He demonstrated notable progress in receptive language comprehension. When targeting the ability to follow 1-step commands related to daily activities, he achieved 70% accuracy, exceeding the established goal of 60% (6 out of 10 opportunities). Joint attention skills were also a focus, with the patient successfully engaging in relational play with the clinician for 5-7 minute intervals in 75% of opportunities, surpassing the targeted 60% accuracy and promoting language learning opportunities. Progress was also observed in pragmatic skills, specifically turn-taking during play-based activities. He is approaching the goal of 80% accuracy, currently demonstrating turn-taking with 70% accuracy while requiring decreasing multisensory cues. In expressive language, the patient continued to develop his use of multi-word utterances. When targeting the use of 2-3 word core vocabulary phrases following a clinician model, he achieved 60% accuracy, demonstrating progress toward the 70% accuracy goal. Finally, speech intelligibility at the word level was addressed by targeting final consonant production in CVC words. The patient showed significant improvement, achieving 50% accuracy in structured tasks with minimal cueing, which exceeds the initial target of 30% accuracy.

Occupational Assessment Summary: Manuel was assessed via: PDMS-2 Standardized Assessment, Clinical Observations and per Parental Report PDMS-2 - Per standardized assessment scores Manuel falls well below average in both subsets assed on the PDMS-2 evident by below average age equivalency scores from 40 months compared to his chronological age of 54 months, placing him at least (-2 to -2) standard deviations behind his peers. Manuel demonstrates significant difficulty with self regulation skills highly likely due to sensory processing difficulties. Child currently demonstrates attention span approximately 1-2 minutes before attempting to move onto a different task. Child struggled to initiate and/or complete simple one step directions with max assistance via visual demonstrations, and verbal prompting for motivation, task initiation and redirection to task. Child exhibits limited attention evident by fleeing/escaping non-preferred activities, and need for constant redirection. Manuel demonstrates deficits in fine motor precision, manual dexterity, manual coordination, sensory processing and self-regulation, which have impaired his ability to complete activities of daily living and instrumental activities of daily living that compromise his routines and roles at home and in the community. Assessment of Goals/Response to Treatment: Manuel participated in structured therapeutic activities targeting attention to task, task initiation, bilateral coordination, and coping with stress. Manuel sustained attention to therapist-directed activities for approximately 3–5 minutes with mod verbal cues and redirection. Demonstrated improved engagement with visual supports and hands-on tasks. Manuel required moderate assistance , including verbal prompts and gestural cues, to initiate familiar tasks (beginning a puzzle or sorting activity). Delayed initiation observed without prompting. He engaged in activities requiring use of both hands (stringing beads, manipulating containers, simple crafts). Demonstrated ability to use both hands together with moderate assistance and verbal cues; occasional hand dominance noted with reduced use of the assisting hand. During mildly challenging tasks, Manuel showed signs of frustration ( withdrawing, vocalizations). With therapist support, he practiced simple coping strategies such as deep breathing, requesting help (with prompts), and short breaks. Required moderate verbal and visual cues to utilize strategies. Passed newborn hearing screening? 3.) Any major illnesses (i.e., ear infections) or accidents since last check up? Any medical diagnoses (i.e., Autism, ADHD, Down syndrome, etc.) or allergies (i.e., medications, foods, seasonal, etc.)? 4.) When were these developmental milestones met: Crawl, sat-up, stood up, walked, fed self, dressed self, potty trained, first words? 5.) Sleeping or breathing issues? Does patient get 8-10 hrs of sleep? Wake up several times a night? Snoring? Sleeps with mouth open or closed? Enlarged tonsils? 6.) Education history (i.e., attends daycare, preschool, etc.)? 7.) Vision or hearing issues? 8.) In a few words write your parental concerns with patient’s speech and/or language, and how you would like it to improve within a year.

No major illnesses, is in the process for a diagnosis (autism and adhd) 4.) Crawled around the 6 month mark, sat up around the 5/6 month mark, stood up around the 7 month mark, walked (unassisted) around the 14 month mark, fed self around the year mark, self dressed still isn't independent, potty training is still in progress but nearly complete, first words were said around the 9 month mark. 5.) No sleeping or breathing problems, sleeps 8-10 hours a night, Manuel snores lightly, sleeps with mouth open and closed, I think he has normal sized tonsils. 6.) He was in pre-k 3 in Korea for around 2.5-3 months before we moved back to the states. 7.) I don't believe he has any hearing or vision issues. 8.) Manuel doesn't pronounce the entirety of words and still has trouble with multiple words sentences, we believe the low quality of communication makes him frustrated and more prone to anger. We'd like to improve his pronunciation on words and give him more tools to help communication.

Manuel is diagnosed with a severe expressive-receptive language disorder. Manuel would greatly benefit from speech therapy services that focus on the following concepts: following 1step commands, engaging with clinician during play, turntaking, using gestures/word approximations to request/comment/label, using core vocabulary, and imitating developmentally appropriate phonemes.
