Speech Background History: Haze is a 4 year 7 month old male who was referred for a speech and language evaluation by his pediatrician and mother’s concern with Haze’s speech production. Haze was accompanied to the evaluation by his mother who served as the informant. According to mother, Haze received developmental therapy and speech therapy from early childhood intervention, but discontinued when the family moved to El Paso. Since then, mother stated that Haze’s speech production is “not like other kids his age.” At this time, mother has stated that language is not a concern as Haze, “talks a lot” but is difficult to understand. Mother stated that Haze had suffered from several ear infections and PE tubes were placed around 2 years of age. At the time of PE tube placement, a right sided hearing loss was suspected. According to mother, the PE tubes fell out last year, but mother could not recall an exact date. During Haze’s last wellness check-up (Dec. 2024), a right sided hearing loss was suspected once again and the family was referred to an audiologist. Based on clinical observations, Haze’s current communication was characterized as short phrases that are unintelligible. A typically developing child that is Haze’s age should have approximately 500+ words in their expressive vocabulary and should be 75%-100% intelligible. The Clinical Assessment of Articulation and Phonology Second Edition (CAAP-2) was used to evaluate Haze’s speech production. Based on Haze’s reported language background and language input/output, the language of intervention will be English.
Speech Medical History: Haze was born at 39 weeks via emergency c-section due to umbilical cord strangulation, but no extended hospital stay was necessary. Haze has suffered from numerous ear infection, which led to PE tubes at 2 years of age. Mother stated that the tubes prevented any further ear infections, but the tubes fell out after a year. Since then, Haze has suffered from significate earwax build-up, especially in the right ear. Haze also suffers from seasonal allergies, but his allergies are not treated with medication.

Speech Assessment Summary: Haze is a 5 year, 2 month old male who was initially referred for a speech and language evaluation by his pediatrician and mother’s concern with Haze’s speech production. Haze is due for a re-evaluation to determine if continued speech therapy services are warranted. Haze’s articulation skills were re-evaluated using the Clinical Assessment of Articulation and Phonology - Second Edition (CAAP-2). The test was administered in English as it is the primary language spoken. Haze received the following scores on the CAAP-2: Consonant Inventory Standard Score of 58 (Raw Score: 32, Percentile Rank: 2, Age Equivalent: <2;6) and a School-Age Sentences Standard Score of 61 (Raw Score: 39, Percentile Rank: 1, Age Equivalent: <5;0). Haze’s scores from the CAAP-2 indicate that he presents with a PROFOUND ARTICULATION/PHONOLOGICAL PROCESS DISORDER. Based on information obtained through the evaluation, Haze would benefit from speech therapy services continuing to target increased independence with accurately producing the following in single words: /s/, /z/, /s/ blends, “ch” and “sh.” Additionally, information obtained through the recent re-evaluation indicates Haze would benefit from speech therapy services targeting increased independence with accurately producing the /r/ phoneme. An oral mechanism was also performed to assess Haze’s oral cavity structures and functions. Haze presents with oral cavity structures and functions observed to be adequate for speech sound production at this time. It is also to be noted that Haze was observed to have excess and possible impacted cerumen in the outer ear. Clinician shared this information with Haze’s mother and recommended that Haze’s mother follow up with Haze’s pediatrician. Based on results from the re-evaluation, Haze presents with a PROFOUND ARTICULATION/PHONOLOGICAL PROCESS DISORDER. Without continued speech therapy, Haze’s safety risk is high since he will likely not be understood if he needs to communicate pain or the need for assistance with safety personnel in the event he becomes separated from his caregivers. Haze’s mother will be responsible for his Home Education Program (HEP). The language of intervention will be English due to Haze’s language background, the language used in the home and school setting, the language the media Haze listens to, and the language of books read to Haze.

