Occupational Background History: Journey is a 7.8 year old girl who was seen on this date for a scheduled occupational therapy evaluation in order to obtain current level of function. Mother was present in order to serve as the primary informant throughout the evaluation. Per mother’s report child was born at 38 weeks gestation via c-section vacuum assisted. Child did not experience any complications at birth, does have visual deficits and no hearing deficits. Child does not have history of aggression and occasional has seasonal allergies.

Child demo delayed response to sensory stimuli on feet, palms and cheeks; per parent report child orally seeks stimuli such as pieces of soft cotton like material which increases choking hazards and decreases her nutrition and safety awareness. Child bites nails and cracks knuckles continuously as a form of sensory input (proprioceptive and oral); per parent report child demos increased difficulty with change in routine and with anxiousness during sleep, which leads to increased seeking of comfort from mother. Journey demonstrates deficits in emotional regulation, sensory processing and self-regulation, which have impaired their ability to complete activities of daily living and instrumental activities of daily living that compromise their routines and roles at home and in the community. Assessment of Goals/Response to Treatment: Journey increased bilateral coordination skills during a writing activity in which she stabilized the paper with her "helper hand" while writing IND. She also engaged in gross motor exercises in which she demonstrated asymmetrical and symmetrical movements with UE and LE with only MIN cues for compensations. Journey attend to and independently completed a 4 step activity (Sorry) IND after initial instruction, to improve attention and task sequencing skills. Journey demonstrated increased emotional regulation skills when stating coping strategies to handle stressful situations during a "social skills" board game with only MIN verbal cues for assistance.

We are able to re-evaluate at 3 months and 6 months so I wanted to check in and see what your thoughts were :)

I’m open to doing the re-evaluation at the 3-month mark, but I do feel that Journey would benefit from continuing OT through the full 6-month period. I’d love to keep her progressing and building on the support she’s receiving. 😊

The only purpose to re-evaluate at the 3 month would be if you felt she no longer benefitted from OT services so we can wait until the 6 month mark :) My schedule is full but if I get cancellations I will let you know so we can complete make-ups so she doesn't fall behind on attendance. Thank you!
