therapy time from insurance. Next week we will continue with speech therapy sessions as normal. :)

They're working on fixing it cause she can only have one but unfortunately her Medicaid coverage will start until December. I know that if more than 30 days passes by she will need to get reevaluated and send the paperwork to the insurance. We don't really want a gap since she just started. We were wondering if we decide to pay how much would it be for 30 minutes or the 1 hr? And when she has Medicaid hopefully we hear something back by then.

Let me know if you all would interested in this option until her Medicaid kicks back in. If not, I can ask the front office if they can hold off until the 30 days are up. She may not need to be re evaluated after those 30 days and can continue with her new plan of care that was submitted. I would have to confirm with the office though.

Just as a reminder next week is my last week at New Beginnings so if they have not cleared me to schedule her then she may not be scheduled again until the new therapist takes over the following week. Is this okay?