Haze Edwards 12/02/2020 2. c-section, had an emergency operation cause cord was wrapped around neck 3. none recently 4. crawl 6 months, sat up 5 months, stood up 7 months, walked 11 months, fed self 6 months, dressed self 2 years (still can't put shirt on), potty trained at 2 1/2 years, 1st words at around 2 years, combined words at 3 1/2 years 5. No sleeping issues, sleeps about 10 hours, sleeps with mouth open 6. Used to attend daycare/preschool hasn't been in either of those since march of this year. Has had speech and developmental therapy for about 6 months before he turned 2 years. 7. English 8. Yes hearing issues, has had ear tubes put in before. 9. I would like for him to speak clearly as other kids have made fun of him for not talking right or other peers not wanting to talk to him also.

The last time he had a hearing test was at his yearly checkup but there was no paperwork. They just informed me that his right ear had decreased.

07-25-2025 05:07 PM	Annette Barron	Good afternoon. This is Annette from New Beginnings. Here is a brief summary from yesterday’s evaluation. Haze has been diagnosed with profound phonological processing/articulation disorder. He would benefit from speech therapy targeting the following: understanding and using the right articulators (tongue, teeth, lips, etc.) to produce sounds correctly, discriminate of correct vs incorrect productions, sound substitution of “s” and “z” sounds, and weak syllable deletion in words. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the treating clinician will reach out to you to schedule your first session. I wanted to mention that telehealth services for speech is also an option for you, but of course if you prefer in-person, I will let the treating therapist know. If you have any questions, please reach out. Thank you.

When working on /s/ and /z/, you can support Haze when practicing the sounds by reminding him to make a big smile, keep his teeth closed, and his tongue trapped behind his teeth. You can also remind him to turn his voice on and make it buzz when practicing the /z/ sound. I will provide other recommendations for feedback to provide Haze as we continue working on the other sounds more in future sessions.

We started with a Halloween themed scavenger hunt around the room while we practiced productions of his “snake” sound in words. Then we used a pumpkin pail filled with index cards with different numbers on them practice producing “sh” in words the number of times on the index cards. We also completed some interactive iPad activities that allowed for repetitive practice of /s/ blends and the “ch” sound. I recommend trying the pumpkin pail activity again at home since he seemed to enjoy it and it will allow for high trial practice. Have a great rest of your week! :)

02-11-2026 05:05 PM	Kristin Oldknow	Good afternoon, it was great seeing Haze today for speech therapy and he did awesome with productions of his target speech sounds throughout today’s session! I sent home 3 different papers with different speech sounds on each. I outlined the pictures that have his target sounds in the initial position for him to continue to practice at home, but he can also try some of the other words for practice of his sounds in different parts of the word. I also sent him home with a paper bag filled with paper hearts with numbers on them where he can take one out of the bag and practice saying words that include his sounds the number of times on the heart to get high repetition practice. Additionally, I apologize about any confusion regarding scheduled visits and insurance coverage. Unfortunately, I am unable see insurance related information in the system for patients. I spoke further with the front office yesterday regarding Haze’s coverage and the front office is sharing the system is showing coverage through March 2nd. I passed along the information you shared regarding coverage until 2027. I recommend contacting the front office to see if they can help determine why it is not reflective in our system to ensure we can continue providing weekly therapy and prevent any gap in services. Lastly, Haze is due for his re-evaluation testing and I plan to conduct the testing next week. Please let me know if you have any questions or concerns. Thank you!

I also apologize if I came the wrong way, i had been on the phone with insurance that day to confirm and it was a whole mess so i was frustrated with that whole situation. Based on scores received on the standardized test administered, Haze presents with a profound articulation/phonological process disorder. Haze’s standard scores have increased since his initial evaluation, which is great news and indicating progress. I also assessed his oral structures and functions, and Haze presents with structures and functions adequate for speech sound purposes. I will be taking some time to analyze and interpret the re-evaluation results to develop an updated plan of care for his weekly speech therapy services.
