Speech Assessment Summary: Jett is a 3 year 0 month old male who was referred to New Beginnings in May 2025 by his pediatrician for a speech and language evaluation due to concerns regarding a speech delay. Jett has a history of receiving speech services through ECI. Jett is due for a re evaluation to determine if continued services are warranted. Jett currently attends daycare. According to his father, Jett has shown improvement in his ability to use words to express his wants and needs; however, he continues to rely on gestures about half of the time. During the re evaluation, Jett presented as a cooperative child with adequate attending skills, clear intent to communicate, and appropriate awareness of others and his environment. His social interaction style appeared somewhat shy, and he demonstrated a mildly delayed response rate. His overall activity level was within normal limits, and he required only minimal prompting for redirection to tasks. Jett primarily communicated using eye contact, word approximations, and one to two word phrases to express himself. An oral mechanism examination revealed a shallow, wide palate, deviation of the uvula to the right side, and possibly enlarged tonsils. Dentition appeared adequate, and lingual mobility was within functional limits. Diadochokinetic (DDK) tasks were unable to be completed due to Jett’s limited speech sound inventory. Per parent report, Jett was evaluated by an ENT at two years of age, and results at that time were within normal limits. He has a history of ear infections but has not experienced one recently. A hypernasal vocal quality was also noted during the evaluation, which may be associated with velopharyngeal insufficiency (VPI) or velopharyngeal mislearning. Referral to a craniofacial specialist is recommended to assess for potential VPI, and an ENT follow up may also be warranted to determine if enlarged tonsils or other structural factors are contributing to Jett’s speech and resonance characteristics. The Preschool Language Scales, Fifth Edition (PLS-5) was administered to re assess Jett’s receptive and expressive language skills. Jett obtained the following standard scores: Auditory Comprehension 94 (34th percentile, age equivalent: 3 years 0 months), Expressive Communication 72 (3rd percentile, age equivalent: 1 year 9 months), and Total Language Score 82 (12th percentile, age equivalent: 2 years 5 months). Scores between 85 and 115 are considered within normal limits. These results indicate that Jett presents with a MODERATE EXPRESSIVE LANGUAGE DISORDER. A speech sound screening was completed using the PLS 5 Articulation Screener; a full articulation battery was not administered due to Jett’s limited expressive vocabulary. Current results on the screener indicate the need for further evaluation at Jett’s next re evaluation . Jett does not yet consistently produce or use the early developing phonemes /p, b, k, g/ and exhibits the phonological process of final consonant deletion. Nasal substitutions for other consonant sounds were also frequently observed. Speech therapy should target the production of /p, b, k, g/ at the isolation and syllable level to support intelligibility for functional communication. At Jett’s age, a typically developing child is expected to use and understand at least 50 words, combine words into two to three word phrases, and begin producing three to four word utterances. Since beginning speech therapy, Jett has made measurable progress, meeting all goals in his previous authorization period, improving by one point on his raw score, and demonstrating parent reported gains in communication. Despite these improvements, Jett’s current speech and language skills remain below developmental expectations for his age and are not yet functional for independent and safe communication. Without continued speech therapy, Jett is at increased risk for communication breakdowns that may affect his ability to express pain, request help, or follow directions in safety related situations. Continued speech therapy is highly recommended to address Jett’s expressive language delay and speech sound development. Therapy should focus on increasing verbal communication over gestures, imitation and spontaneous use of two to three word phrases, expanding expressive vocabulary, and producing target phonemes /p, b, k, g/ to improve intelligibility. Jett’s parents will be responsible for implementing his Home Management Intervention Plan (HMIP), which includes encouraging word use, modeling two word combinations, and reinforcing therapy strategies at home. Based on Jett’s language exposure and use, English will be the primary language of intervention. In summary, Jett presents with a moderate expressive language disorder (F80.1), characterized by reduced expressive vocabulary, limited phrase use, and speech sound production delays impacting intelligibility. With ongoing intervention, Jett is expected to continue making meaningful progress toward age appropriate communication skills.

Unfortunately, due to insurance guidelines, services at two different locations cannot overlap—even during the evaluation phase. We understand how difficult this situation can be, especially when continuity of care is important. While we cannot guarantee approval for ongoing therapy until after the evaluation, we do our best to ensure each case is thoroughly assessed and that families are supported throughout the process. He tries moving his l lips but he can not communicate outward and gets frustrated.

Jett would benefit from speech therapy targeting the following: imitation of gestures/actions, imitation of speech sounds, naming/labeling via word approximations, and use of total communication for a variety of pragmatic functions. Once the office has processed the report, they obtain the needed signatures, and insurance approves the report, the assigned treating therapist will reach out to you to discuss beginning treatment.
