Speech Background History: Natalie is a 6 year 8 month old female that came into the offices with her parents. Mother and Father came into the evaluation room with Natalie with some prompts to enter the "speech" room. With prompting from mother and father, Natalie sat down at the table but quickly got up to look for something to play with, she did not respond to therapist or play with play dough on the table. She was comfortable with magnetic doll that one could dress like a princess. While therapist and parents spoke, Natalie dressed the doll many times while expressing scripted phrases and sentences. Once interview was completed, therapist attempted to establish rapport with Natalie. She did not respond to comments, requests, or labels. Natalie did put items back that therapist moved, or take from the therapist any item that she picked up. No direct interaction was noted or written down.

Speech Assessment Summary: Natalie is a 7 year 1 month old female that has been receiving speech and language therapy at New Beginnings Pediatric Therapy services and was re-evaluated to determine need for continued services. Since previous evaluation, Natalie has begun receiving ABA services. During today’s re-evaluation, the patient demonstrated limited willingness to participate independently. The patient’s parent was required to remain in the treatment room due to previous patient refusal to participate in activities. Multiple breaks were provided throughout the session secondary to escalating distress behaviors. The patient presented with intermittent crying and attempts to hit clinician during transitions and task demands. Both parent and clinician provided frequent redirections to support safety and engagement. A visual schedule was implemented to facilitate predictability; however, it was partially effective as the patient continued to demonstrate difficulty following sequence of activities. On the Clinical Evaluation of Language Fundamentals (CELF-5), Natalie obtained the following scores: Sentence comprehension: raw score: 12, scaled score: 3, percentile rank: 1, age equivalent: 4:6; Word structure: raw score: 10, scaled score: 2, percentile rank: 0.4, age equivalent: 3:3; Formulated sentences: raw score: 6, scaled score: 3, percentile rank: 1, age equivalent: 4:10; Recalling Sentences: raw score: 17, scaled score: 5, percentile rank: 5, age equivalent: 4:9. The sum of previous subtest scaled scores resulted in a sum of 7 which resulted in a Core Language standard score of 61. Natalie’s score on a standard deviation is at least 2.5 Standard deviations below the mean. Natalie presents with a SEVERE RECEPTIVE/EXPRESSIVE LANGUAGE DISORDER. The following scores reported should be viewed with caution due to behavioral regulation challenges as well as the CELF-5 is not being geared toward children that are on the spectrum with Autism. Since her last re-evaluation, Natalie has made measurable outcomes in her receptive and expressive language skills, as evidenced by meeting most of her targeted therapy goals and showing increased performance on standardized assessment. Natalie has also demonstrated improvements in transition to therapy room as well as making progress in using scripted words to express/protest likes and dislikes. However, Natalie’s language skills are still not functional for his age. Speech therapy continues to be a medical necessity due to Natalie’s high safety risk. Speech therapy warranted to promote functional communication which will promote safety and independence. Language of intervention should remain in English. Parents will remain responsible for implementing the Home Management Intervention Plan (HMIP).

I wanted to reach out regarding those developmental questions I had mentioned earlier. When you have a few minutes you can just number and respond here on the system. I see on the referral that Natalie is diagnosed with Autism and she is hyperlexic. 1. What is your main concern for having the initial evaluation for Natalie? Where do you see her and her language development six months from now and possibly a year? 2. When did you first notice she was have problems with developing her language? 3. Does anyone in the family have a history of receiving speech/language therapy? 4. Are there siblings in the family? How does family interact with Natalie and vice-versa? Is more than one language spoken in the home? 5. How was Pregnancy and Delivery? Any hospital stay? 6. Any major illnesses or accidents? When was her last wellness check? 7. How were her Developmental Milestones? Sitting up? Crawling? Walking? any delays. 8. Does she attend school? 9. Any concerns with vision or hearing? Did she pass her New Born Hearing Screening? Lots of scripting, echolalia, gestalts. she's incredibly bright and loving and we'd love her to be able to communicate with others better and develop friendships. 2. We noticed late, like age 3, because she doesn't have any siblings or cousins or anyone really around her age that we could compare her to. Also, she was that age during Covid quarantine so it was just the three of us at home together for so long. 3. No one in our family receives speech therapy. 4. No siblings. She interacts with love and cuddles with mommy and daddy, grandma, papa, and her Gigi (another grandma). She speaks to all of us and speaks even more at school and at daycare. We all communicate in English. She is starting to experiment with Spanish for fun. 5.

All of those milestones were met normally. 8. Natalie attends North loop elementary school. She just finished kindergarten in an SSC unit class (autism) 9. No hearing or vision issue. She passed the newborn hearing test on her second try. She was crying and screaming through the first one and the nurse gave up.
