Speech Background History: Kawann is a 2 year 10 month old male who was referred for a speech and language evaluation by his pediatrician and mother’s concern with Kawann’s speech production. Kawann was accompanied to the evaluation by his mother, who served as the informant. According to Mrs. Estelhomme, there is a familiar history of articulation issues as mother was in speech therapy her first 3 years of life. Mother stated that Kawann has developed the same “pronunciation issues” she had as a child with Haitian Creole influence. Per parent report, Kawann responds to questions directed at him and will ask/request items regularly at home, school, and daycare, but he “cuts off the ends of words.” Based on clinical observations, Kawann’s current communication was characterized as short phrases that were unintelligible. A typically developing child that is Kawann’s age should use 2-3 word utterances and be at least 50% intelligible. The Clinical Assessment of Articulation and Phonology Second Edition (CAAP-2) was used to evaluate Kawann’s speech production. Speech Assessment Summary: Kawann is now 3 years and 3 months and is due for a review of his speech production since beginning speech therapy services 6 months ago. According to mother, Kawann has improved his speech production skills, but has continued to be difficult to understand, especially with unfamiliar communication partners. Kawann’s current communication was characterized as short 3-4 word phrases that were unintelligible, and reminders to slow his rate of speech for others to understand him. A typically developing child that is Kawann’s age should have approximately 400+ words in their expressive vocabulary and should be 75%- 100% intelligible. Kawann was initially evaluated using the Clinical Assessment of Articulation and Phonological Second Edition (CAAP-2) to measure his speech production on phonemes in words, and it will continue to serve as the tool of assessment to note his progress since beginning speech therapy. Kawann required several repetitions and/or redirection to attend to the assessment. Kawann received the following score on the CAAP-2: Consonant Inventory Score 88 (raw score/errors: 34, percentile: 17, age-equivalent: <2 years 6 months) with 27 less word level errors as compared to his initial assessment. Based on this standard score, Kawann’s articulation skills are within functional limits for his age, and presented with a MILD ARTICULATION/PHONOLOGICAL DISORDER. Although Kawann has made improvements, he continues to have difficulty with the following phonological processes: final consonant deletion, fronting, stopping, consonant cluster reduction, weak syllable deletion, and deaffrication. Kawann was observed to have struggle with the following speech sounds: /k, g, f, v, s, z, sh, ch/ in all word positions. Continued speech therapy is warranted to continue improvement with his production of phonemes in words and in Kawann’s conversation. Without speech therapy services to address his needs, Kawann’s safety and functional health risks are high, as well as his independence, in that others will not understand him if he needed to ask for help or assistance in his daily routine or environment. Kawann’s mother will be responsible for his Home Management Intervention Plan (HMIP).

The clinician attempted to offer another opportunity for a make-up session this week and offered Friday, at 9 AM or 11 AM. However, upon sending out the offer, the clinician received the following message: Kawann Estelhomme 08-18-2025 01:00 PM Kawann (Prim) Replied: Actually I would need to reschedule this week. My mom has a doctors appointment and I couldn't take off Once again, the clinical probed parent to see if the offer for one of the Friday appointments would work for the family. The clinician is awaiting a response form the parent.

We focused on words that end with "t" Jet, hot, fast, and Kawann took home a paper airplane using the word jet to practice including the "t" at the end of the word. We behind on visits, so I can offer two sessions for you all next week. However, grandma told me about the passing of a family member. Sorry to hear that. However, mother only agreed to the Wednesday appointment. The additional spot was held for Kawann should his mother had changed her mind about only accepting the one appointment for the week. Based on the score I obtained, Kawann is now up to age limit with his speech production as compared to other children his age. There are still some errors I hear, but his intelligibility has increased a lot. Although his score is now within normal limits, I can attempt to request more time to continue to work on the problematic sounds you still hear, but I wanted to ask about your thoughts of continuing speech therapy services.
